1.Clinical investigation of lipoid pneumonia in adults.

Background: Exogenous lipoid pneumonia is caused by inhalation or aspiration of animal, vegetable or mineral oil. Most cases are ascribed to aspiration of oil in laxatives or nose drops Petroleum, another pure hydrocarbon used as a base in various medications, is occasionally involved. Especially animal oil produces severe tissue inflammatory reaction, but most patients present with only abnormal chest X-ray and no specific clinical symptoms or signs. Method: Seven patients, 3 males and 4 females, with exogenous lipoid pneumonia, who was hospitalized or referred to pulmonary division at Samsung Medical Center from December 1994 to July 1996, were included. They hadn a history of taking shark liver oil(so-called "squalene") for varying period of time. We reviewed clinical, radioloic and pathologic findings. Result: Patients took 7 to 30 capsules of "squalene" a day for at least one month to 5 years. Six cases had chronic disease such as diabetes, hypertension, or cerebrovascular accident. Respiratory symptoms of mild fever, cough and sputum were present in 3 cases and in 3 cases there was no clinical symptoms and signs but abnormal findings by chest X-ray. The major radiologic findings by simple chest X-ray and computed tomography consisted of consolidation, infiltration involving mainly right middle and both lower lobes, and ground-glass opacity. Five of six bronchoscopic examinations demonstrated both lipid droplets floating on the surface of bronchoalveolar lavage fluid and lipid-laden macrophages in bronchoalveolar lavage fluid or lung tissue. Follow-up chest X-ray showed improvement in 4 cases but no marked interval change in 3 cases after removal of exposure to "squalene". Conclusion: Shark liver oil can induce lipoid pneumonia in adults. In case of high clinical suspicion, confirmation of "squalene" use by careful history taking is required and bronchoscopy is helpful in diagnosis.

3.Effect of surface treatment of porcelain on tensile bond strength.

The purpose, of this study was to evaluate the effect of surface treatment of porcelain on tensile bond strength. To accomplish this purpose, this study was carried out with 120 samples which were divided into 12 groups with each 10 samples, and the first group was not surface treated, groups 2 through 5 underwent single surface treatment, and groups 6 through 12 underwent compound surface treatment. The results were as follows: 1. In statistic, all the single surface-treated groups showed higher tensile bond strength than the non surface-treated group and the sandblasted group showed the highest tensile bond strength as 10.34+/-2.50MPa. 2. All the compound surface-treated groups showed no noticeable difference in the tensile bond strength(9-11.5MPa). 3. In statistic, no significant difference was found between the sandblasted group and the compound surface-treated groups. 4. There was no fracture of porcelain while testing in this study. Above study demonstrated that compound surface treatment or sandblasing, if used single surface treatment, should be employed to guarantee successful clinical application.

Takayasu's arteritis is a chronic inflammatory arteriopathy of unknown etiology involving mainly the aorta and the large arteries arising from its arch. It is characteristically a disease of young females of Oriental descent. It causes occlusion, coarctation or aneurysmal dilatation of the affected vessels. We report, with the review of the literature, a patient with Takayasu's arteritis who developed the cerebrovascular disease.

6.A Case of Congenital Dislocation of the Knee.

Congenital dislocation of the knee is very rare skeletal deformity and was firstly described by Chatelaine in 1822. Since then there have been a few number of case reports and some series discussing the etiology and treatment. The tibia is displaced anteriorly in relation to the femur. It is generally subdassified as simple hyperextention, subluxation, and dislocation depending on the degree of the joint displacement and the severity of disease. Early gentle manipulation and serial splintage or plaster cast are recommended for treatment and were successful in the majority except the case of quadriceps contrarture or late correction. The authors experienced one case of congenital dislocation of the right knee and brief review of the related literatures was made.

7.Detection of C-erbB-2 Internal Domain in Bladder Tumor.

c-erbB-2 overexpression appears to play a role in determining the malignant potential of some human cancers. Using a specific monoclonal antibody to c-erbB-2 internal domain, 25 transitional cell carcinoma and I squamous cell carcinoma specimens of the human bladder, and 30 cases of BBN induced rat bladder tumors were analyzed immunohistochemically to detect the expression of this putative transmembrane receptor. c-erbB-2 internal domain was not expressed in rat urinary bladder carcinomas induced by BBN, but in 32% (8/25) of human transitional cell carcinomas. Although c-erbB-2 was expressed in human bladder tumor, there was no correlation between expression of this oncoprotein and characteristics of bladder tumor. Positivity for c-erbB-2 oncoprotein was easily identifiable as an intense red staining localized predominantly at the cell membrane, supporting the view that c-erbB-2 protein is a transmembrane molecule. In most of the positive tumors, the distribution of staining is heterogenous. This suggests that small populations of tumor cells that have elevated levels of c-erbB-2 oncoprotein can be detected by staining. The availability of antisera that detect many oncoproteins in paraffin embedded tissue makes it feasible to design retrospective and prospective studies on selected groups of patients.

9.Heterotopic Prostatic Tissue with Cystic Change in Retrovesical Space: A case report.

Heterotopic prostatic tissue has been reported in a variety of sites within and outside the urinary tract. Extra-urethral ectopic prostatic tissue is a distinct entity and may be more common than previously thought. We report a case of heterotopic prostatic tissue in 71-year-old man. Pelvic CT scan showed a well circumscribed cystic mass in the retrovesical space. Grossly, the tumor was 7.5 7.0 2.8 cm and revealed an ovoid unilocular cyst containing grayish amorphous granular materials. The prostatic origin of the tissue was confirmed by immunohistochemical staining for prostate specific antigen.

10.Multimedia Message Service (MMS) Improves Comprehension of Discharge Instruction in the Patients with Lacerations.

PURPOSE: This study was carried out to determine whether the use of multimedia message service (MMS) as discharge instructions about post-suture care improves patient's comprehension for those information. METHODS: One hundred nineteen (119) patients with lacerations and their caregivers who visited the Emergency Room at Soonchunhyang University Cheonan Hospital were included in this study. Participants were randomly assigned to receive MMS (n=66) or paper (n=53) discharge instructions after providing informed consent to engage in this study. Within 48 hours of discharge, an investigator called each participant by phone and asked a series of questions designed to test the participant's comprehension of their discharge instructions. In addition, participants were asked to complete questionnaires about confidence, accessibility, compliance, and the usefulness of discharge instructions. RESULTS: There were no differences in age, sex, educational background and subject (patient or caregiver) between the MMS and the paper group. In both groups, the mean and standard deviation in comprehension score was 8.1+/-1.3 and 6.6 +/-1.8, respectively. The difference was statistically significant (p<0.001). 'Yes' group of MMS and paper was 40(60.6%) and 16(30.2%) in accessibility, 39(59.1%) and 19(35.8%) in compliance, 26(39.3%) and 20(37.7%) in usefulness, respectively. CONCLUSION: The use of MMS as discharge instructions for patients with laceration improves patient comprehension.
