1.A human case of house dust mite Tarsonemus floricolus collected from sputum.

A 23-year-old medical student showed a positive reaction on a skin test for Paragonimus westermani, and two Tarsonemus floricolus mites were subsequently found by sputum examination and identified morphologically. Our report is the first human case of Tarsonemus floricolus in Korea.

2.Percutaneous transhepatic biliary drainage in 107 patients

Percutaneous transhepatic biliary drainage has played a major role in treatment of patients with bilairy tractdisease, especially obstruction by malignant disease. Percutaneous transhepatic biliary drainage was performed 128 times in 107 patients of obstructive jaundice for recent 2 years and 8 months from July, 1981 to March, 1984 at departement of radiology, Seol National University Hospital. The resuls were as follows; 1. The male to femaleratio was 2:1 and the age ranged from 19 to 88 with average of 54. 2. The causes of obstructive jaundice included 94 malignant diseases and 13 benign diseases. Malignant diseases were 48 cases of bile duct cancer, 20 cases of metastasis, 20 cases of pancreatic cancer, 4 cases of gallbladder cancer, 1 case of ampulla Vater cancer, and 1case of duodenal cancer. Benign diseases were 8 cases of comon bile duct stone and 3 cases of benign stricture and2 cases of cholangitis. 3. The most common indication was palliative drainage of obstruction secondary to malignant tumor in 78 cases. 4. The overall success rate was 93.7%. Internal drainage was acheved in 34 (26.5%)and external drainage was accomplished 86(67.2%). 5. Decline in serum bilirubin level was found in 100cases(5.6%). Percutaneous transheatpic biliary drainage is a proven technique for non-operative biliarydecompression and established alternative to surgery.

3.A Clinical Study of the Ankle Fractures

A clinical analysis was done on 129 patients (131 cases) of the ankle fractures that were admitted and treated in Orthopedic Department, Inchon Christian Hospital from January 1973 to December 1980. The following results were obtained: l. Of the 129 patients, male was 104 patients and female 25 patients, the average age of the patients was 37. 2. The main cause of the injuries was traffic accident (38.2%), and the remain causes were industrial accident (28.2%), fall down (18.3%), slip down (11.5%), and sports injury (3.8%) in order. 3. The most common type by Lauge-Hansen classification was supination-external rotation type (37.4%). 4. In method of treatment, operative treatment was done in 72 cases and nonoperative in 59 cases. 5 The average duration of the cast immobilization in nonoperative treatment was 8 weeks and operative 7 weeks. 6. The better result was obtained by operative treatment than nonoperative one. 7. The most poor result was noticed in pronation-external rotation type according to Lauge-Hansen classification. 8. We agree that the accurate reduction of the lateral malleolus make the talus in good anatomic position in ankle joint. 9. We agree that Lauge-Hansen classification is useful in diagnosis and treatment of ankle fracture.

4.Operative Treatment Using Minimized Internal Fixation and Tension Band Technique on Proximal Humeral Fracture.

Most proximal humeral fractures respond satisfactorily to simple conservative treatment, but operative treatment is recommended in cases where poor results are anticipated by prolonged immobilization, or because of severe displacement and comminution. We used a minimal internal fixation and tension band technique in severely displaced and comminuted cases to achieve firm fixation and avoid subacromial impingement. Passive range of motion of the involved shoulder began within the first postoperative day and active range of motion exercise was encouraged as soon as possible within the first postoperative week. Between February 1996 and November 1997, we operated using this technique on 15 patients ranging in age from 16 to 71 years (average 52). Follow-up averaged 13 months. During the follow-up period, all patients except one patient, had a active full range of motion on the affected limb. According to the evaluation on the 10 patients who had follow- up periods over 12 months by Hawkins rating scale of shoulder function, 6 patients had achieved a good result and 4 patients had achieved a fair result at the last follow up. In conclusion, tension band technique with minimal internal fixation could afford sufficient fracture stability to allow early, aggressive rehabilitation. As a method of internal fixation on a proximal humerus fracture, this technique may have particular advantages in elderly osteoporotic patients.

5.Impact of Anti-HCV(+) on Renal Transplantation.

