The cyst, 3 X 3 X 5mm of size, was located within the muscle sheath and removed surgically in its capsule, confirmed as cysticercosis microscopically. We presents here a report of cysticercosis with a brief review of literature.

3.Magnetic Resonence Imaging of Anomalous Pulmonary Venous Connections.

PURPOSE: We evaluated the capability of MR in the diagnosis of anomalous pulmonary venous connection (APVC). MATERIALS AND METHODS: The patient group consisted of 11 total APVC and 8 partial APVC diagnosed with MR. Echocardiography was performed in all cases, cardiac angiography in 12 cases and operation in 12 cases. We compared MR findings with those of operation, echocard iography and cardiac angiography. RESULTS: In surgically proven 12 cases, diagnostic accuracy of preoperative MR, echocardiography and cardiac angiography was 100%, 67% and 63%, respectively. In the remaining cases, MR findings well correlated with those of echocardiography or cardiac angiography. Stenosis of common pulmonary vein or superior vena cava was identified in 4 cases. In one patient, MR depicted associated cor triatriatum clearly. CONCLUSION: MR is an effective modallty in depicting anomalous pulmonary venous con nections.

4.Extracorporeal Membrane Oxygenation for the Support of a Potential Organ Donor with a Fatal Brain Injury before Brain Death Determination.

The shortage of available organ donors is a significant problem and various efforts have been made to avoid the loss of organ donors. Among these, extracorporeal membrane oxygenation (ECMO) has been introduced to help support and manage potential donors. Many traumatic brain injury patients have healthy organs that might be eligible for donation for transplantation. However, the condition of a donor with a fatal brain injury may rapidly deteriorate prior to brain death determination; this frequently results in the loss of eligible donors. Here, we report the use of venoarterial ECMO to support a potential donor with a fatal brain injury before brain death determination, and thereby preserve donor organs. The patient successfully donated his liver and kidneys after brain death determination.

5.The Study of Proliferating Cell Nuclear Antigen (PCNA) Reactivity in Fibrohistiocytic Tumors.

Fibrohistiocytic tumors are a diverse group of benign and malignant soft tissue lesions, including dermatofibroma, dermatofibrosarcomaprotuberans, and malignant fibrous histiocytoma. On the clinical point of view, the distinction between benign and malignant lesions and malignancy grading is far more important. Therefore, we investigated 23 fibrohistiocytic tumors, using PCNA (PC10) which was a useful marker of proliferating activity, to differentiate the benign lesions from the malignant and correlate with other prognostic factors including tumor necrosis. cellularity, histologic grade, and mitotic counts. The results obtained were as follows 1) Positive tumor cells were clearly identified by the characteristic diffuse or granular nuclear staining. 2) The number of PCNA-positive tumor cells were 2.16+/-2.39% in dermatofibroma, 16.12+/-7.38% in dermatofibrosacoma protuberans, and 28.02+/-17.47% in the malignant fibrous histiocytoma. The numbers of PCNA-positive tumor cells in the malignant lesions higher than in the benign (p<0.001). 3) Deep seated, large size (>5 cm) and recurred or metastatic cases of MFH were more the high PCNA index (more than 20%) than the low index (less than 20%) groups. 4) PCNA index in MFHs had positive correlation with the number of mitotic counts (r=0.7582, p<0.001), cellularity (r=0.5908, p<0.05) and histologic grade (r=0.4164, p<0.05). These results suggested that reactivity on PCNA might assist in the distinction between benign and malignant lesions in fibrohistiocytic tumors, and could be a useful prognostic factor in the patients with malignant fibrous histiocytoma.

7.Chromic Cyclitis, Pars Planitis.

This disease entity, first described under the desgnation of cyclitis by Ernst Fuchs, has received renewed attention in the recenet years, having been described as peripheral uveitis, pars planitis, and cyclitis with peripheral chorioretinitis. Apparently new ophthalmoscopic findings have caused some obserbers to believe they are dealing with a new entity, because indirect ophthalmoscopy with scleral depression shows exudation in the pars plana region in severe or advanced cases. In this entity, The following characteristic findings are noted, occurring bilaterally, inflammatory cells in anterior vitreous, slight flare and cells in the anterior chamber in come instances, dilated veins, and snowball deposition of inflammatory cells over the pare plana and ora serrata, seen with scleral depression and indirect ophthalmoscope extending over the lower 180 degrees. Cystoid macular degeneration are demonstrated with fluorescein angiography in over half the caces; papilledema may occur in late cases; and peripheral retinal perivascular sheathing is noted in long standing cases.

8.Ultrasonographic diagnosis of clonorchiasis

In order to assess the reliability of ultrasonographic criteria of clonorchiasis, a prospective study was performed in 14 patients who were diagnosed as clonorchiasis by ultrasonogram only, using the criteria of peripheral duct dilatation without dilatation of extrah epatic duct. Eleven out of 14 patients were turned out to be clonorchiasis, showing 93% accuracy. Authors in tend to stress these ultrasonographic signs are very well correlated with the CT and E RCP findings and are proved to be highly reliable for clonorchiasis, and thus clonorchiasis could be diagnosed by ultrasonogram only.

9.Extraspinal Tuberculosis of Bone and Joint

Recently, tuberculosis of bone and joint is decreased with good nutrition and environment, development of preventive medicine and improvement of treatment regimen. But it is still one of the common inflammatory diseases in Korea, and must be considered in the differential diagnosis of common orthopedic complaints. There are few reports on extraspinal tuberculosis of bone and joint. Forty one cases of extraspinal tuberculosis of bone and joint were studied in our department from January 1988 to August 1993. The results were as follows; l. Extraspinal tuberculosis of bone and joint were 41 cases (39 patients, 27.8% of 147 tuberculosis of bone and joint including spine). 2. The proportion of children and young adults was 56%, hips were involved in 11 cases, knee joints in 8 cases, ankle joints in 6 cases, feet in 6 cases and elbow joints in 5 cases. 3. Coexisting pulmonary tuberculosis was found in 19 patients and active lesion in 15 patients. 4. Confirmative diagnosis could be made by smear and culture of the lesion or pathologic findings or recently available polymerase chain reaction(PCR) method. 5. Various treatment, such as antituberculous medication, external immobilization, synovectomy, curettage and bone graft, arthrodesis, had been carried out. 6. Reactivation of other site was found in 7.3%(3 cases) and resistant tendency was found in 9.7%(4 cases).

10.A Case of Congenital Optic Pit.

Optic pits are congenital craterlike holes or indentation in the surface of the optic disc. They are oval in shape and located in the inferior temporal quadrant of the optic disc. The authors experienced a case of congenital optic pit of the left eye of 18 year old male-patient. Fluorescein angiography showed hypofluorescence of the pit in early arteriovenous phase, becomming hyperfluorescence in the late phase. Ophthalmoscopic view of the diseased eye showed no abnormality of macular area. There was no visual field defect except the enlarged physiologic blind spot. Here we briefly report a case of congenital optic pit. referring the literature relating to the optic pit.
