3.An Ultrastructural Study on the Structural Development of the Cardiac Ganglion in the Human Fetuses.

The development of the cardiac ganglion was studied by electron microscopy in human fetuses ranging from 30mm to 270mm crown rump length. At 40mm fetus, the cardiac ganglia were observed in the adventitia of both the aorta and pulmonary artery, superior aspect of the left and right atrium, and interatrial septum. The cardiac ganglia were comprised of clusters of undifferentiated cells, neuroblasts, and unmyelinated nerve fibers. The ganglia were small and uncapsulated until 70mm fetus. At 70mm fetus, the cardic ganglia consisted of neuroblasts, satellite cells, and unmyelinated nerve fibers. Each ganglion was ensheathed in a connective tissue capsule. The cytoplasm of neuroblast contained Nissl bodies, mitochondria, coated vesicles, extensive Golgicomplex, and rough endoplasmic reticulum. Synaptic contacts between the cholinergic preganglionic axon and dendrites of postganglionic neuron were first observed. At 100mm fetus, the cardiac ganglia consisted of small clusters of ganglion cells and dendrites, together with supporting elements and blood vessels. During next prenatal stage from 170mm fetus, the ganglion cells were large and each contained a large nucleus with one or more nucleoli. The cytoplasm of ganglion cells contained much rough endoplasmic reticulum and extensive Golgi complex. Cholinergic preganglionic axons were numerous and interposed between the satellite cells. Adrenergic axons were rarely observed. A great number of synaptic junctions between the cholinergic preganglionic axon terminals and the dendrites of postganglinic neuron were found, and a few axosomatic synapses were also observed. Adrenergic nerve terminals did not seem to be involved in the synaptic transmission. The cardiac ganglion cells of the human fetal heart were innervated only by cholinergic nerve.

4.Biomechanical Study of the Partial Tear of the Rotator Cuff: Experimental Study in Rabbits.

Purpose of this study was to investigate the biomechanical fates of the partial thickness tear of the rotator cuff in rabbit, and try to provide guideline of treatment for the partial thickness tear of the rotator cuff tendon. Infraspinatus tendons of fifty rabbits were used for study. The rabbits were divided into three groups, twenty for one-third resection(superficial partial resection group), twenty for two-thirds resection(deep partial resection group) and ten for control group. Extraarticular portion of the infraspinatus tendons of the rabbits were resected partially with one-third or two-thirds thickness for each experimental group. We sacrificed five rabbits of each experimental group at 3, 6, 9, and 12 weeks after the resections, and five rabbits for the control group at 6 and 12 weeks. Average maximal load per area of the infraspinatus tendons was measured by tension test. There was significant difference between the deep partial resection group, 5.3 and 6.3 N/emat 6 and 12 weeks respectively, and the control group, 13.2 N/mm(P<0.05). There was no difference between the control and superficial partial resection group, 12.8 and 14.1 N/mm at 6 and 12 weeks, respectively. Shallow partial tears might be healed spontaneously. Deep partial tears would not be healed naturally. When surgical treatment is indicated for deep partial tears of the rotator cuff, tendon repair, rather than debridement, would provide more predictable results.

6.A case of pulmonary edema developed after intraarterial injection of iodinated contrast medium

Pulmonary edema is a rare adverse reaction to the iodinated contrast medium. Complaining of huge abdominal mass, a 52 years-old female was admitted to the Hangang Sungsim Hospital. On physical examination, the patient appeared to be healthy. She had stable vital Signs, i.e. BP: 120/80 mmHg, pulse rate: 80/min. etc. An adult head sized mass was palpated in the left mid and lower abdomen. Otherwise nonspecific. On laboratory studies, the positive findings were 8-10 WBC/HPF in urine, 25.6 mg/dl for BUN and PVC in EKG. It was negative for urine protein, serum creatinine and liver function test. We injected 100ml and 30 ml for Urografin 60 through the abdominal aortadividing 3 times and major branches of the abdominal aorta, respectively. Immediately after completing angiography, interstitial pulmonary edema was found, showing blurring of the vascular margins, perivascular haziness and thickening of the interiobular septal lines in the both lower lung fields. The blood pressure was dropped to 80/60 mmHg, but pulse rate was normal. She did not complian of dyspnea, and cyanosis was not developed. The urine volume was normally maintained. She was treated for pulmonary edema, which was completely absorbed after 20 hours. And the blood pressure was also normalized. We has experienced a case of pulmonary edema developed after intraarterial injection of the iodinated contrast medium without underlying cardiac, renal and hepatic problems, and reviewed the literatures on mechanisms of pulmonary edema caused by intravascular injection of the iodinated contrast materials.

