2.Charcoal-induced anthracosis of the terminal ileum.

Anthracosis is a very common disease of the bronchus, while anthracosis of the gastrointestinal tract is extremely rare. Only a few cases of anthracosis of the esophagus have been reported and no cases of anthracosis of the terminal ileum are known, except one patient in Korea who was described as having melanosis ilei. A black pigmented lesion was detected in the terminal ileum on colonoscopic examination of a 51-year-old woman. Histological examination revealed a pigmented lesion beneath the mucosal epithelial layer and lymphoid follicular hyperplasia. The lesion consisted of an aggregation of histiocytes containing abundant tiny black pigments. She has taken oral charcoal for 7 years. We diagnosed anthracosis of the terminal ileum caused by oral charcoal and report a case of charcoal-induced anthracosis of the terminal ileum.

3.Consecutive Reversible Changes of Peripheral Nerve Conduction in Tetrodotoxification.

Tetrodotoxin (TTX) cause neurologic dysfunction by blocking the voltage-gated sodium channels located in all of the peripheral nerves and muscles. We experienced two patients presenting with generalized motor weakness after ingestion of pufferfish. The nerve conduction study showed diffuse slowing of motor and sensory nerve conduction velocity, prolonged motor terminal latency and decreased sensory nerve action potentials without temporal dispersion or conduction block. Abnormal findings of nerve conduction study improved rapidly without any deterioration. Clinical symptoms and signs ameliorated in accordance with findings of nerve conduction study without any neurologic sequelae. These imply that tetrodotoxication is reversible and functional neurologic disorder. We suggest that nerve conduction studies can be available in serial monitoring of tetrodotoxication as an objective means.

5.The Effect of Amniotic Membrane on the Fibrosis between Orbital Connective Tissue and Porous Polyethylene Sheet Iimplant.

PURPOSE: Soft tissue or muscle adhesion to the Porous Polyethylene Sheet Implant (PPSI) can cause postoperative extraocular motility disturbance after orbital fracture repairs using PPSI. This study was designed to examine the efficacy of amniotic membrane transplantation in the suppression of fibrosis between orbital connective tissue and PPSI. METHODS: Twelve New Zealand white rabbits were grouped as A, B, and C and conventional PPSI were implanted between orbital connective tissue and orbital floor in group A; PPSI with a barrier surface (PPSI-B) in group B; PPSI with amniotic membrane transplantation in group C. The implants were harvested at 8 weeks postoperatively, and stained with Masson trichrome and hemaoxylin and eosin staining techniques. RESULTS: In group A, the PPSI demonstrated dense adhesion on both the soft tissue implant and the bone implant interfaces. A lot of fibroblasts were observed on the surface of PPSI. In group B, the PPSI-B demonstrated less fibrosis over the barrier surface, and less fibroblasts than in group A. In group C, the PPSI with amniotic membrane transplantation manifested least fibrous adhesion among three groups. CONCLUSIONS: In orbital fracture repair, amniotic membrane transplantation could reduce the fibrosis between orbital connective tissue and PPSI, and might prevent the postoperative extraocualr motility disturbance.

6.Effects of Endogenous Nitric Oxide Synthesis Inhibition on the Depressor Response to Intracerebroventricular Calcium.

BACKGROUND: Aside from its well known peripheral antihypertensive effects, calcium also lowers blood pressure, when administered into the cerebral ventricle. The present study was aimed to determine whether the central depressor response to calcium is mediated by a stimulation of endogenous L-arginine-nitric oxide (NO) pathway. METHODA: Mean arterial pressure (MAP) and heart rate (HR) were continuously recorded from the femoral artery in anesthetized rats. Administration of calcium was performed into the right lateral cerebral ventricle. The effects of N G-nitro-L-arginine methyl ester (L-NAME) on the cardiovascular response to calcium were examined. RESULTS: Intracerebroventricular (ICV) injection of calcium consistently produced a decrease in MAP and HR. The depressor and bradycardiac responses to calcium showed a dose-dependent fashion. Pretreatment with a calcium channel blocker, diltiazem (1 micromol, ICV), attenuated cardiovascular responses to calcium. ICV infusion (1 microl/min) of L-NAME (200 microgram/kg and 20 microgram/kg/min for 60 min) increased MAP without significant changes in HR. Chronic ingestion of L-NAME (5 mg/100 ml in drinking water, 4 weeks) also increased the systolic blood pressure as compared with control. The depressor effect of ICV calcium was significantly diminished in acute or chronic L-NAME treated rats. CONCLUSION: These findings suggest that the central depressor response to calcium, at least in part, is NO-dependent.

7.The Postoperative Recovery of Ocular Motility in Pediatric Blow-out Fracture.

