1.Clinical Study and Therapeutic Experience in Tinea Versicolor ( 3 ).

BACKGROUND: Tinea versicolor may be treated by topical oroal antifungal drugs. OBJECTIVE: Ihe objertive of this study was to evaluate the their, peutic effect and safety of oral itraconazole(100-200mg/day), cloconazole cream, 20% sodium chiosulfate solution, sulconazole cream and 2% ketoconazole shampoo in Tinea versicolor. METHODS: Forty patients with Tinea versicolor were studys clinically and treated in five groups, with oral itraconazxale(100-200mg/day), cloconazole crean 20% sodium thiosulfate solution, sulconazole cream and 2% ketoconazole shampoo, from JuIIy 1991 to June, 1994. The writers also compared the results with the writers previous similar studies in 1986, 1990. RESULTS: The average age was 29.9 years, the oldest patient were 52 year-old and the youngest patient was a 14 year-old. The ratio of male to female a 1.2: 1. The distributions of lesions was most common in the axilla(26.9%), and the other sitsuere the anterior chest(19.2%), the back(17.3%), the neck(15.4%), the upper extremites(9.6%) the abdomen(7.7%) and the lower extremites(3.9%), in order. The incidence of hyperpigrietted lesions was 76.8% and of hypopigmented lesions was 23.2%. The average duration of treatment until cure was 3.00+0.55 weeks in the oral itraconaxile treated group, 3.31+1.18 weeks in the cloconazole cream group, 3. 25+1.03 weeks in the 20%, sodium thiosulfate solution group 3.45+0.52 weeks in the sulconazole cream group and 3.40+0.33 weeks in the 2% ketoconazole shapen group. Mere were no statistically significant differences of therapeutic efficacy in each the five groups(p>0.05). Compared with the study in 1990, the average age was higher from 5.7 to 29.9 years, and the male to female ratio was slightly less, showing a relative increase in male incidence. But, there was no statistical difference in therapeutic efficacy(p>0.05). CONCLUSION: This study was shown that the above five theraieric regimens can be used safely and simply, according to patients preferences.

2.Treatment of Palmoplantar Hyperhidrosis with Iontophoresis.

In the present study, tap water iontophoresis as a method of treatment of palmoplantar hyperhidrosis was evaluated. Eighteen patients, twelve males and six females, with singnificant palmoplantar hyperhidrosis were treated on every weekday for 2 weeks. The hand and foot of the same side were randomly selected for the treatment by tap water iontophoresis according to the the method described by Levit. The selected hand and foot were immersed in a pair of pans containing tap water and exposed to 30V and 3 to 14mA of direct current for 20 minutes. The results obtained were as follows . Among 18 patients treated, 14(77.8%) accomplishei3 sufficient control of hyperhidrosis after 6 12(average 9.1) treatments. Two patients showed a moderate response and one weak response. One patient showed no response. Several patients experienced untoward effects such as mild itching, erythema, tingling sensation, exfoliation, compensatory hyperhidrosis, but the discomfort was not so severe as to stop the treatment

3.Etiological Classifications of Children with Chief Complaint of Short Stature.

Purpose : As the recombinant human growth hormone has been widely available, a lots of parents having short statured children are interested in promoting growth of them whatever the etiologies of short stature they have. However, the growth hormone therapy for growth-promoting effect is only justified in well-established indications such as growth hormone deficiency, fumer syndrome, and chronic renal insufficiency. This study was undertaken to classify the children with chief complaint of short stature by its cause and giving the basic epidemiologic data for it so that the size of population in which growth hormone is indicated can be estimated. Methods : According to Ranke's etiologic classification, we categorized the 579 children who visited our pediatric endocrinology clinic with chief complaint of short stature during the period of March 1994 to August 1996. In this prospective study, history regarding growth was taken, physical examination and laboratory tests including bone age, thyroid function, blood chemistry were carried out. The auxological data were analyzed. Additional chromosomal study or growth hormone provocative tests were performed when needed. Results : Out of 579 patients, 360(62.2%) were classified as normal and 127(21.9%) were classified as normal variants which consist of familial [74(12.8%)], constitutional [48(8.5%)], and mixed familial & constitutional short stature[5(0.9%)]. Pathologic short stature was found in only 80(13.8%). Those are growth hormone deficiency(28), Tumer syndrome(16), intrauterine growth retardation(14) in order. Other etiologies list varieties of dysmorphism, skeletal dysplasia, chromosomal disorders. Conclusions : This results suggest the vast majority of children with chief complaint of short stature are normal or normal variants. Only 7.8% of children who visited our clinic were indicated for growth hormone therapy.

