2.Pedal Indirect Lymphangiography.

PURPOSE: Recently, indirect lymphangiography has been developed as a relatively good and noninvasive imaging modality of the lymphatic system at extremities. But the disadvantage of the indirect lymphangiography is a low contrast ratio between the surrounding tissues and the contrast media in lymphatic vessels, because dimeric nonionic contrast media is water soluble and diluted in the proximal leg lymphatic vessels. We could have relatively better image than previously published images for the leg lymphatic system, when we injected contrast media with adequate high pressure in intradermal space of the interdigital areas at the foot dotsum. So, we would like to report the results. MATERIALS AND METHODS: We could study all 9 lymphedemas(primary :6, secondary: 3) from April 1990 to May 1993 on outpatient base. They were diagnosed as lymphedema clinically and radiologically. Ten ml of dimeric nonionic aget, iotrolan(Isovist 300 ) was injected into intradermal space with five 30-gauge needles The injection speed was more than 0.2mi/min. We have done one side pedal lymphangiogram in 30 minutes. The evaluation of the anterior superficial lymphatics was accordig to the criteria of the Weissleder(2). RESULTS: The results were as follows:1. All lymphatic vessels from foot to inguinal area could be visualized. 2. Two or three inferior inguinal lymph nodes could be visualized about 42%. 3. The most common abnormal finding of the lymphedma was the neovascularization of the lymphatics on indirect pedal lymphangiogram. CONCLUSION: If we use adequate technique relatively high pressure injection, correct intradermal needle insertion, adequate soft tissue exposure technique indirect lymphangiography is considered to be a safe and noninvasive imaging modality for the evaluation of the lymphedema of lower extremity lymphatics including inferior inguinal lymph nodes.

4.Allergic Contact Dermatitis due to Plaster of Paris Cast: A Case Report

A 42-year old man was admitted for treatment of a fracture of the medial condyle of the left femur. After 10 days of cast immbilization of the above mentioned extremity, erythematous papulo-vesicular eruption was noted under the external splintage. We concluded that this was an unusual allergic contact dermatitis which is caused by CaSO4.1/2H20, and proved it by patch test in our clinic.

5.Factors affecting foveal threshold measured by short-wavelength automated perimetry in primary open angle glaucoma.

PURPOSE: To evaluate the factors affecting foveal threshold measured by blue on yellow perimetry in moderate to severe open angle glaucoma. METHODS: Fourteen eyes of 14 patients who were diagnosed as moderate to severe open angle glaucoma underwent macular threshold test of the Humphrey Field Analyzer using white and blue targets. The foveal threshold using white targets (white threshold) was more than or equal to 18 dB in all cases. The cases of foveal threshold using blue targets (blue threshold) less than or equal to 0 dB were defined as foveal sensitivity loss group (7 eyes) and those more than 0 dB as no foveal sensitivity loss group (7 eyes). Those of age, contrast sensitivity, visual acuity, refraction, mean deviation, pattern standard deviation, corrected pattern standard deviation, vertical and horizontal C/D ratio, foveal white threshold, intraocular pressure, any point within central 5degrees with sensitivity less than or equal to 0 dB in visual field test were statistically analyzed between the two groups. RESULTS: Foveal white threshold and any point within central 5degrees with sensitivity less than or equal to 0 dB in visual field test showed significant difference between the two groups. CONCLUSION: Foveal dysfunction represented as decrease in blue threshold is possibly related to the paracentral scotoma which means any point within central 5degrees with sensitivity less than or equal to 0 dB in moderate to severe open angle glaucoma. However, significant deficits in either visual acuity or white threshold were not presented.

8.Four Cases Which were Mis-diagnosed as Granuloma Pyogenicum.

We have experienced 4 cases which were clinically diagncsed as granuloma pyogenicum, but histologically as a malignant melanorna, an eccrine angiomatous hamartoma with granuloma pyogenicum, a subungual exostosis, and a lipoma, with granulation tissue. A granuloma pyogenicum may have clinical resemblances to various benign or malignant lesions. Therefore, any lesion that resembles a granuloma pyogenicum should have this clinical impression confirmed by histologic examination.

9.Impact of attachment, temperament and parenting on human development.

The purpose of this review is to present the basic concepts of attachment theory and temperament traits and to discuss the integration of these concepts into parenting practices. Attachment is a basic human need for a close and intimate relationship between infants and their caregivers. Responsive and contingent parenting produces securely attached children who show more curiosity, self-reliance, and independence. Securely attached children also tend to become more resilient and competent adults. In contrast, those who do not experience a secure attachment with their caregivers may have difficulty getting along with others and be unable to develop a sense of confidence or trust in others. Children who are slow to adjust or are shy or irritable are likely to experience conflict with their parents and are likely to receive less parental acceptance or encouragement, which can make the children feel inadequate or unworthy. However, the influence of children's temperament or other attributes may be mitigated if parents adjust their caregiving behaviors to better fit the needs of the particular child. Reflecting on these arguments and our childhood relationships with our own parents can help us develop the skills needed to provide effective guidance and nurturance.

10.Surgical Efficacy of Probing with Silicone Intubation for Lacrimal Apparatus Obstruction in Children.

PURPOSE: The surgical efficacy of probing with silicone tube intubation for congenital and acquired lacrimal obstruction was investigated. METHODS: Medical records of 51 children (55 eyes) with lacrimal obstruction were reviewed. Twentyfive children (28 eyes) had congenital obstruction and 26 children (27 eyes) had acquired lacrimal obstruction. Age ranged from 13 to 78 months (mean 29.8 months). All of the children were examined preoperatively by dacryocystography and treated with probing and silicone tube intubation with or without inferior turbinate fracture. RESULTS: After the follow-up period of 6~41 months (mean 13.2 months), primary success rate of probing and silicone tube intubation was 88.1% (30/37 cases) : 88.9% (16/18 cases) in congenital obstruction and 73.3% (14/19 cases) in acquired obstruction. After combined inferior turbinate fracture and silicone tube intubation, success rate noted 100% in both congenital and acquired cases. The success rates showed 100% in the cases with maintenance period of silicone intubation over 16 weeks and 65.0% in the cases with maintenance period of silicone intubation under 16 weeks. The success rates revealed no statistical significance in etiology, age, obstruction site and lacrimal sac size. CONCLUSIONS: For congenital and acquired lacrimal apparatus obstruction in children, dacryocystography, probing, internal fracture of the inferior turbinate and silicone intubation should be maintained more than 16 weeks.
