2.Tuberculous Uveitis in a Hemodialysis Patient.

Tuberculous uveitis is a rare manifestation of tuberculosis infection. Although early diagnosis is important to saving the sight of patients, it is difficult. In dialysis patient, the diagnosis of tuberculosis is often delayed because of non-specific symptoms and extrapulmonary involvements. We report a case involving a 37-year-old man receiving hemodialysis who was presented with persistent fever and left- sided blurred vision. At ophthalmic examination, active papillitis, vitritis, and peripapillary retinal detachment were observed. Neck computed tomography showed multiple necrotic lymphadenopathies (LAPs) in right lateral neck, which was proven as tuberculous lymphadenitis on histological examination. After anti-tuberculosis therapy, fever was subsided and LAPs were improved. Visual acuity was also improved.

3.Focal Atrial Tachycardia Arising from the Inferior Vena Cava.

The inferior vena cava (IVC) is a rare site of focal atrial tachycardia (AT). Here, we report a 20-year-old woman who underwent catheter ablation for anti-arrhythmic drug-resistant AT originating from the IVC. She had undergone open-heart surgery for patch closure of an atrial septal defect 17 years previously and permanent pacemaker implantation for sinus node dysfunction 6 years previously. The AT focus was at the anterolateral aspect of the IVC-right atrial junction, and it was successfully ablated under three-dimensional electroanatomical-mapping guidance. We suspect that the mechanism of this tachycardia was associated with previous IVC cannulation for open-heart surgery.

4.Laser and Light Sources in the Treatment of Onychomycosis.

Onychomycosis is a common fungal infection of the nails and there is wide distribution across the continent. Aside from Trichophyton rubrum (88%) being the main causative agent, other dermatophytes, yeasts or non-dermatophyte molds can affect nails and adjacent tissues and cause sundry symptoms: subungual hyperkeratosis, onycholysis, nail thickening, discoloration, malodorous condition. Oral antifungal agents are the mainstay treatment for onychomycosis but undesirable side effects, such as hepatotoxicity, remain a main reason for drug withdrawal. Moreover, other modalities as a reliable treatment option are disappointing with a low cure rate. The advancement in laser and light technology has opened a new field of opportunity for the treatment of refractory skin disease. As supported in clinical studies, neodymium-doped yttrium aluminium garnet laser, diode laser, fractional carbon dioxide laser, and erbium:glass laser have produced a favorable outcome, with or without additional agents, for the treatment of onychomycosis. But the mechanism is still unclear and reliable data are relatively scarce despite the potential as an alternative armamentarium. To date, as with laser therapy, there have been only a few studies conducted concerning the role of light therapy in the superficial fungal infection. Herein we aimed to review the medical literature on laser and light therapy of onychomycosis and provide our point of view for health providers and researchers.

8.Presumed Intraocular Natural Killer/T-cell Lymphoma Combined with Nasal Lymphoma.

PURPOSE: To report a case of presumed intraocular natural killer/T-cell lymphoma (NKTL). CASE SUMMARY: A 61-year-old male presented with visual disturbance of the left eye for duration of 3 years. He had been treated with systemic chemotherapy and radiotherapy for nasal NKTL 1 year prior. Inflammatory reaction in the anterior chamber and vitreous opacity were observed in the left eye. The patient was diagnosed with uveitis in the left eye at a local clinic 3 years prior to visiting us. Because the patient did not respond to anti-inflammatory therapy, we performed diagnostic and therapeutic vitrectomy. Intraoperatively, vitreous opacity was thin sheet like in appearance. The vitreous specimen contained few lymphoid cells and was positive for Epstein Barr virus-encoded RNA (EBER). Systemic workups showed no metastasis to other organs. The patient was treated with systemic methotrexate chemotherapy and intravitreal methotrexate injected 3 times (once per week). During the 12-month follow-up period after the last intravitreal injection, the recurrence of lymphoma and related uveitis was not observed.

9.A Case of Generalized Eruptive Syringoma.

Syringoma is a common, benign, adnexal tumor of eccrine origin. Generalized eruptive syringoma, however, is a rare variant of syringoma which presents as a widespread papular eruption. It occurs in large numbers on the anterior parts of the neck, chest, abdomen, axillae, periumbilical region, or sometimes proximally on the extremities. It is as frequent in prepubertal as in postpubertal age. A 20-year-old woman presented with multiple asymptomatic papules on the face, neck, abdomen, back, upper and lower extremities. Family history showed that her mother had similar lesions on the abdomen. Histologically, numerous ducts are embedded in a fibrous stroma and their walls are lined by two rows of epithelial cells. We report a case of generalized eruptive syringoma.

10.A Case of Intravascular Papillary Endothelial Hyperplasia on the Finger.

Intravascular papillary endothelial hyperplasia (IPEH) is a rare benign vascular tumor. It represents a reactive proliferation of endothelial cells arising in an organized thrombus. And it is histologically characterized by the development of endothelial-lined papillary projections in a vascular lumen. It occurs most frequently in the head and neck region or on the extremities. We report a case of intravascular papillary endothelial hyperplasia developed on the finger.