7.The Effect of Neoadjuvant PI(Cisplatin, Ifosfamide) and PAIB(Cisplatin, Adriamycin, Ifosfamide, Bleomycin) in the Cervical Cancer.

For cervical cancer, the combination chemotherapy could increase the trial of radical hysterectomy due to reduction in tumor volume even in advanced stage and help to decrease corrected dose of radiotherapy. It also reduces postoperative radiotherapy to avoid the side effect and therefore make the quality of life better. Cisplatin chemotherapy alone was known to result in objective response rates of 20%. Todays animal experiment studies stated that Ifosfamide could accelerate the efficacy of Cisplatin. Thus in this study, neoadjuvant chemotherapy was applied to those cervical cancer patients in the use of the combination chemotherapy including Ifosfamide, PI(Cisplatin+ Ifosfamide) and PAIB(Cisplatin+ Ifosfamide+ Adriamycin+ Bleomycin). A total of 43 patients were entered in this trial, 23(53%) achieved at least a 50% reduction in tumor volume, 20(47%) showed no change. After completion of combination chemotherapy, 30(70%) of the 43 were done radical hysterectomy, 13(30%) given to radiotherapy. 9(30%) of the 30 patients were applied postoperative radiation therapy. Nausea and vomiting occurred in 35 of the 43 patients(81%) and alopecia 15(34%) for toxicity. Severe CNS toxicity(Grade 4) was detected in one patient. This study suggest that Ifosfamide combination chemotherapy is effective in tumor reduction and minimal toxity and can performed radical operation in cases of large tumor volume.

8.Treatment of Radiation Induced Hemorrhagic Cystitis: Angiographic Embolization of Hypogastric Arteries.

PURPOSE: We assessed the long term efficacy and complications of angiographic embolization of hypogastric arteries in treating the intractable bladder hemorrhage induced by radiation. MATERIALS AND METHODS: From January 1990 to December 1997, a total of 43 patients with radiation induced hemorrhagic cystitis were evaluated. Of 43 patients, 5 patients were treated by bilateral selective angiographic embolization of the anterior branches of the hypogastric arteries. The embolic material used in all patients was gelfoam. RESULTS: Five patients with intractable bladder hemorrhage were treated by bilateral selective embolization of the anterior branches of the hypogastric arteries by gelfoam and successful in completely stopping or considerably decreasing intractable bladder hemorrhage in all patients. The average number of treatments was 1.6(range 1 to 2). There was no significant sequelae ocurred as a consequence of simultaneous bilateral occlusion of the hypogastric arteries. CONCLUSIONS: Radiation induced hemorrhagic cystitis that do not respond to irrigations with chemical cauterizing agents or transurethral fulguration of bleeding vessels can be successfully treated with bilateral selective angiographic embolization of the anterior branches of the hypogastric arteries.

9.Lower Caliceal Stone Clearance after Shock Wave Lithotripsy: Impact of Lower Pole Radiographic Anatomy.

PURPOSE: We analyzed our experience to determine the influence of infundibulo-pelvoureteral angle, lower calyceal width and length in predicting the clearance of fragments after extracorporal shock wave lithotripsy(ESWL) for lower calyceal stone. MATERIALS AND METHODS: From October 1995 to April 1998, a retrospective analysis of 51 patients with a solitary radiopaque lower pole calyceal stone who underwent excretory urography(IVP) preoperatively were treated ESWL with a Storz Modulith SLX third generation. RESULTS: The overall stone-free rate was 54.9%. Stone-free status after ESWL was significantly related to each anatomical measurement. Of the stone-free and not stone-free groups, with the infundibulo-pelvoureteral angle 40 degree or greater 21 patients were found in stone-free groups(75%) and 5 patients in not stone-free groups(21.7%), with the infundibular width >5mm 22 patients were found in stone-free groups(78.6%) and 11 patients in not stone-free groups(47.8%) and with the infundibular length < or =30mm 16 patients were found in stone-free groups(57.1%) and 5 patients in not stone-free groups(21.7%), respectively. CONCLUSIONS: We recommend that the 3 major radiographic features of the lower pole calix should be assessed during intravenous urography to facilitate the planning of treatment for lower calyceal stones.
