PURPOSE: To describe the characteristic CT findings of ameloblastomas in the mandibleand maxilla. MATERIALS AND METHODS: CT findings of 11 patients with ameloblastoma (9 cases in the mandible and 2 cases in the maxilla) proved by excisional biopsy were evaluated retrospectively with regard to the location, size, multilocularity, solid and cystic component, cortical destruction, soft tissue invasion, and contrast enhancement RESULTS: These were 8 multilculer expansile and I unicystic (developed in a dentigerous cyst) mandibler ameloblasftomas, and 2 unilocular maxillary ameloblasftomas. All cases showed inhomogenously enhancing solid component, nine of which were larger than cystic component. Nine cases, larger than 5 cm in diameter, revealed either a focal or extensive cortical destruction with various degree of invasion into the adjacent structures. One maxillary ameloblastoma contained a thick calcification along the margin of the lesion. CONCLUSION: Maxillomandibular ameloblastomas appeared as expansile lesion containing enhancing solid component relatively larger than cystic portion and having cortical destruction in large lesions(5cm>). Maxillary ameloblastomas were unilocular in appearance in spite of multilocularity in mandibular counterparts.

Intra-Arterial Digital Subtraction Angiography(IADSA) was performed in 128 patients during 13-month period. Weexperienced the same advantages of IADSA over conventional film-screen angiography:(1) significant reduction incontrast material dose: 1/3 of the dose of conventional angiography, (2) reduced film cost: 18% of the cost ofconventional technique, (3) decreased need for selective catherization,(4) shortened examination time, and (5)more ready detection of cnntrast material.

Diabetes is accompanied by delayed wound healing and insufficient granulation tissue formation, possibly because of a defect in fibroblast function. Maintaining glucose level at acceptable low level is considered to be an important part of the clinical treatment of diabetes, but the exact mechanism by which diabetes delays wound repair is not yet known. This study was designed to evaluate the effects of glucose on proliferation([3H]-thymidine uptake) and collagen synthesis([3H]-proline uptake) in dermal fibroblasts from non-lesional skins and chronic wounds. The subjects were divided into 3 groups: non-diabetic control(n = 5), glucose controlled(glycated Hb < 7%) diabetics(n = 5), and glucose uncontrolled(glycosylated Hb > 8%) diabetics(n = 5). The fibroblasts were cultured in two conditions, low glucose(5 mM) and high glucose(20 mM) medium. The data were compared using Mann-Whitney test. The results are as follows: 1. There were no significant differences in the proliferation and collagen synthesis of non-lesional skin fibroblasts in all groups. 2. In high glucose medium culture, the proliferation of the fibroblasts from the wound of the glucose uncontrolled diabetic group was significantly lower than those of the other 2 groups(p< 0.05), and the collagen synthesis of the wound fibroblasts of the control group was significantly higher than those of 2 diabetic groups(p< 0.05). 3. The proliferation and collagen synthesis of the wound fibroblasts in all groups were significantly lower than those of the non-lesional skin fibroblasts(p< 0.05). 4. In the control group and the glucose controlled diabetic group, the proliferation of the wound fibroblasts in the high glucose culture media were lower than those in the low glucose culture media(p< 0.05). Also, in the glucose controlled diabetic group, the collagen synthesis of the wound fibroblasts in the high glucose culture medium was lower than that in the low glucose culture medium(p< 0.05). These results demonstrate that the glucose level may affect the proliferation and collagen synthesis of fibroblasts, and the strict blood glucose control is beneficial to the prevention and treatment of diabetic chronic wounds.

For seven years from 1962 to 1968, fecal specimens of the patients were examined not only for the discovery of intestinal protozoa and helminths, but also to provie data on the incidence of intestinal parasites among the residents of Kyungpook Province, Korea. The Formalin-ether sedimentation, Lugol solution, and Heidenhain's iron-hematoxylin stained films were prepared for the recovery of intestinal parasites. Of 2,414 samples of feces examined, 35.7 percent were found to be infected with one or more species of protozoa. Among six species of protozoa detected, Entamoeba coli was observed most frequently(15.3%) followed by Entamoeba histolytica (11.9%), and then the Enteromonas hominis least often(1 case). For the survey of helminth, of a total of 5,288 fecal specimens examined, 86.7 percent were positive for one or more species of helminths. Trichocephalus trichiurus revealed the highest infection rate (83.6%), while Trichostrongylus orientalis was next (61.6%), and Ascaris lumbricoides was unexpectedly low(40.9%). The incidence for hookworm and Clonorchis sinensis was 22.4 percent and 29.8 percent respectively. Triple infections were found more frequently than double infections, and mixed infections of six or more species of helminths were observed in 3.8 percent of specimens.

Furfurol(Furfudol), a new anthelmintics, was administered to 7 adults and 21 children (6-12 years old) in total amounts of 15.6 mg(base) for 1 to 3 days. It has proven to be effective against hookworms. The anthelmintic effect against hookworms with a single dose of 15.6 mg of furfurol was similar to that of a single dose of 2.5 gm of bephenium hydroxynaphthoate. However, when a total of 15.6 mg of the base was given to both children and adult, the side reactions were so mild that the drug can be administered for therapeutic regimens in mass treatment. Minor abdominal discomfort was the only common side effect. This is the primary advantage of furfurol as compared with bephenium hydroxynaphthoate. These results have indicated that furfurol is safe in therapeutic dosage and is an effective agent to treat patients infected with hookworms.