PURPOSE: To evaluate the clinical presentation, type of fracture, time of intervention and postoperative restoration of ocular motility disturbance in pediatric blow-out fracture. METHODS: The authors reviewed medical records of 30 consecutive cases which were operated for blowout fracture from January 1998 to December 2000. RESULTS: The most common cause of injury was violence (80%). The most common location of fracture was inferior wall (50%), followed by inferior-medial wall (30%). The most common type of fracture was trap door type fracture. The trap door type fracture showed much severe ocular motility disturbance preoperatively. Six cases of trap door fracture showed ocular motility restriction postoperatively. Early surgical intervention (within 5 days) resulted in a more complete return of ocular motility compared with the late intervention group (p<0.05). CONCLUSIONS: Trap door fracture was the most common type of orbital fracture in the pediatric age group. Early surgical intervention within 5 days will improve ocular motility postoperatively.

8.The Prognostic role of electrocardiographic signs of cor pulmonale in chronic obstructive pulmonary disease.

BACKGROUND: In patients with chronic obstructive pulmonary disease(COPD), several factors have been associated with a poor prognosis. These include old age, low FEV, low diffusing capacity, high alveolar-arterial oxygen pressure difference, and finally cor pulmonale. This study was done to investigate in the ECG signs suggesting cor pulmonale were independent prognostic factors in patients with COPD. METHOD: We analyzed ECG, pulmonary function data and arterial blood gas values in 61 patients who were admitted through the emergency department with an acute exacerbation of COPD. The ECG signs reflecting cor pulmonale were right atrial overloading(RAO), right bundle branch block, right ventricular hypertrophy and low-voltage QRS. The 61 patients were divided into 2 groups, group I with no ECG signs(n=36) and group II with one or more ECG signs(n=25) suggesting cor pulmonale. RESULTS: Poor prognostic factors by univariate analysis were low FEV1, FEV1 % pred., VC% pred., DLco, DLco % pred., PaO2 and SaO2 high PaCO2 presence of ECG signs reflecting cor pulmonale, presence of mental status change, use of mechanical ventilator, and long tern use of glucocorticoid. A multivariate analysis indicated that age(risk ratio=1.13, 95% confidence interval 1.05~1.23), Dlco % pred. (risk ratio=0.97, 95% confidence interval 0.94~0.99), PaO2 (risk ratio=0.95, 95% confidence interval 0.90~0.99) and RAO(risk ratio=5.27, 95% confidence interval 1.40~19.85) were independent prognostic factors of survival. There was a significant difference in survival between the patients with and without RAO(p=0.038). The survival rates at 1, 2, and 5 years were 94.5%. 81.4%, and 50.0% in patients without RAO and 82.4%, 70.6%, and 27.5% in patients with RAO, respectively. CONCLUSION: These results suggest that the presence of ECG signs reflecting cor pulmonale is a predictor of survival and that RAO of these ECG signs is a significant independent predictor of survival in patients with COPD.

Systemic lupus erythematosus(SLE) is a systemic disease of unknown etiology. Its main pathology is vasculitis and serositis, due to deposition of the immune complex or antibodies. Most findings are nonspecific ; abdominal manifestations include enteritis, hepatomegaly, pancreatic enlargement, serositis, lymphadenopathy, splenomegaly,nephritis, interstitial cystitis, and thrombophlebitis. We described radiologic findings of various organinvolvement of SLE; digestive system, serosa, reticuloendothelial system, urinary system, and venous system.Diagnosis of SLE was done according to the criteria of American Rheumatism Association. Understanding of thevariable imaging findings in SLE may be helpful for the early detection of abdominal involvement andcomplications.

10.Prevalence of alpha1-Antitrypsin Genotypes in Koreans.

BACKGROUND: Alpha-1-antitrypsin (A1AT) deficiency is the only established genetic resk factor for emphysema. This study was undertaken to investigate the prevalence of the genotypes of A1AT genotypes in healthy Koreans. METHOD: The study population consisted of 380 healthy Koreans enrolled at the Health Promotion Center in Kyungpook National University Hospital. The polymerase chain reaction (PCR) and restriction fragment length polymorphim (RFLP) for detecting the A1AT variants M1(Ala), M1(Val), M2, S and Z were used. RESULTS: The genotypes of subjects were as follows : M1(Val)/M1(Val), 254(66.8%) ; M1(Val)/M2, 105(27.6%) ; M2/M2, 19 (5.0%) ; and M1(Val)/M1(Ala), 2 (0.5%). There was no case with 'deficiency' alleles such as S and Z found in this study. CONCLUSION: These results suggest that A1AT deficient alleles are either extremely rare or not present in Koreans.
