1.The Clinical Study of Macular Hole Retinal Detachment Associated with Peripheral Retinal Hole.

Most of the macular hole retinai detachinents are limited to the perimacular area. Therefore, we must look for the peripheral retinal hole when the macular hole retinal detachment extends beyond equator. And the principle of operation should be on the peripheral retinal hole rather than macular hole. Authors studied the clinical feature of macular hole retinal detachment associated with the peripheral retinal hole. Among the 39 macular hole retinal detachment patients, peripheral hole was found in 12 patients (30.7%). Sex distribution was the same and mean age was 55.9 years old. Retinal detachment was not diagnosed for more than I month in 7 cases (58.4%). Nine cases (75.0%) were extensive retinal detachments extending beyond 3 quardrants. Nine cases (75.0%) had one peripheral hole and most of peripheral holes were found on the ternporal side. Associated diseases were cataract (5 cases, 38. 5%) and high myopia (5 cases, 38.5%). In 4 cases (33.3%) hole was found on the opposite side. Among the 12 macular hole retinal detachment patients, 11 operations were performed with the technique of gas injection and scleral buckling which lead to successful attachment in 5 cases (41.7%). Among the 7 cases in which reoperations were needed, 3 cases (42.9%) were recurred in 3 months. In 8 cases (66.7%) there were improvements of post-operative visual acuity.

2.Two Cases of Generalized Syringoma.

We experienced two cases of generalized syringoma in a 21-year and a 34-year- old healthy woman. A 34-year-old woman have had numerous yellowish to pink papules on the face, anterior chest, upper part of the back and right buttock for the past twelve years. A 21-year-old woman have had numerous yellowish to brownish papules on the face, neck and anterior chest for the past ten years, and her mother had syringoma on the eyelids. Diapnosis was confirmed by clinical and histopathological findings as generalized syringoma.

3.Treatment of Recalcitrant Rosacea with 13 - Cis - Retinoic Acid.

Eight men with a one-ta-seven-year history of rosacea which poorly controlled by oral tetracycline or metronidazole, with or without topical medications, were given 13-cis-retinoic acid in dosages of p.5 to 0. 7 mg/kg/day for eight weeks. The size of erytherna and the number of papules had been gradually decreased since the second week of treatment. Eight weeks later more than 75p, regression was recognized in six patients, however telangiectasia remained unchanged. All eight patients complained of only mild degrees of cheilitis and dryness of the nose, and had no evidence of the serologic abnormalities on the tests examined during the period of therapy.

4.An Immunohistochemical Study of Oncogene Interaction in Skin Tumors.

BACKGROUND: Alteration of a wide variety of cellular oncogerms have now been implieated in the causation of many human cancers. However, genetic studies have indicated that an abnormality of a single gene is usually insufficient to elicit the fug transformed phenotype and that two or more genetic leaions may be necessary for this to occur. OBJECT: The purpose of this study is to examine the possibity of oncogene interaction which might be involved in the pathogenesis of human skin tugois. METHODS: The expression of the epidermal growth factor rattor(EGFR), c-jun protein, c-fos protein and p53 protein was assessed in frozen and corresgoAhng paraffin-embedded sections of 37 separate skin lesions (6 actinic keratoses, 6 squamo is ell carcinomas and 25 basal cell carcinomas using immunohistochemistry. RESULTS: Substantial number of the p53 positive cases were negative for c-fos, in contrast to normal tissue, which was p53 negative and c-fos positive. The negative correlation between p53 and c-fos staining was statistically significant(P<0.005). CONCLUSION: Although p53 appeared to have lost its normal role of regulatory early S phase protein in some lesions, in others high levels of p53 were assocated with underexpression of c-fos, reflecting the diversity of c-fos oncogene and the possible down regulation of c-fos expression by high levels of p53 protein.

Dieulafoy's lesion is a rare distinetive arterial malformation that can cause massive gastrointestinal henorrhage. Although in most cases the lesion is encountered in the proximal stomach within 6 cm of the gastroesophageal junction, similar lesions have been described in the antrum, duodenum, jejunum, colon, and rectum. We report 3 unusually located Dieulafoy-like lesion, two gastric antrum and one jejunum in patient who had gastrojejunostomy for pancreatic caneer. After a year follow up there was no bleeding episode in patient after endoscopic treatment.

