1.Herpes Zoster Treated with Vidarabine-5-Monophosphate.

We present four cases of herpes zoster treated successfully with vidarabine-5-monophosphate, two patients receiving chernotherapy and radiation therapy due to Hodgkins lymphoma or colon cancer, a patient combined with viral meningitis, and a patient with herpes zoster ophthalmicus. Viclarabine can be used to treat herpes zoster in immunosuppressed patients or patients with malignancy or patients with severe complications.

3.Sources of Formalin: III. Formalin Concentration in Adhesives.

Formaldehyde in adhesives is used as preseratives as well as adhesives themeseles such as phenol-formaldehyde, carbamide-formaldehyde and melarnine-forma 1dehyde. Formaldehyde content in 43 kinds of adhesives used at present in Korea was determined by lutidine method. The study result was as follows. 1. The The distribution of formaldehyde concentrations of 43 different kinds of adhesives was in the 0 to 333. 9 ppm ranges. 2. High concentration of formaldehyde was observed in 3 kinds of wall paper adhesives and 1 kind of stationary adhesive, which has an important clinical significance because of their frequent contact sources of domestic uses.

4.A Clinical Study on Cysticercosis.

Authors reviewed the clinical features and diagnostic approaches in 70 patients of cysticercosis who had been diagnosed at Seoul National University Hospital from 1978 to 1982. The ages of the 70 patients ranged from 4 to 65 years at the time of the diagnosis (mean age 36.9years) with 62.9 percent between the age 20 and 50 years. Male were more frequently involved than female (2:1). Forty-seven patients (67.1%) presented with neurological symptoms and signs, among which epileptic seizures (66%) was the most common one. Subcutaneous nodules were observed in 36 patients (50.1%) and 24 patients of them had no abnomal clinical findings other than subcutaneous nodules. Computed tomography (CT), employed in 46 patients, showed multiple low densties in 31 patients (67.4%).

6.Recurrent Vestibulopathy: Clinical Characteristics and Efficacy of Combination Therapy .

BACKGROUND AND OBJECTIVES: Recurrent vestibulopathy is defined a disease characterized by more than a single episode of vertigo of duration characteristic of endolymphatic hydrops but without auditory or clinical neurological symptoms or signs. To investigate the clinical characteristics and the efficacy of combination therapy, we analyzed the clinical records of the patients diagnosed as recurrent vestibulopathy. MATERIALS AND METHOD: Clinical records of sixty four patients diagnosed as recurrent vestibulopathy were retrospectively reviewed. The data on age, sex distribution, natural history, family history of recurrent vestibulopathy, concurrent headache, caloric response was analyzed. The efficacy of combination therapy for vertigo control in the patients with a minimum 24-month follow-up was also evaluated. RESULTS: Mean onset age of recurrent vestibulopathy was 43 years and there was a female preponderance. Concurrent headache and elevated SP/AP ratio in electrocochleogram was frequently observed in these patients. After the combination medical therapy, patients with severe recurrent vestibulopathy showed significant decrease in the number of vertigo spells with 37.5% of complete control of vertigo. CONCLUSION: As a distinctive clinical disorder with unknown cause, recurrent vestibulopathy should be always considered to the patients complaining recurrent episodic vertigo. Combination therapy individualized to the symptoms and signs of the patients with recurrent vestibulopathy might be effective in reducing the frequency of vertigo attacks. Further case-control studies with large population should be necessary.

7.Long-Term Results of Reconstruction of Orbital Wall Fracture With Resorbable Copolymer Mesh.

PURPOSE: This study examined the postoperative long-term results of the reconstruction of orbital wall fracture using Macropore(R). METHODS: The long-term results for 10 patients who were diagnosed with orbital wall fracture and received the repair of orbital wall fracture using Macropore(R), were examined postoperatively after an average of 23.5 months. RESULTS: The mean difference in orbital volume between the fractured orbit and the unaffected orbit after an average of 23.5 months postoperatively was 1.01+/-0.70 cm3 (4.75+/-2.92%), which slightly increased compared to 0.70+/-0.52 cm3 (3.26+/-2.10%) immediately after the operation, although the difference was not statistically significant (p-value=0.190). At the last follow-up, the shape of Macropore(R) was not observed in 3 out of the 10 patients, and intense ossification was observed at the site of fracture in 6 patients. At the last follow-up, one patient showed 1.5 mm enophthalmos, and the other patients did not show the occurrence of diplopia or the progress of enophthalmos. CONCLUSIONS: According to the long-term results of the reconstruction of orbital wall fracture, Macropore(R) is considered an implant that produces a safe and satisfactory effect without notable complications.

Insulin-like growth factor binding proteins (IGFBPs) are major regulators of insulin-like growth factor bioavailability and activity in metabolic signaling. Seven IGFBP family isoforms have been identified. Recent studies have shown that IGFBPs play a pivotal role in metabolic signaling and disease, including the pathogenesis of obesity, diabetes, and cancer. Although many studies have documented the various roles played by IGFBPs, transcriptional regulation of IGFBPs is not well understood. In this review, we focus on the regulatory mechanisms of IGFBP gene expression, and we summarize the findings of transcription factor activity in the IGFBP promoter region.

9.The Histological Change of Orbicularis Oculi Muscle according to the Age.

This study is to investigate the patients who had blepharoplasty for cosmetic purposes or facial nerve disorders. 21 patients were examined with an age from 17 to 63 years. Sixteen cases were classified as a normal group for investigation of the histopathologic change according to the age. The rest were patients of Bell's palsy(2), Botox(R) injection(2) and blepharospasm (1). We measured a mean diameter of myofiber and dimension's proportion of muscluar tissue per regular visual field. First, we measured a statistical significance according to the age in normal cases by grouping them into younger than 30-year-old, 30's, 40's, 50's and 60's. Second, we divided normal cases(16) into Group A and B by two bases for statistical analysis. In conclusion, although there were several differences as vacuolation, internalization of sarcolemnic nuclei between 35-year-old and 62-year-old patients in simple comparison, there was no uniform decrease according to the age. There were also no significant statistical changes in the value of a diameter of myofibers and dimension's proportion of muscular tissue according to the age in any of the two normal groups. But in Botox(R) injection and Bell's palsy, blepharospasm patients, two values showed remarkable difference compared to the normal group of same age.

10.The Role and Difficulties of the Use of Routine Screening Abdomen Ultrasound for the Detection of a Liver Metastasis in Postoperative Breast Cancer Patients.

PURPOSE: To evaluate the role and difficulties of the use of routine abdomen ultrasound (US) in postoperative breast cancer patients. MATERIALS AND METHODS: We reviewed the clinical records of 2460 patients who received breast cancer surgeries and underwent routine follow-up abdomen US for more than five years. We evaluated the number and clinical conditions of patients with a liver metastasis. We also evaluated the cut-off point of the breast cancer stage where a metastasis was likely to occur using the chi-squared test and receiver operator characteristic (ROC) analysis. RESULTS: A metastasis developed in 238 patients (9.7%), and the liver was the third most common organ site. However, just 24 (0.98%) patients presented only with a liver metastasis. Among these 24 patients, a metastasis was detected in 17 patients with the use of routine abdomen US. The cut-off point for a metastasis was Stage 3A. CONCLUSION: The use of routine screening abdomen ultrasound for the detection of a liver metastasis in postoperative breast cancer patients is not recommended. However, US can be used selectively in patients with clinical symptom or that present with a high stage equal or greater than Stage 3A.