BACKGROUND: A high incidence of chronic liver disease is reported in end-stage renal failure patients due to hemodialysis and blood transfusion. An average of 20% of the patients who received renal hemodialysis are infected with hepatitis C virus, but the incidence of infection in these patients varies widely according to geographic location and the diagnostic methods used. Controversy exists regarding the impact of pretransplantation HCV infection on the outcome of renal transplantation. We measured the seroprevalence of the antibody to hepatitis C (anti-HCV) in renal transplant candidates and compared the prevalence of posttransplantation liver disease, graft, and patient survival among renal transplant recipients with and without anti-HCV at the time of the transplantation, and we attempted to define the possible factors affecting the clinical course following renal transplant in positive HCV patients. METHODS: Between June 1990 and December 1997, 634 patients underwent renal transplants at our institute. Viral infection with hepatitis were analyzed in these patients by using anti-HCV positivity using first, second, and third generation EIA, and RT-PCR. RESULTS: Twelve (12) of the 634 (1.9%) had positive anti-HCV before renal transplantation. During a mean follow-up of 29.4 months, viral mRNA was detected in the pretransplantation serum in 3 out of 8 (37.5%) positive anti-HCV patients. Among the 12 patients with positive anti-HCV, 2 (16.6%) showed early liver dysfunction, and 1 (8.3%) showed histologic progression to chronic hepatitis leading to hepatic failure and death. Graft loss occurred in 1 of the 12 (8.3%) patients with positive anti-HCV and in 62 of the 622 (9.8%) patients with negative anti-HCV. Three (3) out of the 12 (25%) patients with positive anti-HCV, and 121 of the 622 (19.6%) patients with negative anti-HCV had episodes of rejection. One (1) of the 12 (8.3%) patients with positive anti-HCV and 26 of the 622 (4.2%) patients with negative anti-HCV died after kidney transplantation. There were no statistical differences in patients or graft survival between the positive anti-HCV (+) and the negative anti-HCV patients. CONCLUSION: From these results, we can assume that the presence of anti-HCV without advance liver disease should not be a contraindication for kidney transplantation.

6.A Case of Prune-Belly Syndromes.

We have recently experienced a case of prune-belly syndrome, in 119/12-year-old male child with congenital defects of abdominal muscles, cryptorchidism, hydronephrosis, megacystis and spina bifida. Diagnosis was confirmed by EMG, IVP, retrograde cystogram, radiorenogram & renal scanning, and roentgenologic examinations. The patient in this report may be one of the most long-term survival case reported in korean literature.

7.One Case of Displacement of Copper - T IUD into Urinary Bladder.

Uterine perforation is a serious complication in users of intrauterine devices for contraception. Authors encountered one case of displacement of Copper-T IVD into urinary blpossibly due to uterine perforation on insertion of the Copper-T IUD in a 28 year old 1-multipara, who experienced normal intrauterine pregnancy in the circumstances. This patient was in silent, asymptomatic condition and underwent cesarean section. And Copper-T IUD was removed under the cystotomy. So we report one case with brief review of literatures

8.A Clininical Study on Congenital Malformation of Ileum.

This is a clinical study of 30 cases of congenital malformation of ileum at Severance hospital from 1966 to 1975. They were confirmed after operation and the results were summarized as follows; 1. Total number of patients was 30 cases including 25 males 5 females. Among them 20 cases were included at pediatric agr from birth to 15 years of age. The most frequent occurrence was within 1 month of age with 11 cases. 2. Confirmed diagnesis after operation were 3 cases of ileal duplication,3 cases of ileal stenosis, 2 cases of ileal atresia, 1 cases of ileal hypoplasia, and 21 cases of Meckel's diverticulum. 3. Except for Meckel's diverticuium all the patients were studied which simple abdominal X-ray, which revealed complete and partial intestinal obstruction. Among them 4 cases were found microeolon and narrowing of distal 1 cases of ileum at 1 cases of ileal atresia and 2 cases. 4. After operation, 26 of 30 cases were recovered but 3 cases (atsesia 1, stenosis 1, and hypoplasia 1) were discharged with complications. And 1 case of multiple ileal stencelsdied of sepsis and wound infection.

9.A Case of Cryptococcal Hepatitis.

A case of cryptococal hepatitis in a 4-year-old girl was presented with a brief review of the literature. She was admitted with the chief complaints of jaundice, abdominal pain, and semicomatose mental state. Treatment was started including dexamethasone under impression of fulminant hepatitis. During treatment massive G-I bleeding appeared which required eleven pints of blood and 20 days for control. Upper G-I series revealed duodenal ucer. And explolparotomy was performed because of free air in the peritoneum. Operative liver biopsy done at that time revealed many cryptococci in the biopsy tissue. Therefore we treated her with-5-fluorocytosine and the result was good. Follow-up of her for a year and half revealed that she was in here good general condition. Therefore we consider that secondary change in liver such as cirrhosis has not yet occurred in her.