4.Distribution of the deposits of immunoglobulin,fibrinogen and fibronectin in psoriatic Lesions.

This study was conducted to observe immunological abnormalities, vscular changes and abnormal maturation, pathway of keratinocyte in the psoriasis, by comparing drposits of IgG, fibrinogen(FG) and fibronectin(FN) in the invnlved and uninvolved skin of 19 psoriaics before treatment, the involved skin of 12 among the above patients after treatment, and the skin of 8 normal controls. The study was carried out by using direct immunofluorescence technique with anti-IpG, anti-FG, anti-FN antibodies. The results are summarized as follows: 1. In the uninvolved skin of 19 psoriatics before treatment, no depasition of IgG was observed while deposition of FG was present at the dermo-epidermal junction in 5.3% of the patients and dermis(10.5 %). Deposition of FN was found at the dermo-epidermal junction(52%), papillary dermis(57.9%) and dermal blood vessels(52.6%). 2. In the involved skin of 19 psoriatics before treatment, deposition of IgG was observed in the horny layer in 84.2% of the patients, upper epidermis(15.8%) and depositicin of FG was present in the horny layer(52.6%), upper epidermi(63.1%), dermo-epidermal junction(15.8%), upper dermis(15.8%) and dermal blood vessels(5.3%). Leposition of FN was found in the horny ayer(26.3%), upper epidermis (36.8%), dermo-epidermal junction(78.9%), upper dermis(89.5%) and ermal blood vessels(68.4%). 3. In the involved skin of 12 psoriatics after treatment, IgG was wealily deposited in the horny layer in 16.7% of the patients and deposition of FG was observed in the horny layer(8.3%), upper epidermis (16.7%), dermo-epidermal junction(8.3%) and dermis(8.3%). Deposition of FN was found in the upper epiderrnis(8.3%), dermo-epiderrnal junction(58.3%), dermis(66.%) and dermal blood vessels (33.3%) 4. In the normal skin of 8 control subjects, no deposition of IgG and, FG was observed while deposition of FN was found at the inummo-epidermal junction in 37.5% of the persons, upper dermis(25%) and dermal blood vessels(12.5%)

5.Change in Pre-, Postoperative Endocrine Function and Growth Pattern in Patients with Craniopharyngioma.

PURPOSE:Craniopharyngioma is one of the most important intracranial tumors in pediatric age which causes multiple pituitary hormone deficiencies. We have studied clinical characteristics in patients with craniopharyngioma, focusing on changes of endocrine function, change of growth pattern, and change of body mass index before and after surgery. In addition, we wanted to know the effect of growth hormone therapy on growth velocity and body mass index, and to identify contributing factors for spontaneous growth in spite of growth hormone deficiency. METHODS:A total number of 17 patients who were diagnosed as having craniopharyngioma at the Department of Pediatrics, Asan Medical Center during the period of January, 1991 to December, 1997 were included in this study. Retrospectively, we reviwed medical records as to their endocrine function tests and auxoloogical data before, after surgery. RESULTS: 1)The number of patients of male and female were 9 and 8 respectively. Mean age was 7.3+/-4.3 years. 2)Preoperatively, 3 patients were thyroid hormone deficient, 3 patients were corticosteroid deficient, and 3 patients suffered from diabetes inspidus. 3)Postoperatively, GH and TSH deficiency were found in 100%, ACTH in 88.2%, ADH in 82.3%, and LH/FSH in 60%, 53% respectively. 4) Mean growth velocity in 14 GH deficient patients without growth hormone treatment for 2 years were 3.5+/-2.4cm/year during the 1st year and 3.1+/-1.7cm/year during the 2nd year. Although height standard deviation score(Ht. SDS) was decreasing trend as -0.24+/-2.19 at diagnosis, -0.94+/-1.73 at 1 year later, and -0.76+/-1.76 at 2 years later, it was not statistically significant. To our suprise, 4 out of 14 patients achieved greater growth velocity than nomal in spite of growth hormone deficiency during the 1st year after operation. 5) Although the mean body mass index of 14 GH deficient patients without GH treatment was increasing trend as 17.9+/-3.5 at diagnosis, 19.0+/-4.5 at 1 year later, and 19.9+/-4.8 at 2 year later, it was not statistically significant. 6) The mean Ht. SDS increased significantly in 7 patients treated with growth hormone(P<0.05) for 2 years, but change of body mass index was not significant. 7) Comparision of postoperative serum prolactin levels and changes of body mass index between spontaneous growth and stunted growth group did not reveal significant difference. CONCLUSION: Since most patients with craniopharyngioma become multiple pituitary hormone deficient after operation, it is important to predict and detect pituitary dysfunction to manage it effectively. Although patients with postoperative GH deficiency responded well to GH treatment, further study is needed to clarify what are the main contributing or prognostic factors for spontaneous growth without growth hormone treatment.

