2.Characteristics and Surgical Outcomes of Intertrochanteric or Subtrochanteric Fractures Associated with Ipsilateral Femoral Shaft Fractures Treated with Closed Intramedullary Nailing: A Review of 31 Consecutive Cases over Four Years at a Single Institution

PURPOSE: To evaluate the clinical characteristics of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures and assess the surgical outcomes of a novel, closed intramedullary nailing surgical approach designed to minimize fixation failure. MATERIALS AND METHODS: Between May 2013 and April 2017, 31 patients with intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures treated with closed intramedullary nailing or long proximal femoral nail antirotation (PFNA) were enrolled in this study. Preoperative data included age, sex, injury severity score, body mass index, location of shaft fracture, injury mechanism, accompanying traumatic injury, walking ability before injury, and surgical timing. Perioperative outcomes, including follow-up period, types of intramedullary nails, number of blocking screws used, operation time, and blood loss were assessed. Radiologic outcomes, including union rate, time from surgery to union, and femoral shortening, and clinical outcomes, including hip flexion, walking ability, and Harris hip score were also evaluated. RESULTS: A total of 29 unions (93.5%) were achieved. The time to union was 16.8 months (range, 11–25 months) for hip fractures (15.7 weeks for intertrochanteric fractures and 21.7 weeks for subtrochanteric fractures) and 22.8 months for femoral shaft fractures. There were no significant differences in surgical outcomes between the two groups except for type of intramedullary nail. CONCLUSION: Closed intramedullary nailing in the treatment of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures may be a good surgical option. However, fixation of femoral shaft fractures might not be sufficient depending on the implant design.

4.Anterior impingement of the Ankle Treated by Arthroscopic Removal of bony Spur

From September 1992 to April 1994, we reviewed 7 patients after arthroscopic surgery for anterior impingement in the ankle, who complained ankle pain and limitation of motion. All patients were mem and main symptoms were severe pain at full dorsiflexion of ankle and limitation of motion. Bony spur was located on the anterior aspect of tibia, and it was possible to resect the tibial bony spur arthroscopically without distraction devices. Mean operation time was 55 minutes. Pain, swelling and limitation of motion were much improved. There was no postoperative complication. Arthroscopic resection of the anterior tibial bony spur is an effective treatment for anterior impingement in the ankle.

STUDY DESIGN: Cross-cultural adaptation of the Korean version of the Oswestry Disability Index (KODI). OBJECTIVES: To conduct the cross-cultural adaptation for translating the original version of Oswestry Disability Index (ODI) (English version) SUMMARY OF LITERATURE REVIEW: Society is multicultural populations income. (This makes no sense?) Since languages and cultures are highly related, any cross-cultural assessment must be addressed in the outcome of the measurements. (I have made this into a proper sentence, but the meaning is still unclear, please confirm?) The Oswestry Disability Index (ODI) has not been translated into Korean, but is the most widely used tool in the world for measuring the intensity of back pain. MATERIAL AND METHOD: We used the guideline for the translation and cultural adaptation process, (Where were these guidelines from?) including forward translation, synthesis of translation, backward translation, committee review, pre-testing, test of the pro-final version, and finally, submission of the documentation to the developers. The Korean version of the Oswestry Disability Index (KODI) was tested on 116 patients with chronic low back pain. RESULTS: The reliability for the KODI, as measured by the Cronbach's alpha value, was 0.9168, and the reliability of the testretest was 0.9331. The process results were reported to the developer (Dr. Jeremy Fairbank), who appraised the KODI. CONCLUSIONS: The KODI was found to have satisfactory reliability. Therefore, the KODI can be recommended as a tool for the measurement of lower back pain in Korea.

9.Expression of Antioxidant Enzymes(Catalase, Superoxide Dismutase, and Glutathione Peroxidase) in Human Bladder Cancer.

PURPOSE: Intrinsic antioxidant enzymes(AOE) are essential for protection against potential cellular damage by reactive oxygen species(ROS), which affect many biological processes including carcinogenesis. The aim of the present study was to characterize the expression of antioxidant enzymes in human bladder cancer tissue and to evaluate the relationship with histopathological characteristics. MATERIALS AND METHODS: Immunohistochemical stainingfor catalase, superoxide dismutase(SOD), and glutathione peroxidase(GPx) was performed on formalin-fixed, paraffin-embedded tissues obtained from 75 bladder cancersand 30 normal bladders. The degree of AOE expression was compared with cancer invasiveness or the cell grade. RESULTS: The expression of catalase and SOD were significantly lower in cancer tissue than in normal bladder tissue(44% vs 73.3%, p=0.012; 49.3% vs 80%, p=0.007, respectively) but GPx expression was not significantly different(45.3% vs 63.3%, p=0.146). Catalase and SOD expression were significantly lower in invasive transitional cell carcinomas than in superficial transitional cell carcinomas(32.4% vs 53.7%, p=0.034; 32.4% vs 63.4%, p=0.014, respectively), but again GPx expression was not significantly different(38.2% vs 51.2%, p=0.26). Moreover, no significant difference was observed between the expression of all three enzymes and the cancer cell grade. CONCLUSIONS: Down-regulation of the antioxidant enzyme system, as indicated by the expression of catalase and SOD, appears to be related with carcinogenesis and progression in bladder cancer.

10.Effect of beta-adrenergics on the Survival and Production of Nitric Oxide in the Cultured Trabecular Meshwork Cells.

PURPOSE: To investigate the effect of beta-adrenergics on the production of nitric oxide (NO) in cultured trabecular meshwork cells. METHODS: Primarily cultured porcine trabecular meshwork cells were exposed to timolol and to propranolol at various concentrations. Cellular survival was assessed by MTT assay and the production of nitrite was assessed by Griess reaction after 24 hours and 3 days respectively. Also investigated was the effect of co-administraton of timolol and isoproterenol. RESULTS: After exposure for 24 hours, neither drug affected the cellular survival. Timolol and propranolol inhibited the production of NO significantly (p<0.05). Isoproterenol abolished timolol-induced inhibition of NO production after 24 hours. These results were similar after exposure for 3 days. CONCLUSIONS: Both timolol and propranolol inhibit the production of NO in trabecular meshwork cells and isoproterenol abolished this effect. These results suggest that beta-adrenerics involves in the production of NO in trabecular meshwork cells.
