BACKGROUND: Good documentation of electrical injuries at the time of presentation is very important to emergency management, so this study was designed to investigate the clinical characteristics and the outcomes of patients with electrical injury. METHODS: A review of 75 cases of electrical injuries admitted to our hospital via the emergency department over a 4 year period from 1996 to 1999 was conducted. RESULTS: There were 49 patients with high-voltage injuries and 19 patients with low-voltage injuries. All but 4 patients were males, with a mean age of 29.5 years. The most common type of injury was 14.5% TBSA in the high-voltage group and 2.5% in the low-voltage group. Forty-nine(72.1%) of the injuries were work related. The number of patient with compartment syndrome was 19, and fasciotomies were performed in all but one patient. Myoglobinuria was noted in 22 patients, but no patient developed acute renal failure due to myoglobinuria. In the high-voltage group, 10 limb amputations were performed. Complication were observed in 12 patients. The most common complication was neurological injury. The average length of hospital stay was 50.7 days in the high-voltage group and 13.8 days in the low-voltage group. The overall mortality rate was 4.3%. CONCLUSION: Prevention of electrical injuries is very important. Education and compliance with safety measures, as well as basic knowledge and precaution in dealing with electricity, are essential to avoid these injuries.

The silicon tracheal T-tube was designed to maintain an adequate tracheal airway as well as to provide support in the stenotic trachea which has been reconstructed. Replacement of the T-tube was obviously undesirable following recent reconstruction of an unstable fracture of the airway. The loss of anesthetic gas through a T-tube while it is in place during anesthesia has been a problem. This report describes a simple method of establishing a satisfactory airway in a patient with a tracheal T-tube, and reviews the anesthesia aspects of modern surgical reconstruction of the larynx and cervical traches.

PURPOSE: To evaluate the performance of Automated Corneal Shaper(R)(ACS) METHODS: we analysed the changes in intraocular pressure, axial length and corneal thickness and its influence on corneal flap following keratectomy by ACS in 20 porcine eyes. RESULTS: Corneal flap was made by ACS with plate 160, and its size and thickness were measured. With the suction ring application, intraocular pressure rised from 17.0 +/-1.96 mmHg to 62.0 +/-11.8 mmHg (p=0.001), while corneal thickness and axial length were decreased from 1090 +/-75.1 micro meter to 1074 +/-73.7 micro meter(p=0.001) and from 19.94 +/-0.89 to 19.72 +/-0.73 mm (p=0.028) respectively. Thickness of corneal flap was 122 +/-23.9 micro meter and its diameter was 8.3 +/-0.15 mm in average. CONCLUSION: special attention is required during LASIK (laser in situ keratomileusis) with Automated Corneal Shaper(R) in order to prevent intraocular damage by abrupt rise in intraocular pressure and change of eyeball contour.

Each diastereomer of 10-thiophenyl- and 10-benzenesulfonyl-dihydroartemisinin was synthesized from artemisinin in three steps, and screened against chloroquine-resistance and chloroquine-sensitive Plasmodium falciparum. Three of the four tested compounds were found to be effective. Especially, 10 beta-benzenesulfonyl-dihydroartemisinin showed stronger antimalarial activity than artemisinin.

PURPOSE: The purpose of this study is to evaluate treatment results of multidisciplinary approach of critical ischemic limb with diabetic foot. MATERIALS AND METHODS: From March 2005 to March 2012, 674 diabetic foot patients were analyzed. Among them, 85 patients were neuroarthropathic type, 383 patients were infectious type, and 206 patients were ischemic type. The subjects were 206 patients who had critical ischemic limbs and major or minor amputations were done. Various single or combined treatment method before amputation was performed. We investigated their ABI, HbA1c, main occlusion lesion, limb salvage and hospitalization period by various treatment method. RESULTS: Major amputation was 27 cases, minor amputation was 179 cases. Mean HbA1c was 8.2%, and mean ABI was 0.66. Main occlusion lesion was 6 cases at common iliac artery, 13 cases at external iliac artery, 9 cases at internal iliac artery, 11 cases at common femoral artery, 23 cases at deep femoral artery, 52 cases at superficial femoral artery, 35 cases at popliteal artery, 40 cases at posterior tibia artery, 35 cases at anterior tibial artery, 28 cases at peroneal artery, and 13 cases at dorsalis pedis artery. Major amputations were decreased, minor amputations were increased, and hospitalization period was reduced by treatment of multidisciplinary approach. CONCLUSION: Treatment of multidisciplinary approach, which include preoperation percutaneus transluminal angioplasty, vascular surgery, and amputation, of critical ischemic limb with diabetic foot had advantages of limb salvage and hospitalization period reduction.

STUDY DESIGN: This is a prospective study. OBJECTIVES: We wanted to analyse the correlation between the radiographic and clinical results after anterior cervical discectomy and fusion (ACDF) for treating degenerative cervical diseases. Summary of Literature REVIEW: ACDF is a successful procedure for treating the degenerative cervical spine. Many studies have reported on radiographic results and clinical outcomes of this procedure. However, few studies have examined the relationships between the changes of the radiographic parameters and the clinical results after ACDF. MATERIALS AND METHODS: 26 patients who had single level ACDF performed for degenerative cervical diseases during the period between Jan, 2000 and Dec, 2004 were evaluated. All the patients underwent autologous iliac bone graft and plate fixation. The radiographic parameters, including the disc height, the disc space angulation and the spinous process distance were measured at the preoperative period, post operative 1 month and the last follow up period, respectively. The clinical changes were measured using the visual analogue scale (VAS) for neck and arm pain at the preoperative and last follow up period. The correlations between the radiographic parameters and the clinical outcomes were assessed by Pearson correlation. RESULTS: There were significant changes in disc height (6.7-7.9 mm) as well as disc space angulation (kyphosis: 1.1degrees, lordosis: 3.4degrees). The reduction in the neck pain VAS score (63.9-33.1) and the arm pain VAS score (57.9-29.7) was significant. None of the correlations between the radiographic parameters and the clinical outcomes were significant (p>0.05). CONCLUSION: Although the clinical outcomes improved significantly, there was no significant correlation between the radiographic parameters and clinical results after performing single level anterior cervical discectomy and fusion for the degenerative cervical disease.
