1.Two Cases of Yersinia Pseudotuberculosis Infection with Acute Renal Failure in Pusan Province.

The usefulness of E-cadherin immunostaining as a marker of malignancy in the body fluids was investigated in the present study. Thirty-three histologically proven cases of cell blocks from the pleural, peritoneal, and pericardial fluids were studied by immunocytochemistry for E-cadherin antibody using LSAB method. These cases were cytologically diagnosed as adenocarcinoma (25 cases) and atypical cells (8 cases). Tumor cells showed strong positive membranous staining for E-cadherin antibody in 21 out of 25 cases (84%) of adenocarcinoma. E-cadherin staining was not found in 6 of 8 cases of suspicious maligancy. The sensitivity and specificity were 84% and 75%, respectively. Reactive mesothelial cells and inflammatory cells scattered were all negative. In conclusion, E-cadherin is an useful adjunctive marker to distinguish reactive mesothelial cells from the carcinoma cells in the body fluids.

The pattern of thickening of the extrahepatic bile duct on computed tomography was analysed in 30 cases with recurrent pyogenic cholangitis diagnosed by surgery (n=19) or clinical basis (n=11). The mean wall thickness of the extrahepatic bile duct was 3.3mm (range, 1-6.3mm). Diffuse thickening of the extrahepatic bile ductal wall was demonstrated in 26 of 30 cases. Diffuse thickening of the extrahepatic bile duct in recurrent pyogenic cholangitis may be differentiated from focal thickening of duct in a common duct cancer or pancreatic cancer.

4.Too many ducts sign: a characteristic cholangiographic finding of clonorchiasis?.

Clonorchiasis procucts diffuse dilatation of the small and medium sized intrahepatic bile ducts and its cholangiogram shows visualization of many bile ducts, especially, tertiary, quaternary, and more peripheral tributaries up to the 6th tributaries. In an attempt to clarify this cholangiographic sign quantitively, we counted the visualized smaller bile ducts in clonorchiasis and compared the number of visualized ducts in normal cholangiogram, recurrent pyogenic chlangitis and carcinoma of the extrahepatic ducts. In clonorchiasis the number of visualized smaller bile ducts was considerably geater than in normal subjects and recurrent pyogenic cholangitis, but there was no singnificant statistical differences in the number of visualized bile duct tributaries between clonorchiasis and carcinoma of the bile ducts. Thus it is considered that too many ducts sign is not a unique cholangiographic finding of clonorchiasis, but we believe that in the presence of this sign with other we l known cholangiographic findings, diagnosis of clonorchiasis is very easy.

5.Cystic Meningioma: A case report.

Meningiomas are usually thought of as firm solid tumors and most standard references make no mention of cystic meningiomas. Although several cases of cystic meningioma have been reported in the literature and their neuroradiological features discussed, the rarity of this entity makes its preoperative diagnosis difficult. Recently, the authors encountered a case of cystic meningioma, which was thought as ependymal cyst or infarction, preoperatively. In this report the authors discussed its clinical, neuroradiological and pathological characteristics with brief reviews of the literature.

8.The Relationship between Low Intrinsic Motivation and Basic Psychological Need in Internet Gaming Disorder.

Due to the increasing use of the internet, various social problems have been emerging, and one of them is internet gaming disorder (IGD). The purpose of this study was to investigate whether low internal motivation was present in IGD and basic psychological needs affected internal motivation. Twenty-one individuals with IGD and 21 healthy controls performed a ‘motivation task’ in which the avatar in virtual reality asked whether they would accept an activity in two conditions stimulating intrinsic or extrinsic motivation. The task performance was compared between the two groups, and was correlated with the Basic Psychological Needs Scale scores. Patients had a significantly lower acceptance rate of the proposal in the intrinsic motivation condition than controls, and this intrinsic motivated tendency was correlated with the satisfaction in their basic psychological needs. As with other substance use disorders, IGD was associated with a lower level of internal motivation, which was related to the dissatisfaction of basic psychological needs such as autonomy, competence, and relationship.

9.The Usefullness of the Adenosine Deaminase Activity for the Diagnosis of the Scrub typhus.

The mainstay of diagnosing scrub typhus is through serologic testing. However, because of the delay of several days from the onset of the illness to the increase in the antibody titer, diagnosis can be difficult in the early stage of the illness. The aim of this study was to determine the clinical usefulness of adenosine deaminase activity (ADA) in diagnosing scrub typhus. A total of 104 patients were enrolled during the study period. Of these patients, 59 patients were diagnosed with scrub typhus and the mean serum ADA level was 46.7+/-11.6 U/L. High serum ADA levels strongly supported the diagnosis of scrub typhus, especially in patient without eschar. The remaining 44 patients did not have scrub typhus and 95% (42/44) of them had normal serum ADA levels. Therefore, evaluating serum ADA level could help diagnose patients with acute febrile disease such as scrub typhus, especially in its early stage of disease progression.

10.Splenic Infarction Associated with Brucellosis in a Non-Endemic Area.

A 45-year-old man was referred from a local clinic with persistent fever, intermittent pain in the left upper abdomen, and weight loss of 7 kg. He quit his animal husbandry 18 months ago when his cows were found to be infected with Brucella. Abdominal computed tomography (CT) scan taken on admission showed splenomegaly with a wedge-shaped hypoattenuating region in the enhanced image, which was consistent with splenic infarction. Serology for Bruculla was strongly positive with the standard tube agglutination test (1/2560). After initiation of doxycycline (100 mg every 12 hrs) and rifampin (600 mg every day), the patient's condition improved, and was discharged with oral antibiotics that were to be continued for 3 months. During the 12 months' follow up at the outpatient department, the patient had no symptoms, and the last agglutination titer for Brucella in serum had decreased to 1/40. To our knowledge, this is the first report on splenic infarction associated with brucellosis in Korea, which was treated successfully with antibiotic therapy.
