3.Changes in Renal Expression of Natriuretic Peptides and Their Receptors mRNA Induced by Head-down Suspension in Rats.

BACKGROUND: Head-down suspension (HDS) of rats has been used as a model for the simulation of a microgravity environment. Atrial natriuretic peptide (ANP), C-type natriuretic peptides (CNP) and their receptors are found in the kidney, suggesting that these peptides could play a significant physiological role in the kidney. Therefore, this study was investigated the changes in the adaptations of renal natriuretic peptides and their receptors syntheses after 4 weeks of HDS in rats. METHODS: Unanesthetized, unrestrained, male Sprague-Dawley rats were subjected to either a horizontal position (control rats) or a -45degreeshead-down tilt using the tail-traction technique (HDS rats). This study observed the renal syntheses of natriuretic peptides as a expression of ANP and CNP mRNA, and also determined the expression of A-type natriuretic peptide receptor (NPR-A) mRNA and B-type NPR (NPR-B) mRNA. The expressions of natriuretic peptide and NPR mRNA were measured by reverse transcription-polymerase chain reaction with [(32)P]-dCTP following 4 weeks of HDS in the kidney of both control and HDS rats. RESULTS: After 4 weeks of HDS, the expression of ANP mRNA significantly (P<0.01) decreased, while CNP mRNA expression was showed the non-significant increasing trend in the kidney of HDS rats. NPR-A, which binds with ANP, was significantly (P<0.001) decreased in renal mRNA expression of HDS rats compared with controls. Expression in mRNA of NPR-B, which binds with CNP, showed a slightly decreasing trend in the kidney of rats following HDS. CONCLUSION: These results suggest that the renal adaptation following 4 weeks of HDS exerts to maintain the blood volume and electrolyte balance through attenuation of syntheses in the natriuretic peptide and its binding receptor, especially in ANP rather than in CNP systems.

6.The Effect of Oral Ketoconazole in Dermatophytosis.

Seventeen volunteers with cutaneous dermatophytosis were enrolled in a clinical trial to evaluate the effectiveness of oral ketoconazole. The group included six patients with tinea cruris(6 malcs), four with tinea corporis, including one with tinea faciai, (3 males, 1 female), three with tinea versicolor(3 males) and four with tinea capitis(3 males, 1 female). The oral ketoconazole was taken within 10 minutes after a meal. The patients under the age of 10 received 100mg of ketoconazole per day, whereas those aged over 11 received 200mg of ketoconazole until the skin lesions are cured. Seventeen patients had complete clinical and mycologic cure, one responded clinically. It required one to four weeks to become culturally negative for tinea cruris, four to seven weeks for tinea corporis, three to eight weeks for tinea capitis. For tinea versicolor it required three to five weeks to become negative by scotch tape method. Adverse reactions to ketoconazole were absent and no patients required discontinuation of the drug. The results indicate that ketoconazole is a safe and effective drug for the treatment of dermatophytosis.

7.IMA-origin Lymph Node Metastasis in Left Colon Cancer.

PURPOSE: Although an extended colon resection with high ligation of the inferior mesenteric artery (IMA) generally has been recommended as curative surgery for advanced left colon cancer (LCC), it shows little or no survival advantage over segmental resection with low ligation of IMA. The present study is to determine the risk factors associated with IMA-origin lymph-node (LN) metastasis and to clarify the implication of IMA-origin LN metastasis. METHODS: We examined the clinicopathological results of 200 cases of LCC. LN dissection was performed as follows: D2 en-bloc resection of the primary tumor, IMA-origin LN dissection, and paraaortic LN dissection. RESULTS: The incidence of IMA-origin LN metastasis of LCC was 4.5% (9 cases), and all cases involved sigmoid colon cancer. The independent risk factors of IMA-origin LN metastasis were four or more regional LN metastases (hazard ratio: 16.51, 95% confidence interval: 1.60~164.12) and a preoperative CEA level of greater than 6 ng/ml of (hazards ratio: 6.63, 95% confidence interval: 1.06~41.32). The incidence of IMA-origin LN metastasis among stage IIIC patients was 26.7%. Five of the 9 (55.6%) cases of IMA-origin LN metastasis had a concomitant paraaortic LN metastasis. CONCLUSIONS: The incidence of IMA-origin LN metastasis among patients with LCC was low; however, IMA-origin LN metastasis should be considered as a systemic metastasis.

Primary gastric squamous cell carcinoma represents a rare entity. Up to date, only seven cases have been reported in Korea. This paper reports a case of a 51-year-old male patient whose stomach cancer was diagnosed during general routine check-up. Upper gastrointestinal endoscopy showed a large ulcero-fungating mass on the body of the stomach. Histological examination of biopsy confirmed squamous cell carcinoma. An extended total gastrectomy including splenectomy and segmental resection of the large bowel was conducted. Postoperative adjuvant chemotherapy with capecitabine and oxaliplatin was followed. The patient has been following up the out patient's clinic after discharge.

10.Histological Comparison of Endoscopic Forceps Biopsy with Endoscopic Resection in Gastric Mucosal Elevated Lesion.

BACKGROUND/AIMS: The correct histological diagnosis of gastric adenoma is important, because it has been reported to be precancerous lesion and associated with focal gastric carcinoma. However, there is some discrepancy between the histology of the forceps biopsy and that of the endoscopic resection. In this study, we compared the histologic findings of gastric mucosal elevated lesion between the specimens of forceps biopsy and endoscopic resection. METHODS: We reviewed retrospectively 137 cases of gastric mucosal elevated lesion which had been removed by the resection such as polypectomy or endoscopic mucosal resection. All patients had undergone forceps biopsy before endoscopic resection. We compared the histologic findings of the specimens by forceps biopsy with those by resection. RESULTS: The histologic fidings were accordant at 101 of the 137 cases (73.7%), and different at 30 cases (21.9%). Among the 86 cases with adenoma in the biopsied specimens, 10 cases (11.6%) were finally diagnosed as gastric cancer in the resected specimens. CONCLUSIONS: Because biopsy specimens may not be presentative of the entire lesion, endoscopic resection of gastric mucosal elevated lesion is needed for accurate histologic diagnosis and treatment if adenoma is suspected.
