2.The Relationship between Nitric Oxide and CAtaractohenesis after Eximer Laser Photorefractive Keratectomy.

To assess the change of nitric oxide(NO) concentration according to photorefractive keratectomy(PRK) depth and the role of NO in cataractohenesis after excimer laser PRK. The treated 36 rabbits were divided into 12 groups according to PRK depth(-3.0, -7.0, -11.0 diopter [0]), the dates of sampling(the 1st day and the 7th day after PRK), and reatment of NOS inhibitor (NG-nitro-L-arginine methylester, L-NAME). NO concentration and superoxide dismutase(SOD) activity in the aqueous humor and malondialdehyde(MDA) concentration in the lens were measured. Changes of lens epithelium were examined by eletron microscopy on the 3rd day and the 6th day after -11.0D of PRK. The NO and MDA concentration in the group without L-NAME treatment on the 1st day after -11.0D of PRK were significantly higher than those in the L-NAME treated group and control group. The activity of SOD increased in all of the PRK treated group on the 1st day and was even higher on the 7th day. The histopathological changes of lens epithelium on the 3rd day after PRK were compatible with cellular necrosis and those of the 6th day after PRK were compatible with cellular reparing. The results suggested that the toxicity of NO after PRK correcting over -11.0D may a role in cataractogenesis, but the progress could be suppressed by treatment of NOS inhibitor.

3.Contralateral Vesicoureteral Reflux in Children with Abnormal Unilateral Renal Development.

A total of 48 cases of tuberculous lesion in the lymph nodes (43 cases), lung (3 cases) and soft tissue (2 cases), was subjected to fine needle aspiration cytology(FNAC). The age of the patients ranged from 19 to 77 year-old (average 33.6 years in age) and the male to female ratio was 1: 4. Thirty-four cases (70.8%) demonstrated distinct granulomatous reaction with or without caseation necrosis, nine cases (18.8%) showed no granulomas, but large amount of necrotic debris with numerous polymorphonuclear cells and histiocytes, and five cases (10.4%) revealed acellular material only. The overall AFB positivity in smears was 62.5%. In areas associated with granulomatous reaction and necrosis, AFB positivity was 55.8%, while it was 80.0% in cases with acellular necrotic material. There were 2 cases of parasitic infestation which could not be easily differentiated from tuberculosis based on aspiration smears only.

4.Expression of c-erbB-2 and Distribution of S-100 Protein Positive Dendritic Cells in Squamous Cell Carcinoma of the Uterine Cervix.

OBJECTIVE: The aim of this study was to assess the relationship between disease progression and expression of c-erbB-2 and S-100 protein positive dendritic cells in Cervical cancer. STUDY DESIGN: Tissues were analyzed from 100 patients. Each of them had invasive carcinoma(44), microinvasive(12), CIS(33), CIN(II) before treatment, c-erbB-2 oncoprotein expression and S-100 protein positive dendritic cell were confirmed by immunohistochemical staining. (Avidin-biotin complex method) RESULTS: C-erbB-2 immunostaining was significantly associated with disease progression (p<0.05). In case of CIN I, there was not noted stained specimen but in case of invasive carcinoma, 24 cases of stained specimen were noted. S-100 protein positive dendritic cell was not associated with disease progression of cervical carcinoma.(p>0.05) CONCLUSIONS: According to our results, c-erbB-2 is possible factor in Carcinogenesis of cervical carcinoma with progression of it. and S-100 protein positive dendritic cell was not associated with disease progression of cervical carcinoma.

5.A Clinical Study on the Incompetent Internal Os of the Cervix.