It remains a problem that successful revascularization and reperfusion after ischemia are associated with high systemic complication rates and severe local tissue injuries. With prolonged ischemia, there is damage to tissue from anoxia, but further injury may occur after reperfusion. The activation of leukocytes and endothelial cells during reperfusion causes neutrophil adhesion in capillaries, resulting in plugging and further ischemia, Alternativety, neutrophil adhesion to endothelium leads to the migration of neutrophil with local edema formation, hemorrhage and thrombosis. Some chemotactic and activating factors are needed to propel neutrophils to the site of local inflammation. The chief cytikines that induce a pro-adhesive state in endothelium are tumor necrosis factor-alpha(TNF-alpha), interleukin-1 beta(IL-1 beta) and endotoxin or lipopolysaccharide(LPS). Similarly, TNF-alpha,and to a lesser extent interleukin-8(IL-8), is the important stimulus that acts on neutrophils and other leukocytes to alter their adhesion. The purpose of this study was to evaluate the pathogenetic role of IL-8 after perfusion with CDl8 monoclonal antibody(CDl8 mAb), the blocking antibody of neutrophil adherence, on reperfusion injury in rat epigastric island skin flap. A 6 X 3 cm-sized island skin flap was made on the abdomen. The epigastric pedicle was occluded for six hours with ambient temperature. Fifteen minutes before reperfusion, the flap was perfused with saline and CDl8 mAb(1 mg/kg). For evaluation of IL-8 levels, tissue fluid and serum were obtained at 4, 8, 12 and 24 hours after reperfusion. IL-8 concentrations of the CDl8 mAb group in the tissue fluid were significantly decreased at 8, 12 and 24 hours compared to the control group(P > 0.01), but the difference between the two groups was not significant at 4 hours after reperfusion IL-8 concentrations of the CDl8 mAb group in the serum were significantly decreased over time compared to the control group(P > 0.05, p > 0.01). Form the above results, we concluded that blocking neutrophil adherence using CD18 mAb within the peak level of IL-8 at 4 hours after reperfusion may be a better method of reducing reperfusion injury to the island skin flap.

The authors analyzed a series of 49 cases of spinal cord tumor confirmed by operation and biopsy at Presbyterian Medical Center, Jeonju, from Jan. 1977 to Dec. 1981. Histologically vertified spinal cord tumors wereneurofibroma(36.7%: 18/49), meningioma(22.5%; 11/49), ependymoma(18.5%; 9/49), metastasis(10.2%; 5/49),astrocytoma(6.1%; 3/49), epidermoid (2.0%;1.49), glioblastoma multiforme(2.0%;1/49), and diffuse non-Hodgkin'slymphoma(2.0%;1.49). The results were as follows; 1. The most frequently encountered tumor was the intraduralextramedulary tumor(55.1%;27/49). The other tumors, in descending order, were intramedullary tumor(26.5%; 13/49),extradural extradural extramedullary tumor(10.2%;5/49), and intra-&extradural extramedullary tumor(8.2%;4/49).The most frequently encoutered tumor was neurofibroma(36.7%; 18/49). Four of the 18 neurofibroma were intra-&extradural extramedullary tumor, and three(3/4) were dumbbell type of neurofibroma. 2. Only 3 cases occurred under20 years of age. They were glioblastoma multiforme, neurofibroma and epidermoid. 3. No sex predominance of spinalcord tumor was seen, but the majority of the meningiomas occurred in females(81.8%). 4. The most frequent site oftumors was the thoracic region(53.1%;26/49). The order sites were the cervical regions(2.0%;1/49) . 5. Thirteencases (26.5%) showed bony changs on preliminary roentgenograms of the spine. None showed bony changes inextradural extramedullary tumor. 6. As a rule, the myelographic deformities produced by intramedullary tumorshowed fairly long fusiform dilatation of the contrast column without displacement of the spinal cord; theextradural extramedullary tumor showed coarse transverse serrated block of the contrast column; and the intraduralextramedullary tumor showed cup defect of the contrast column.

An animal model was used to study the effects of normobaric hyperoxia on pulmonary mechanics and histopathology. Thirty two rats (227+/-46gm) were divided into control (Group 1) in room air and 3 hyperoxic groups (n=8, each). Hyperoxic groups breathed F1O2 0.98 for 24 hr(Group 2), 48 hr(Group 3), and 72 hr(Group 4), respectively. After hyperoxia, rats were intubated through tracheostomy, and inflation and deflation compliances of lung-thorax [CLT(I), CLT(D)] were calculated. Body weight(BW) was measured, and lungs were removed and weighed(LW). The lung specimens were examined microscopically. At the 3rd day of hyperoxia, 5 of 8 rats were died (63% mortality). CLT(I) of Group 4 was low (p<0.05) compared with others, and CLT(D) of all hyperoxic groups were low (p<0.05) compared with control. Mean LW/BW (lung weight divided by body weight) was elevated (p<0.05) in Group 3 and Group 4. Pulmonary pathologic findings such as inflammation, congestion, alveolar and bronchial wall damage, pulmonary edema etc. became apparent (p<0.05) at the 3rd day. We conclude that during hyperoxia pulmonary compliance might be decreased and lung weight might be increased progressively, and pulmonary pathological appearance of normobaric hyperoxia (F1O2 > 0.98) resemble those of adult respiratory distress syndrome (ARDS).
