1.Treatment of Femur Shaft Fracture by Intrlocking Intramedullary Nailing

The results of treatment of fracture of the femoral shaft with interlocking nailing were evaluated to determine the clinical usefulness. Eighty-two femoral fractures that had been treated by interloking nailing between October 1988 and June 1993 and had been followed for more than one year were included in this study. Fifty-one cases were fresh closed fracture and twenty-four, fresh open fracture. Five cases were delayed union and two were non-union. Closed intramedullary nailing was used in seventy-five cases and open intramedullary nailing in seven cases. The results were as follows; 1.According to Winquist-Hansen classification, 30 cases were type I, 16 type II, 13 type III, 14 type IV, and 9 type V 2. Static and dynamic interlocking nailing were done in 67 and 15 cases respectively. 3. The mean fracture healing period was 17 weeks with a range of 9 to 52 weeks. 4. Intraoperative complications were new fracture near the original fracture site(7 cases), femur neck fracture(l case), pudendal nerve neuropraxia(1 case). Postoperative compliction were delayed union(13 cases), limb shorteniln(4 cases), nonunion(3 cases), infection(3 cases), disal screw brokening(3 cases), nail breakage(l case), proximal screw brodening(1 case) and 12 valgus angulation(l case).

2.Clinical Application of Hepatic Arterial Embolization

Transcatheter arterial embolization was applied in 7 cases of hepatic disease including hemobilia of various causes and malignant tumors at Department of Radiology, Seoul National University Hospital during recent several years. The embolic materials were autologous blood clot and Gelfoam particle. Successful control of bleeding or devascularization of tumor vessel was made in 6 of the 7 patients. There was no serious complications; however, post embolization syndrome was found including pain, fever, nausea and vomiting in all cases and subsided after a few days. Transcatheter hepatic arterial embolization is suggested to be a safe and effective treatment in control of hemobilia and devascularization of malignant hepatic tumors.

3.Collateral circulations in inferior vena cava obstruction

Obstruction of the inferior vena cava(IVC) is an uncommon condition, and the collateral pathway variesaccording to the level, extent, duration and the cause of obstruction. Membranous obstruction of IVC in itshepatic portion might be one of the principal cause among Korean, though not reported till now. Analytical studywas perfomred in 26 cases of IVC obstruction with various cause. 1. The level of the obstruction showed relativelyeven distribution as follows, upper caval in 11 cases, middle caval in 6 cases and infrarenal in 9 cases. 2. Themain cause of upper caval obstruction was membranous obstruction . 3. As a whole, the main collateral pathway wasthe central route(22 case:85%). 4. Characteristic collateral pathway unique to upper caval obstruction wastranshepatic vanous collateral, developed between the obstructed segment and unobstructed segment of IVC. 5.Scalloping of left cardiac border produced by pericardiophrenic venous collateral was characteristic simple chestX-ray finding in IVC obstruction.

5.Mucosal Melanoma Arising in the Eustachian Tube: A Case Report.

Mucosal melanoma is a very rare disease. Most cases of mucosal melanoma have their origins in the head and neck region. To date, only three cases of melanoma originating from the eustachian tube have been reported. We present a case of mucosal melanoma of eustachian tube origin in which a complete excision was performed. The treatment of mucosal melanoma is not well established, but surgical excision is considered to be the treatment of choice. Radiotherapy is controversial but given consideration in the present case because of the anatomic complexity and difficulty in performing a complete excision. In this case, the patient underwent adjuvant radiotherapy and the mass size was greatly decreased which aided the subsequent surgical excision. Systemic chemotherapy was not utilized in this case. However, it is often used for palliative purposes. A literature review was conducted in the presentation of this case.

7.Factors associated with Hypokalemia in Continuous Ambulatory Peritoneal Dialysis Patients.

Hypokalemia is a frequent problem in patients on continuous ambulatory peritoneal dialysis (CAPD) and is affected by multiple factors. To evaluate factors associated with hypokalemia, we studied 68 patients on maintenance CAPD treatment for at least six months. In univariate analysis, patients with hypokalemia were associated with older age and the presence of diabetes mellitus. Serum albumin, calcium-phosphate product, triglyceride, body mass index, protein nitrogen appearance, and lean body mass assessed by creatinine kinetics were significantly lower as compared to those without hypokalemia. Serum C-reactive protein was significantly higher in the patients with hypokalemia. Multivariate stepwise linear regression analysis revealed that the serum albumin level and the ultrafiltration volume at the peritoneal equilibration test were independent factors associated with hypokalemia. This suggests that the serum potassium level may be an important nutritional marker in CAPD patients. Further longitudinal investigation is needed to clarify this relationship.