6.Changes of Histopathological Findings with Time in the Dermographism Lesion.

Changes of histopathological findings with time were studied after scratching the skin of 37 patients with dermographism. Biopsies were also done in 13 normal healthy controls for comparison with unstroked skin of the patients. 1. Biopsies of unscratched skin of the patients showed no histologic difference from those of the skin from controls. 2. Neutrophils increased in number with time after scratching and maximum neutrophil count (mean 16.08+/-24.17/HPF) was observed at 90 minutes after scratching 3. Eosinophilic infiltration was also similar to that of neutrophils. Maximum eosinophil count (mean 324+/-4.76/HPF) was found at 60 minutes after scratching. 4. L ymphohistiocytic infiltration showed a similar tendency to that of neutrophils, but the degree of change was not so prorninent. 5. Before scratching, mast cell count in patients with dermographism showed no difference in number when compared with norrnal controls. In patients with dermographism, mast; cell count inclined to decrease after scratching. 6. Edema and lymphatic dilatation in the upper dermis were most prominent at 5 minutes after scratching and disappeared slowly thereafter.

7.Clinical Features and Natural Course of Hashimoto's Thyroiditis.

PURPOSE:Although Hashimoto's thyroiditis is the most common cause of goiter in children and adolescents, it is not clear what proportion of patients become hypothyroid and which tests are the best predictors of this state. To determine whether these kinds of variations occur in the course of Hashimoto's thyroiditis and whether the size of the thyroid gland or immunologic markers correlate with the course or outcome of Hashimoto's thyroiditis are main objects of our study. METHODS:A total number of 48 patients who were diagnosed as having Hashimoto's thyroiditis at the Department of Pediatrics, Asan Medical Center during the period of January, 1992 to December, 1997 were included in this study. Retrospectively, we reviewed medical records as to their clinical and labaratory data. RESULTS:Thyroid function status at initial diagnosis of Hashimoto's thyroiditis were euthyroidism(33.3%), compensatory hypothyroidism(33.3%), overt hypothyroidism (27.1%), hyperthyroidism(8.3%) in order. Positivity of antithyroglobulin antibodies and antimicrosomal antibodies were 77.1% and 66.7% respectively. In 33 patients, 12(36.4 %) were on remission status after 3 years from initial diagnosis. Antithyroglobulin antibody titer was significantly decreased after 2-year follow up in remission group. Initial antithyroglobulin antibody titer and thyroid function status were not related to remission status after 3-year from diagnosis. CONCLUSION: About 36% of patients with Hashimoto's thyroiditis can be in remission after 3-year from diagnosis. Decrease of antithyroglobulin antibody titer is related to remission status. Further study is necessary to know what can be the predicting factors for early remission, for example, initial thyroid function status, initial antithyroid antibody titier, circulating immune complex, age, sex and size of thyroid.

8.A Case of Traumatic Epidermoid Cyst.

Traumatic epidermoid cysts are rare tumors occuring almost exclusively on the palms, palmar aspects of fingers, and soles. These cysts are thought to originate from a bit of epidermis implanted into the dermis after a penetrating or blunt wound. We report a case of typical traumatic epidermoid cyst on the sole of the right foot of a 23-year-old male student. The lesion was a coin-sized, tender, fluctuating cystic mass occuring 3 years after a penetrating wound by an iron nail. Histopathlogical examination revealed a deep dermal cyst lined by a thick, flattened, keratinizing wall with partial disruption accompanied by foreign-body reaction.

10.Preoperative Staging of Endometrial Carcinoma by MRI.

BACKGROUND: In patients with endometrial carcinoma, preoperative evaluation of exact staging has important prognostic and therapeutic implications. The incidence of pelvic and aortic lymph node involvement in endometrial carcinoma depends on grade of tumor differentiation and depth of myometrial invasion. MATERIAL AND METHOD: To evaluate whether MRI provides a preoperative assessment for staging of endometrial carcinoma, MRI was undertaken in 28 patients, a few weeks before operation. Myometrial invasion was devided in three categories, and involvement of cervix, adnexa, and pelvic cavity were classified. RESULTS: The results of MR imaging were compared with these of pathology. The preoperative MRI staging of endometrial carcinoma was correct in 22 out of 28 patients. In the evaluation of myometrial invasion, the MR imaging underestimated in 4 cases and overestimated in 1 case. CONCLUSION: In patients with endometrial carcinoma, MR imaging is very useful in the assessment of the depth of myometrial invasion, stromal invasion of cevix, lymphatic & pelvic metastases and extent of the lesion.
