3.Choledochal cyst with ectopic distal location of the papilla of Vater.

In cholangiographic techniques, the close relationship between choledochal cyst and anomalous union of pancreaticobiliary duct has attracted medical attention. There have been rare cases in which the papilla of Vater was found in a position other than its normal position, and such cases have been reported sporadically. However, such cases are interesting in the anatomical context. In this review, we present our experience of choledochal cyst in a 30-month-old boy in whom the papilla of Vater was positioned in the third portion of the duodenum.

4.Home Treatment of Rotator Cuff Disease.

We treated 617 patients who were diagnosed as rotator cuff disease at the Department of Orthopaedic Surgery, Asan Medical Center in Korea between March, 1995 and December, 1996. We evaluated 175 cases who were followed up over five months and were treated according to our home treatment protocol. Average follow-up period was eleven months(range, five to eighteen months). We educated every patient who were not suspected of having full thickness tear of the rotator cuff based on clinical and MRI findings. According to patients condition, we treated them with both stretching and strengthening exercise or strengthening exercise only. This physician-directed home treatment was occasionally supported by the use of NSAID in case severe pain. Patients did stretching and strengthening exercise in 93(53%)cases, and strengthening exercise initially in 82(47%)cases. One hundred and forty five(83%) cases had satisfactory results. Home treatment of the rotator cuff disease is effective, convenient and not dangerous. We recommend this physiciandirected home treatment for primary management of almost all the patient with rotator cuff disease except full thickness tear of the rotator cuff tendon.

5.Significance of the AFP Level and HBsAg in Differentiation of Hepatic Masses.

Hepatic masses show different enhancing patterns in N bolus computed tomography: Hepatocellular carcinoma shows high-attenuation in the early enhancing phase and low-attenuation in the late enhancing phase, hemangioma shows peripheral dot-like high-attenuation in the early enhancing phase and central high-attenuation in the late enhancing phase, and metastatic cancer and cholangiocelluar carcinoma show peripheral high-attenuation rim in the early enhancing phase and central portion gradulally high attenuation in the late enhancing phase. but sometimes enhancing patterns of the hepatic masses are confuse. To evaluate the significance of the AFP level and HBsAg in differentiation of the hepatic masses, we retrospectively analyzed AFP level and HBsAg' in 228 pathologically or radiologically confirmed hepatocellular carcinomas, and 137 pathologically nonhepatocellular cacinomas. The results were as follows In hepatocellular carcinoma, AFP level above 20ng/ml was 77.8% and HBsAg positve was 72.6%. In nonhepatocellular carcinoma, AFP level above 20ng/ml was 3.7% and HBsAg positve was 16.1%. We concluded that AFP level and HBsAg are helpful to distinguish hepatocellular carcinoma from nonhepatocellular carcinoma, when IV bolus computed tomogram finding is uncertain.

8.Psychological Study of Psoriatic Patients.

The subjects of the present study were 111 patients admitted to a hospital and 304 healthy people living in similar condition. A self report symptom inventory, SCL-90(symptom check list-90) was used and the two groups were analysed and compared by many factors. The results of theis study are as follows. 1. All dimensional values of the psoriasis group were much higher than the normal healthy group. In order of significance: depression, anxiety, obsessive-compulsive, psychotism, interpersonal sensitivity, hostility, and somatization(all p<0.01). This result indicates a significantly heightened incidence of psychological problems among psoriatic patients. 2. There were significant differences between each clinical type. As a whole, common and pustular types showed highter dimensional scores than guttate type. 3. In the psoriasis group, subjects over 30 years old had higher scores except in somatization and paranoid ideation dimensions. However there was no significant relationship between age and dimensional scores. 4. Female psoriatic patients showed higher values than male patients on all dimensions excopt paranoid ideation. T verification showed significance in depression and hostility (p<0.05). 5. Analysed by marital status, generally unmarried patients had higher dimensional scores than married patients, but there was no significant differences. 6. In contrast with the control group, psoriatic patients of low educational status showed higher dimensional scores than patients of high educational status. 7. Analysed by job at onset of disease, student patients showed the lowest scores. There were significant differences for somatization(p<0.01) and paranoid (p<0.05) dimensional scores. 8. Analysed by the age of onset of disease, the patients who developed psoriasis before 20 years of ages showed higher dimensional scores than the patients, after 20 years of age. 9. Compared according to duration of the disease, dimensional scores rose in patients under 1 year duration decreased in patients of 1 to 2 years' duration and rose again in parients over 3 years' duratio. 10. As the accuracy of understanding about the disease decreased, the dimensional scores tended to be higher.

Castleman disease is a benign disorder, usually occurring within mediastinum, characterized by proliferation of lymphold tissue. The authors report a rare case of Castleman disease originating from mesenteric root. The tumor was highly vascular, proved by dynamic CT examination and splanchnic angiography.

10.Perforation of idiopathic small bowel ulceration after blunt abdominal trauma in a child: a Case Report.

Idiopathic small bowel ulceration occurring beyond the duodenum is rare, and less than 5% of all the reported cases have occurred in children. In most of the cases, single ulcer of unknown cause is observed in jejunum or ileum. The diagnosis is difficult and usually made at the time of surgical exploration for the complications, such as perforation, hemorrhage or obstruction. We experienced a case of perforation of idiopathic ileal ulceration due to blunt abdominal trauma in a 11-year-old boy. The ileal segment including the lesion was resected and the pathologic findings were compatible with idiopathic small bowel ulceration. The clinical and pathological aspects are discussed, and the literatures were reviewed.