8.Overexpression of cyclin D1 and cyclin E in 1,2-dimethylhydrazine dihydrochloride-induced rat colon carcinogenesis.

Deregulation of G1 cyclins has been reported in several human and rodent tumors including colon cancer. To investigate the expression pattern of G1 cyclins in 1,2- dimethyl-hydrazine dihydrochloride (DMH)-induced rat colon carcinogenesis, we studied the expression of cyclin D1 and cyclin E by quantitative reverse transcription-polymerase chain reaction (RT-PCR) analysis and immunohistochemistry (IHC). The mRNA level of cyclin D1 was increased 1.2-fold in adenocarcinomas but not significantly in adenomas, when compared with normal rat colonic mucosa (p<0.05). The cyclin E mRNA level was increased 2.7-fold in adenomas and 3.3-fold in adenocarcinomas (p<0.05). The PCNA mRNA level was also increased 1.9-fold in adenomas and 1.8-fold in adenocarcinomas (p<0.05). Immunohistochemical staining revealed exclusive nuclear staining of the neoplastic cells for cyclin D1, cyclin E and PCNA. Cyclin D1 expression was detected in 56.3% of the adenomas and in 61.5% of the adenocarcinomas examined, whereas cyclin E expression was detected in 87.5% of the adenomas and in 92.3% of the adenocarcinomas. Overall, cyclin D1, cyclin E and PCNA expression was significantly increased at both the mRNA and protein levels in normal colonic mucosa, adenomas and adenocarcinomas, but there was no significant difference in the degree of expression of these genes in adenomas and adenocarcinomas. Our results indicate that the overexpression of cyclin D1 and cyclin E may play an important role during the multistage process of rat colon carcinogenesis, at a relatively early stage, and may disturb cell-cycle control in benign adenomas, and thereafter, participate in tumor progression.

9.A Case of Verruciform Xanthoma Transformed from Lesions of Epidermal Nrvus Syndrome.

We experienced a case of 16-year-old female with verruciform xanthoma transformed from lesions of epidermal nevus syndrome. The biopsy specimen of yellowish brown to red verrucous papule on the right buttock showed 1) uniform and verrucous acanthosis without cytological atypia, 2) parakeratosis and hyperkeratosis,3) variable foci of superficial necrosis with crusting and microabscess in the granular layer, 4) rete ridge elongation and papillomatosis, 5) characteristic foam cells located within the papillary dermis, but not below the rete ridges. Our case was diagnosed verruciform xanthoma by histopathologic finding which occured as a secondary development in lesions of epidermal nevus syndrome.

10.Does Painful Heels in Ankylosing Spondylitis Demonstrate Distinctive Features on Plain Radiographs: A Study of 104 Cases.

OBJECTIVE: To investigate simple radiographic findings on painful heels in ankylosing spondylitis (AS). Heel radiography in most studies was from AS patients' non-painful heel. METHODS: Seventy AS patients (34 bilateral cases) with heel pain at the time digital radiographs were taken were studied. Standing lateral views (104 radiographs) of the heel were reviewed. Associations between radiologic abnormalities and disease duration and among various abnormal findings were analyzed. RESULTS: Ninety-six (93.4%) had radiographic abnormalities (82.7% in soft tissues/61.5% in bone). Abnormalities of bone only were observed in 9.6%, of the soft tissues only in 30.8%, and of both were 51.9%. These included Kager's triangle's blurring (77.9%), posterior soft tissue swellings near the Achilles tendon insertion (65.4%), obliterations of the retrocalcaneal recess (65.4%), erosions of the superior pole of the posterior calcaneus (31.7%), subplantar irregular spurs (20.2%), posterior traction spurs (16.3%), subplantar erosions (14.4%) and cortical thickenings of the inferior calcaneal body (5.8%). There was a significant association between swelling in the posterior soft tissue and obliteration of the retrocalcaneal recess (p<0.001). CONCLUSION: Digital radiography in AS is useful for observing not only bony lesions but also soft tissue abnormalities of the heel, particularly of the posterior heel. For assessing the symptomatic enthesitis of the Achilles, this simple and quick diagnostic tool is valuable when examining for soft tissues' alterations of the posterior heel.
