3.Statistical Evaluation of Lineage Markers in Individual Identification.

Mitochondrial DNA (mt DNA) and the non-recombining region of the Y chromosome are passed down, unaltered, from generation to generation, matrilineally and patrilineally, respectively. Therefore, the Y-chromosome DNA and mtDNA are known as lineage markers, and they play important roles in studies based on human migration and evolutionary history. Y-chromosome DNA is used in forensic analysis to identify individuals involved in cases of sexual assault. In this paper, we review the methods of statistical evaluation of lineage markers used in forensic identification. We also review the combined approach of autosomal and lineage marker evidence.

4.Absence of Clinical Value of TZAP Mutation and Expression in Non-small Cell Lung Cancers

The zinc finger protein ZBTB48 is a telomere-associated factor and renamed it as telomeric zinc finger-associated protein (TZAP). It binds preferentially to long telomeres competing with TRF1 and TRF2. However, its mutation in cancers has not been studied. In the present study, we analyzed TZAP mutation in 134 non-small cell lung cancers (NSCLCs). And its big data analysis was performed using COSMIC and TCGA data analysis. TZAP mutation was not found in 134 NSCLCs. And big data also showed that TZAP mutation was extremely low (0.59%, 15/2548). TCGA survival analysis showed no prognostic value of TZAP expression in lung adenocarcinoma (p = 0.185) and squamous cell carcinoma (p = 0.817). When stratified patients sorting as 25:25 (quarter), it has a significance (p = 0.003). This result suggested that genetic change of TZAP did not appear to be a possible molecular marker in lung cancer.

6.Efficacy of Silicone Nasolacrimal Intubation with Mitomycin C Treatmentfor Treatment of Incomplete Nasolacrimal Duct Obstruction.

PURPOSE: To investigate the efficacy and safety of adjunctive Mitomycin C (MMC) during silicone intubation (SI) for treatment of acquired incomplete nasolacrimal duct obstruction (NLDO). METHODS: Nineteen eyes with incomplete NLDO underwent SI only (group I). 20 different eyes were administered adjunctive 0.4 mg/ml MMC for 4 minutes (group II). Diagnosis of incomplete NLDO was based upon subjective complaints of tearing, dye disappearance testing, clinical evidence of epiphora, and nasolacrimal duct irrigation. Follow-up examination was conducted at 1, 3, and 6 months. RESULTS: Follow-up ranged from 3 to 15 months (mean, 6.1 months). Twelve (63.1%) eyes in group I and 14 (70.0%) eyes in group II had a successful outcome and remained symptom-free. Four (21.1%) eyes in group I and three (15.0%) eyes in group II showed improvement but still experienced periodic epiphora. The difference was not statistically significant (p=0.873). CONCLUSIONS: SI as a treatment for incomplete NLDO provides complete relief or substantial improvement in a significant percentage of patients but MMC application during SI did not benefit outcome.

7.Computed Tomographic Findings of Liver Injury in Adults: Role of CT Classification on Management.

PURPOSE: We studied to compare computed tomographic(CT) findings of liver injury with management methods in adults and, moreover, to present the CT basis for the management. MATERIALS AND METHODS: We retrospectively reviewed CT scans of 43 adults diagnosed as liver injury during a 66-month period. Thirty-eight patients were hemodynamically stable. Thirty-two of them were managed conservatively, whereas six managed operatively. Five unstable patients underwent emergency operation. We classified CT findings according to the severity of liver injuries(ie, hematoma, laceration, and periportal tracking) and hemperitoneum, ranging from grade 1 to 5 and from 0 to 3+, respectively. Thus, we compared the CT classificafions with their management(ie, operation rate), especially hemodynamically stable patients. RESULTS: Operation rates of all patients and hemodynamically stable patients were 26% and 16%, respectively. Operation rate at each grade of liver injury was low, especially in hemodynamically stable, despite relatively high operation rate in grade 4. Operation rate of 3+ hemoperitoneum was 100%, including hemodynamically stable patients, in contrast to otherwise low operation rate of others. CONCLUSION: Most liver injury in adults, including grade 4, were managed conservatively, especially hemodynamically stable. Though large amount of hemoperitoneum(ie, 3+) required operation, most hemoperitoeum were managed conservatively. Thus, CT findings of liver injury is helpful in the decision for the management method.

8.Two Cases of Aortic Intramural Hematoma Diagnosed with Transesophageal Echocardiography.

Aortic intramural hematoma(AIH) is known as a variant of acute dissection due to rupture of the vasa vasorum without a intimal tear. In elderly hypertension patients patients with aortic dissection but without the characteristic echocardiographic findings of a double-channel aorta, this disease entity should be suspected and attention should be given to find segmental wall thickening of the aorta >0.7cm. AIH may progress to typical dissection or to rupture in considerable number of the patients, so careful follow-up is necessary and the same management principles for the patients with dissection can be applied in this variant. We report two cases of AIH showing typical eccentric mural thickening without dissection membrane with transesophageal echocardiographic follow-up.

9.Primary repair of untreated cleft palate in an elderly patient:a case report

Untreated adult or elderly cleft lip and palate patients are rarely seen, but studies on delayed primary palatal closure have been performed in the less developed Asian and African countries, where access to medical care is difficult. A 64-year-old woman visited our clinic with untreated cleft palate with a 40×20-mm-wide defect in the medial palate. Two-flap palatoplasty under general anesthesia was performed to close the cleft palate. After 1 month, the result was favorable without any complications including oronasal fistula. Cleft palate primary repair in an elderly patient is rare and has some surgical problems that are associated with a wide range of defects, but good results can be obtained if surgery is performed well with appropriate considerations.
10.The Refill Rate and Reasons for Abandonment of Phosphodiesterase5 Inhibitors in Erectile Dysfunction Patients.

PURPOSE: The aim of this study was to determine the rate of abandonment of phosphodiesterase (PDE)5 inhibitor therapy in patients who had reported good treatment efficacy, and to assess the reasons for abandonment of therapy and rate of prescription refilling. MATERIALS AND METHODS: Between January 2004 and December 2005, patients with erectile dysfunction (ED) who had begun PDE5 inhibitor therapy in our center were enrolled in this study. A telephone survey of these patients was conducted to determine the rate of prescription refilling and the reasons for abandonment. After receiving the first prescription, patients were asked whether they had used the medication and whether the treatment improved their erectile function using a Global Assessment Questionnaire (GAQ). Medical records for these patients were reviewed retrospectively. RESULTS: 753 patients with ED began PDE5 inhibitor therapy. Only 200 subjects (26.5%) asked for a refill of PDE5 inhibitors. Of 553 subjects, 330 consented to the telephone survey and 226 subjects (68.5%) had positive response on the GAQ. Forty-one patients continued taking PDE5 inhibitors in other clinics. Finally, the reasons for abandonment were assessed in 185 patients who had reported good treatment efficacy but abandoned therapy. The majority reported that they had experienced improvement of spontaneous erectile function (21.6%). Two hundred subjects requested refills in our center (26.5%) and 41 patients (5.4%) continued a prescription in other clinics, so the rate of refill was greater than 31.9%. CONCLUSIONS: According to the results of this study, the improvement of ED was cited as the major reason for abandonment of PDE5 inhibitor therapy. The rate of prescription refilling was greater than 31.9%.
