1.Clinical Study and Skin Tests of Patients with Drug Eruption.

BACKGROUND: Although drug eruption refers to a common dermatologieal condition, there are numerous setbacks in the diagnosis. In addition there are no annual average prevalence rates of drug eruptions according to time sequence in Korea. OBJECTIVE: This study attempts to detme the clinical features and causative drugs of drug eruptions, the diagnostic value of various skin tests and to evaluate the dome,stic annual average prevalence rate of drug eruptions according to time sequence. METHOD: 270 patients with drug eruptions were reviewed clinically. In 103 patients, intradennal, patch and scratch-patch tests were performed using suspected drugs. The annual average prevalence rate of drug eruptions was calculated from 10 different general hositals in Korea. RESULT: Exanthematous eruptions(49.3), fixed drug eruptions(30.1), and urticaria(10.0%) were the common types of drug eruptions. The common causative agents were antibiotics/antimicrobials, antipyretics(NSAIDs, and CNS depressants. 52 patients out of 103 had positive results after at least one skin test with suspected drugs. CONCLUSION: In our study, we were ahle to obtain somewhat meaningful results basnl on data from the combination of various skin tests. As a result, physicians need to prescrible drugs with care.

2.Pigmented Photoallergic Contact Dermatitis Due to Musk Ambrette.

Pigmented photoallergic contact dermatitis is a disease usually resulting from recurrent cutaneous exposure to cosmetic photoallergens which produce bizarre dark brown hyperpigmentation. Histologicnl examination of this condition reveals liquefaction of the basal cells of the epidmis and melanophages in the upper dermis. A 50-year-female developed dark brownish mottled hyperpigmented patches on her face after using a soap named O.E. and taking a nap every afternoon for 4 years. The histological fmding on the lesion site was consistent with pigmented photoallergic contact dermatitis. The cosmetic photopatch test with, O.E. soap, and the Scandinavian scries resulted in a positive reaction only on one side where 5 J/cm2 lights were applied by using Waldman 800UV machine. The other side howed a negative finding. We fmally diagnosed pigmented photoallergic contact dermatitis based on the patients histoty of using the soap containing musk ambrette, a histological examination of a biopsy and the results of photopatch tests. We report rare case of pigmented photoallergic contact dermatitis.

3.Personification of plegic limb following right hemispheric stroke: A case report.

We report a case of 53-year-old woman with personification following right middle cerebral artery territory infarction. Although she knew that her paralyzed left limbs belonged to her, she behaved as if it is a separate person. Verbalization of hatred toward her left arm was frequently observed. She attributed her neurologic deficit to the left arm instead of herself, frequently complaining in a fashion that 'I(a person except for the left arm) am all right but he/she(left arm) is wrong'. This delusional beliefs were mainly confined to her paralysed left limb, and were accompanied by left spatial neglect, transient anosognosia for hemiplegia and right-left disorientation.

In carpal injury, a good functional end result seems to correlate with final anatomic alignment of carpus after treatment. Some cases are often misdiagnosed and result in inadequate treatment. Forty six cases of carpal injuries were analysed which were treated for 9 years from 1975 to 1984 at the Department of Orthopedic Surgery, Seoul National University Hospital. The observations can be summarized as follows. 1. The male to female ratio was 3.5: 1 and 82% of total cases were in the third and fourth decades. 2. The most frequent cause of injury was fall from height with outstretched hand. 3. Trans-scaphoid perilunar dislocation and its variant were the most common diagnosis in this analysis. 4. All of the scapho-lunate dissociation showed volar flexion intercalated segmental instability with scapho-lunate gap more than 2 mm in our series. 5. In 11 cases of trans-scaphoid perilunar dislocation and its variant, carpal collapse was found in 10 cases. In 5 cases dorsiflexion intercalated segmental instability pattern and in one case volar flexion intercalated segmental instability pattern were found to be present. 6, There was an increase in scapho-lunate angle in all cases of dorsiflexion intercalated segmental instability, but a decrease in scapho-lunate angle is not a consistent finding in volar flexion intercalated segmental instability. 7. In 24 cases more than 6 months follow-up, excellent and good results were obtained in 20 cases. 8. As a result of above findings, authors conclude that scapho-lunate gap more than 2mm has diagnostic value in scapho-lunate dissociation but that dorsiflexion intercalated segmental instability or volar flexion intercalated segmental instability only means the changes in the relationship between carpal bones and does not have definite or specific diagnostic value for a specific carpal injury.

