3.A Case of kerion Celsi in An adult Caused by Trichophyton rubrum.

Kerion celsi caused by Trichophyton(T.) rubrum is rare endemically and there has been no report of it in Korean literatures. We report a case of karion celsi caused by T. rubrum in a 71-year-old male, who showed anindurated, swollen, boggy mass exuding pus on the right parietal scalp for about 7 weeks. The colonies and microconidia were identified as T. rubrum by fungus culture in potato dextrose agar. After administration of itraconazole for 11 weeks, the lesion was healed and the regrowing of new hairs was observed.

4.Characteristics and Surgical Outcomes of Intertrochanteric or Subtrochanteric Fractures Associated with Ipsilateral Femoral Shaft Fractures Treated with Closed Intramedullary Nailing: A Review of 31 Consecutive Cases over Four Years at a Single Institution

PURPOSE: To evaluate the clinical characteristics of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures and assess the surgical outcomes of a novel, closed intramedullary nailing surgical approach designed to minimize fixation failure. MATERIALS AND METHODS: Between May 2013 and April 2017, 31 patients with intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures treated with closed intramedullary nailing or long proximal femoral nail antirotation (PFNA) were enrolled in this study. Preoperative data included age, sex, injury severity score, body mass index, location of shaft fracture, injury mechanism, accompanying traumatic injury, walking ability before injury, and surgical timing. Perioperative outcomes, including follow-up period, types of intramedullary nails, number of blocking screws used, operation time, and blood loss were assessed. Radiologic outcomes, including union rate, time from surgery to union, and femoral shortening, and clinical outcomes, including hip flexion, walking ability, and Harris hip score were also evaluated. RESULTS: A total of 29 unions (93.5%) were achieved. The time to union was 16.8 months (range, 11–25 months) for hip fractures (15.7 weeks for intertrochanteric fractures and 21.7 weeks for subtrochanteric fractures) and 22.8 months for femoral shaft fractures. There were no significant differences in surgical outcomes between the two groups except for type of intramedullary nail. CONCLUSION: Closed intramedullary nailing in the treatment of intertrochanteric or subtrochanteric fractures associated with ipsilateral femoral shaft fractures may be a good surgical option. However, fixation of femoral shaft fractures might not be sufficient depending on the implant design.

7.Factors Affecting the Morbidity after Transurethral Prostatectomy for Benign Prostatic Hyperplasia.

Medical records of 570 patients underwent transurethral prostatectomy from 1985 to 1991 were reviewed to analyze the factors affected morbidity after transurethral prostatectomy retrospectively. Transurethral prostatectomy constituted 16.9% of the urologic operation during above periods and there were benign disease in 513 patients(90%), and carcinoma in 57 patients(10%). Age of the patients ranged from 48 to 93 years old with a mean age of 68.3. Of the patients, 298 patients (52.3%) had significant pre-existing medical problems. The resected net tissue weight ranged from 3 to 73 grams, with a mean of 11.8 grams. The mean operation time was 53 minutes with a range from 25 minutes to 126 minutes. The mortality was 0.36% and the operative morbidity was 20.3%. Increased morbidity was found in patients with a resection time of more than 90 minutes, used glycine of more than 20,000ml and resected weight of more than 31 grams (P<0.05) But age greater than 80 years, azotemia, urinary tract infection, serum Na+ change and pre-existing disease did not increased the operative morbidity. Voiding symptom was improved in 551 patients(96.7%) but in 19 patients(3.3%). persistent voiding difficulty was found. postoperatively at three months The causes of voiding difficulty were as follows ; 10 incontinence patients, 8 detrusor areflexia patients and 1 urethral stricture patient. In conclusion, transurethral prostatectomy is one of the most common operations today with a very low mortality and the overall postoperative results are excellent. Further studies, such as a randomized trial, will be necessary to evaluate the long-term outcome of transurethral prostatectomy further.

8.A Case of Blue Rubber Bleb Nevus Syndrome.

The blue rubber bleb nevus syndrome(CRBNS) is a rare disorder characterized by distinctive vascular malformations of the skin and gastrointestinal tract. The syndrome is associated commonly with iron deficiency anemia due to gastrointestinal bleeding. Most cases are sporadic, although a few have been reported with autosomal dominant inheritance. We report a case with clinical characteristics of the blue rubber bleb nevus syndrome. The patient had typical skin manifestations of the BRBNS and chronic iron deficiency anemia. However, we could not find any gastrointestinal vascular malformations radiologically.

9.Prognostic Significance of Deoxyribonucleic acid in Stage T1 Bladder Carcinoma.

Flow cytometric nuclear deoxyribonucleic acid (DNA) ploidy analysis was done successfully on 35 specimens of stage T1 bladder carcinoma treated with Bacillus Calmette-Guerin(BCG) between June 1986 and June 1993. The histologic tumor grading was done by the same pathologist. Of the specimens 16(46%) were DNA diploidy, 14(40%) were aneuploidy and 5(14%) were tetraploidy. In the relationship between tumor grade and ploidy, diploidy was seen in all (100%) of 5 patients with grade 1, in 13(48%) of 27 patients with grade 2. On the other hand, non-diploidy in 14(52%) of 27 patients with grade 2, and in all(100%) of 5 patients with grade 3. Mean followup period was 29 months(12-97 months). None of the patients died of tumor during the followup period. None(0%) of 16 patients with diploidy had tumor recurrence, in contrast 8(42%) of 19 patients with non-diploidy tumors recurred(p<0.05). None of the 16 patients with diploidy had progression. but 5(36%) in 14 patients with aneuploidy (5 patients(26%) in 19 patients with non-diploidy) progressed to an advanced stage(p<0.05). The DNA ploidy analysis appears to significantly contribute in the elucidation of possible future recurrence and progression in patients with stage T1 bladder carcinoma.

10.Clinical and Mycologic Studies of Dermatophytosis in Chungchong Province.

OBJECTIVE: The study was made in order to inverstigate the clanical and mycological status of dermatophytosis in Chungchong province, incidence and friquency of disease, sex and month distribution of the disease, varieties of the causative organism. METHODS: We performed clinical and mycological studies of 86 cases of dermatophytosis which have showed positive on KOH examination and/or positved result in culture among out- patients of Dermatologic Clinics of Chungnam National Univesy during 7 years, from 1986 to 1993. RESULTS & CONCLUSION: 1. The ratio of male to female patients 330: 256 and make little ifference. 2. The incidence rate was bighest in the third decade(25.4%) 3. The seasonal prevalence of dermatophytosis was highest in the summer 4. The incidence of dermaiophytosis was T. pedis(30.2%), T. (22.9%), T. ungium(13.1%), T. corPoris(10.1%), T. capitis(9.0%) in decreasing order of freqercy 5. A mean positive rate for the KOH mount examination A. 5. 83.8%, T. 90.2%, T. corporis 89.8%, T. ungium 86.5% in decreasing order of frequency 6. A mean positive rate of culture was 77.1%; T. capities 94.3, . faciale 87.0%, T. cruris 84.3 %, T. cpor6 78.0% in decreasing order of frequency 7. T. rubrum was the mot common causative organism of cernatophytosis(69.5%), with T. mentagroPhyte(16.1%), M. canis(13.5%), M floccosum(0.7%), 4 gypseam(0.2%) presenting in decreasing order of frequency