This study was undertaken for the clinical analysis and evaluation on 121 patients with incompetent internal os of the cervix, who were admitted and treated with McDonald operation or Shirodkar operation at the Soonchounhyang Medical Center from January 1991 to December 1995. The results of this study were as follows : 1. The incidence of this IIOC was 1.1% of 11,116 cases of total delivery. 2. The mean age of IIOC was 31.7 years old. 3. The average number of gravida was 3.2. 4. The most common contributary factor was previous history of artificial abortion (51.2 %), and midtrimester abortion (17.4 %), cervical laceration due to previous vaginal delivery (8.3 %) etc. was followed. 5. The success rate of operation was 76 %, and the highest success rate (85.7 %) was reveald with period from 15th weeks to 16th weeks of gestation. 6. When cervical dilatation was abscent or small, the success rate of operation was high. 7. The factors of failed operation were preterm labor (58.7 %), PROM (34.5 %), and PIH, bleeding. 8. The delivery method after operation was vaginal delivery in 83 cases (68.6%) and cesarean section in 38 cases (31.4 %).

7.Radiological evaluation of tricuspid atresia: an analysis of cineangiography in 11 cases

Total 11 cases of tricuspid atresia were diagnosed radiographically at Seoul National University Hospital in recent two years since 1979. Some characteristic radiological findings were analyzed in chest P-A view and cineangiographies of right atrium and left ventricle. The results are as follows; 1. Among the 11 cases, 4 cases were male and 7 cases were female. Age distribution was from 2 months to 19 years and mean age was 3 1/2 years. 2. Anatomaic types of those cases according to Keith's classification were as follows; type Ia in 2 cases, type Ib in7 cases, type Ic 1 case and type IIb in 1 case. 3. The cineangiographic features of tricuspid atresia are sequential filling of contrast media in the order of right atrium, left atrium, and left ventricle, and triangular filling defect at the base of the heart in all cases, and right atrial dimple in two cases. 4. It is essential for the diagnosis of tricuspid atresia to undertake cineangiographies of right atrium and left ventricle, and the interrelationship of great arteries.

8.Comparison Between Reliavilty and Clinical Usefulness of Saville's Spine Index and those of Singh's Index in Osteoporosis

The Saville's index of the lumbar spine and the Singh's index of the femoral neck is very simple method to measure degree of osteoporosis. Authors compared Singh's index and Savilles index with Dual Photon Absorptiometry(D.P.X.) and studied the reliability and clinical usefulness of two indices. We took the D.P.X. and simple L-spine lateral view in 103 patients in same time and also performed D.P.X. and both hips A-P view in 70 patients, then analyzed and compared each two indices with D.P.X. The Singh's index had less interpersonal difference and intrapersonal difference compared with Saville's spine index. There was some confidence of Singh's index and Saville's spine index compared with the D.P.X., but the relationship between Saville's spine index and D.P.X. was less promising than that of Singh's index. Authors suggest that the Saville's spine index has less clinical usefulness than Singh's index in measurement of osteoporosis.

9.Reconstruction Of Achilles Tendon Using Fascia Using Fascia Lata Allogrft: A Case Report

We had a female patient with soft tissue tumor involving large portion of Achilles tendon. After resection of tumor, Achilles tendon was reconstructed using an allogenic human fascia lata manufactured by Pfrimmer-Viggo. EMG findings on postoperative 8 months show marked improvement of function of medial and lateral gastrocnemius muscles. Biopsy specimen of fascia lata allograft, which was acquired during TAL 8 months postoperatively, shows myxoid degeneration, a few lymphocytic infiltration and some foreign body giant cells. Functional, histologic and EMG findings suggest that human fascia lata allograft could be used to bridge defect of Achilles tendon being resected during tumor surgery.

10.Significance of Preoperative Serum CA-125 and TPA Concentrations in Patients with Pelvic Tumors.

We measured serum levels of CA-125 and Tissue polypeptide antigen(TPA) in 135 patients with pelvic tumors(129 benign pelvic tumors and 6 malignant ovarian tumors) preoperatively. Each tumor marker was measured by immunoradiometric assay. Serum CA-125 levels of 35.0U/ml, 65.0U/ml and TPA levels of 80.0U/ml, 100.0U/ml were determined as cut-off values.