6.Failed TKA with Titanium-Induced Tissue Response

Titanium alloy has been widely used as material for total knee arthroplasty(TKA). However, titanium alloy has frequently been reported as causing black staining of the periprosthetic tissues as a result of debries from metallic wear and tissue reaction. We report six cases of failed TKA in five patients, in which the tibial tray and/or metal backed patellar components were composed of titanium alloy. Wear, breakage or separation of the ultra-high molecular weight polyethylene of tibial insert and/or patellar component, which allowed metal-on-metal friction and aggravated the production of metal wear debries, were observed in all cases. The average time from primary operation to revision was 5 year and 3 months (range, 9 months to 8 year and 8 months). In all cases, patients showed painful swelling of knee jont and radiopaque periarticular deposits on radiogrphs. Most of radiographs showed abnormal position of the metallic components and metal line sign. Histology of synovium showed abundant titanium particles lying free and within histiocytes, and polyethylene and cement particles with foreign-body type giant cell reaction. The synovial lining showed a preliferative reaction and abundant fibrosis was noted. On polarizing briefringent. Elemental analysis of metal particles on histologic section, using energy dispersive X-ray spectrophotometer (EDS ; EDAX PV9900) unit attached to a transmission electron microscope, detected the titanium peak. Radiopaque metallic densitien in the painful knee after TKA are highly suggestive findings of titanium reaction. Tissue reaction in response to the metal wear debries from titanium prosthesis may have contributed to the early failure of these implants.

Enteric fistulae result from gastrointestinal perforations in which communication is established between the site of perforation and another hollow viscus, potential space, or skin surface. Certain types of enteric fistulae are difficult to demonstrate by conventional radiographic methods, and CT is unique in its ability to demonstrate the extent and nature of extraluminal changes. The purpose of this study is to illustrate the CT findings of enteric fistulae occurring in a variety of abdominal and pelvic organs.

9.Influence of surface treatment on the insertion pattern of self-drilling orthodontic mini-implants.

OBJECTIVE: The purpose of this study was to compare self-drilling orthodontic mini-implants of different surfaces, namely, machined (untreated), etched (acid-etched), RBM (treated with resorbable blasting media) and hybrid (RBM + machined), with respect to the following criteria: physical appearance of the surface, measurement of surface roughness, and insertion pattern. METHODS: Self-drilling orthodontic mini-implants (Osstem implant, Seoul, Korea) with the abovementioned surfaces were obtained. Surface roughness was measured by using a scanning electron microscope and surface-roughness-testing machine, and torque patterns and vertical loadings were measured during continuous insertion of mini-implants into artificial bone (polyurethane foam) by using a torque tester of the driving-motor type (speed, 12 rpm). RESULTS: The mini-implants with the RBM, hybrid, and acid-etched surfaces had slightly increased maximum insertion torque at the final stage (p < 0.05). Implants with the RBM surface had the highest vertical load for insertion (p < 0.05). Testing for surface roughness revealed that the implants with the RBM and hybrid surfaces had higher Ra values than the others (p < 0.05). Scanning electron microscopy showed that the implants with the RBM surface had the roughest surface. CONCLUSIONS: Surface-treated, self-drilling orthodontic mini-implants may be clinically acceptable, if controlled appropriately.

10.Rocuronium and Lidocaine Pretreatment for Prevention of Biochemical Changes, Fasciculations and Myalgia following Succinylcholine Administration.

BACKGROUND: The purpose of this study was to assess the changes in serum potassium and creatine kinase concentrations and the incidence of fasciculations and myalgia when rocuronium and lidocaine were used in combination and separately as pretreatment before succinylcholine. METHODS: We studied 60 patients undergoing a minor elective surgery, in a prospective double blinded method. Three groups each with 20 patients were pretreated before a 1.5 mg/kg succinylcholine administration with 0.05 mg/kg rocuronium three min before (group R), 1.5 mg/kg lidocaine 30 sec before (group L), or both rocuronium and lidocaine (group RL). Serum potassium and creatine kinase were respectively measured 5 min after succinylcholine administration and 24 h after the operation. Fasciculations and myalgia on postoperative day 1 and day 2 were evaluated. RESULTS: The increase in creatine kinase and incidence of myalgia on postoperative day 1 was less in the RL group than in the R group and L group. The incidence of fasciculations was higher in the L group than in the R group and RL group. There was no increase in serum potassium in any group. CONCLUSIONS: The combined use of rocuronium and lidocaine was more effective in reducing creatine kinase and postoperative myalgia than when they were used separately. However, the fasciculations were only reduced by the use of rocuronium.
