A functional upper airway obstruction due to a vocal cord dysfunction(VCD) is characterized by a paradoxical adduction of the vocal cords throughout the respiratory cycle with no obvious organic cause for the obstruction. It commonly occurs paroxysmally and imitates acute asthmatic attacks, often in patients with coexisting asthma. They present with episodes of dyspnea associated with inspiratory wheezing that persists despite conventional asthma treatment and a flattening of the inspiratory limb of the flow-volume curve; an adduction of the vocal cord during inspiration. Failure to recognize concurrent vocal cord dysfunction and asthma has led not only to the excessive use of bronchodilators and corticosteroids, but also to intubation and tracheostomy. Here, we report a case of coexistent obstructive pulmonary disease and functional upper airway obstruction due to a vocal cord dysfunction where a bronchoscopy showed a paradoxical vocal cord motion and typical features of a variable extrathoracic obstruction and a lower airway obstruction on the Flow-volume loop during a symptomatic period.

BACKGROUND: Few studies have investigated magnetic resonance (MR) characteristics of traumatic posterosuperior rotator cuff tears involving the supraspinatus and infraspinatus. We hypothesized that traumatic rotator cuff tears may have MR characteristics distinguishable from those of non-traumatic tears. METHODS: Preoperative MR arthrography and intraoperative tear size measurements were compared in 302 patients who underwent MR arthrography and subsequent arthroscopic rotator cuff repairs for traumatic (group T, 61 patients) or non-traumatic (group NT, 241 patients) tears. The inclusion criteria for both groups were posterosuperior full-thickness rotator cuff tear and age between 40 and 60 years. For group T, traumas were limited to accidental falls or slips, or sports injuries, motor vehicle accidents; injuries were associated with acute onset of pain followed by functional shoulder impairment; and time between injury and magnetic resonance imaging (MRI) was 6 weeks or less. RESULTS: In group T, 72.1% of shoulders (44 patients) had tendon tears with blunt edges while 27.9% of shoulders (17 patients) had tears with tapering edges. In contrast, 21.2% of patients in group NT (51 patients) had blunt-edge tears, while 78.8% (190 patients) of tears had tapering edges. These results were statistically significant (p<0.001) and estimated odds ratio was 9.6. The size of tear did not vary significantly between groups. CONCLUSIONS: We found no exclusive MR characteristic to define traumatic tears. However, oblique coronal MRI of traumatic tears showed a significant tendency for abrupt and rough torn tendon edges and relatively consistent tendon thicknesses (without lateral tapering) compared to non-traumatic cuff tears.

PURPOSE: This study was performed to evaluate differentiating features of spinal compression fractures between benign and metastatic lesions. MATERIALS AND METHODS: We reviewed MR imaging in 52 patients (benign 38, metastasis 14) with vertebral compression fracture. Signal intensity of fracture and uninvolved areas, presence of contrast enhancement, fragmentation, and paravertebral mass were analyzed retrospectively. RESULTS: Signal intensity of fracture site was variable in benign lesions, but low signal intensity on T1-weighted image and high on T2*-weighted image were seen in all cases of metastasis. Signal intensity of uninvolved area was high on T1-weighted image and low on T2*-weighted image in 84% of benign lesions. On the contrary, normal marrow signal intensity was not seen in the uninvolved areas of all metastatic fractures. Contrast enhancement were observed in all cases of benign & metastatic compression fractures. Fragmentation were seen in 1 case of metastasis(7%) and in 11 cases of benign lesions(29%). Paravertebral mass were seen in 5 cases of metastasis(36%) and in 7 cases of benign lesions(18%). CONCLUSION: Presence of normal marrow signal intensity in the uninvolved area of fracture site could be the most useful sign for differentiating benign causes from metastasis.

Parricide, the crime of murdering a parent, accounts for about 5% of all homicides. Filicide is the crime of murdering one's own child. This study aimed to review demographic features and criminal characteristics of individuals who committed parricide and filicide in Republic of Korea (ROK). This study is based on data from the Korea Police Crime Analysis System, from 2006~2013. We assessed the diverse characteristics of both victims and perpetrators. Over the selected period, 381 parents were killed by their children and 230 children were killed by parents in the ROK. Parricides caused by schizophrenic murders accounted for 39.6% of all cases. Moreover, approximately 44.4% of the perpetrators attempted suicide following the maternal filicide. In our findings, psychiatric illness was a very important predictor in parricide, and these further suggest that young mothers with severe mental illness require careful monitoring by mental health support service.

To attempt a rigorous definition of the structure of the accessory spleen (AS) in the Chinese hamster, we examined twenty-one animals, and found AS in 5 animals (23.8%), which were over 7-month-old. The AS had no connection with the main spleen and was seen as a dark red oval organ (0.7 mm x 1.5 mm), which was embedded in the adipose tissue near the tail of the pancreas. It was demarcated from the adipose tissue and some pancreatic tissue. The organ was encapsulated by thin collagenous connective tissue and smooth muscle fibers, and contained lymphatic nodules, reticular fibers, nodular central arterioles, macrophages and megakaryocytes. Notably the incidence of AS appeared to increase with age in the Chinese hamsters.

PURPOSE: Exogenous surfactant replacement therapy reduces morbidity and mortality rates for premature babies with respiratory distress syndrome (RDS). A significant increase in clinical pulmonary hemorrhage has been reported in premature babies treated with surfactant. Pulmonary hemorrhage, one of the major cause of death, may develop due to an increased pulmonary blood flow. We investigated the prognostic factors for mortality in premature infants with pulmonary hemorrhage after surfactant replacement, and to predict the survival of infants having RDS. METHODS: We investigated the characteristics and clinical courses of the 22 premature infants who developed pulmonary hemorrhage after surfactant therapy for RDS hospitalized at Seoul Red Cross Hospital between Dec 1994 and May 1998. We divided the patients into two groups ' Group I were those who survived (n=6) and Group II were those who expired after pulmonary hemorrhage (n=16). RESULTS: There were no differences in birth weight, gestational age and radiologic staging between two groups (P>0.05). Low Apgar score at birth was significantly related to mortality in premature infants with pulmonary hemorrhage (P<0.05). The interval between birth and onset of pulmonary hemorrhage and the interval between the surfactant replacement and onset of pulmonary hemorrhage were significantly longer in Group I than in Group II (P<0.05). CONCLUSION: The clinical conditions at birth, the interval between birth and onset of pulmonary hemorrhage, and the interval between surfactant replacement and onset of pulmonary hemorrhage were the prognostic factors of mortality in preterm infants with pulmonary hemorrhage.

PURPOSE: This study was conducted to investigate the effects of blood flow restriction and different support surfaces for bridge exercises on the thickness of the transverse abdominis and multifidus, which are trunk-stabilizing muscles. METHODS: The study's subjects were 45 adults who were divided into three groups that performed bridge exercises over a six-week period on their respective support surfaces after blood flow restriction. Changes in the thickness of the subjects' transverse abdominis and multifidus muscles were measured using ultrasonography before the experiment, then three and six weeks after the experiment. The changes in each variable over time were analyzed using a repeated-measures analysis of variance (ANOVA). RESULTS: The transverse abdominis showed significant differences in muscle thickness with regard to time and the interaction between time and each group (p<0.05), but no significant differences with regard to changes among groups (p>0.05). The multifidus showed significant differences in muscle thickness with regard to time, the interaction between time and each group, and changes between groups (p<0.05). CONCLUSION: Blood flow restriction and different support surfaces for bridge exercises led to significant differences in the thickness of the transverse abdominis and multifidus. This study's results may be used as the basis for future studies and for rehabilitation in clinical practice.
