2.A Case of Hypereosinophilic Syndrome.

The hypereosinophilic syndrome consists of peripheral blood eosinophilia of 1500/ mm3 or more without a known cause, plus signs and symptoms of organ eosinophilia. We report a case of hypereosnophilic syndrome with cutaneous manifestation. A 31 year-old female had intermittent fever, sbdominal pain, anemia, systolic murmur, hepatosplenomegaly, cheat discomfort, dry cough without rales, and skin-colored wheal on the trunk. Diagnoais of hypereosinophilic syndrome wss established by clinical findings, marked blood eosinophilia without a known cause, bone marrow aspiration and biopsy, liver scan, and histopsthologic findings of the skin.

3.Bacteriological Study of Pyodermas.

Bacteriological study, including antibiotic sensitivity tests, of 81 patients with pyodermas such as impetigo, folliculitis, furuncle, carbuncle, cellulitis and acuete infectious eczematoid dermatitis, was carried out during 4 months period from June, 1980 to September, 1980 at the department of dermatology, Han Il Hospital. The results were as follows: 1) The causative agents of impetigo in 42 patients, were coagulase positive Staphylacoccus aureus in 33, p-hemolytic Streptococcus in 6 and both organisms in R (S. aureus and 3-hemolytic Streptococcus). Coagulase negative Stgaphylococcus was not found. 2) The causative agents of superficial and deep follicultis in 23 cases, were coagulase positive S. aureus in 12, coaulase negative Staphylococcus in 5, p-hemolytic Streptococcus in 4 and S. aureus and p-hemolytic Streptococcus in 3. 3) The causative agents of cellulitis in 2 cases, were coagulase positive S. aureus in 1, and coagulase negative Staphylococcus in 1. 4) The causative agents of acute infectious eczematoid dermatitis in l4 cases, were coagulase positive S. aureus in 6, coagulase negative Staphylococcua in 1, p-hemolytic Streptococcus in 2, and two organisms in 5 (4 cases were mixed).

4.An implant-supported removable partial denture on milled bars to compromise the inadequate treatment plan: a clinical report.

Presurgical prosthetic treatment planning is critical for the success of the implant prosthesis. Inadequate treatment plan, due to insufficient discussion between prosthodontist, and surgeon, may result in poor prognosis. A 26-year-old male patient was referred for prosthodontic treatment after implant was placed in the area of teeth #17, 16, 22, 25 and 27, without adequate discussion nor the treatment planning between oral surgeon and prosthodontist. It was found that the patient had two hopeless teeth, and a severely resorbed alveolar ridge. Additional tooth extraction was needed and the type of definitive prosthesis was shifted from fixed type to removable one. Proper pre-surgical treatment planning is essential for the good prognosis. Implant-supported removable prosthesis on milled bars may be a useful treatment option in patients with incorrect angled placement on severely resorbed alveolar ridge.

7.Two Cases of Yersinia Pseudotuberculosis Infection with Acute Renal Failure in Pusan Province.

The usefulness of E-cadherin immunostaining as a marker of malignancy in the body fluids was investigated in the present study. Thirty-three histologically proven cases of cell blocks from the pleural, peritoneal, and pericardial fluids were studied by immunocytochemistry for E-cadherin antibody using LSAB method. These cases were cytologically diagnosed as adenocarcinoma (25 cases) and atypical cells (8 cases). Tumor cells showed strong positive membranous staining for E-cadherin antibody in 21 out of 25 cases (84%) of adenocarcinoma. E-cadherin staining was not found in 6 of 8 cases of suspicious maligancy. The sensitivity and specificity were 84% and 75%, respectively. Reactive mesothelial cells and inflammatory cells scattered were all negative. In conclusion, E-cadherin is an useful adjunctive marker to distinguish reactive mesothelial cells from the carcinoma cells in the body fluids.

8.Invasiveness and Proliferative Activity of Oral Squamous Cell Carcinoma : Immunohistochemical Study Using Laminin, Type IV Collagen, and Ki-67 Antibody.

Since the invasiveness and metastatic potential of the carcinoma and the proliferation of the tumor cells are the decisional factors in determining the prognosis of carcinoma, it has been a principal area of study to foresee these factors in early stage of the disease process. The purpose of the study was to propose a method to precisely determine the prognosis of tumor. The investigation was performed, using laminin, type IV collagen, and Ki-67 antibody, to reveal the invasiveness and proliferation of the squamous cell carcinoma which is the most prevalent cancer in the area of oral and maxillofacial complex. Frozen tissue sections, 6-8mum in thickness, were prepared from the surgical excision of fresh squamous cell carcinoma for the experimental group. The control group was composed of sections from the normal oral mucosa of the contralateral side. Following incubation in an appropriate primary antibody including laminin, type IV collagen and Ki-67, LSAB method was employed and the tissue sections were treated with DAB for light microscopic immurlohistochemistry. Semiquantitative analysis of the basement membrane was composed of five different positivity of laminin and type IV collagen according to the intensity of the staining. Mean number of Ki-67 positive cells per 2000 total cells in one slide were calculated along with the standard deviation. The obtained results were as follows : 1. In the basement membrane of normal oral mucosa, antibodies against laminin and type IV collagen showed regular thickened, continuous line. Positive reaction was also detected in the vascular walls. Ki-67 labelling index was 4.10+/- 0.80%, and the positive cells were distributed mainly in the basal and parabasal cell layers. 2. In basement membrane of the proliferating tumor adjacent to carcinoma, antibodies against laminin and type IV collagen showed irregular thin, discontinuous line. Ki-67 labelling index was 18.95+/-4.67%, and the parabasal cell layer exhibited increased number of positive cells. 3. In the basement membrane of the main proliferating tumor, antibodies against laminin and type IV collagen showed almost indistinct interrupted line and/or almost indistinct line. Ki-67 labelling index was 31.56+/-5.51%, and the parabasal and spinous cell layer indicated increased cell positivity. 4. Invasive islands showed almost negative reaction to the antibody against type IV collagen, and almost indistinct line of the laminin antibody was observed. Based upon these findings, the loss of laminin and type IV collagen and the Ki-67 labelling index were the highest in the basement membrane of invasive islands. Therefore, it is thought that invasive islands were more deeply involved, than main proliferating tumor, in the invasiveness and proliferation of tumor, and the more the invasiveness of the tumor, the more the cell proliferative activity.

9.Serum Insulin-like Growth Factor Binding Proteins Profiles During the Normal Oulatory Menstrual Cycle.

The insulin-like growth factor (IGF)s are believed to one of several growth factors that play an adjunctive role in ovarian follicular development. These factors circulate bound to a family of IGF-binding protein (IGFBP)s. It is known that circulating IGFBPs are involved in the transport of IGFs to tissues and modulate IGFs actions at local tissue. The purposes of this study were to evaluate changes in serum IGFBPs profiles during normal ovulatory menstrual cylce and to compare serum IGFBPs profiles in periovulatory phase of between normal ovulatory menstrual cylce and controlled hyperstimulated cycle. Fasting blood samples were obtained from 15 normal healthy women throughout normal ovulatory menstural cycle and on the day of aspiration of oocyte from 10 patients undergoing ovarian hyperstimuation for in vitro fertilization-embryo transfer. Serum IGFBP-1-IGFBP-4 were measured by western ligand blot and immunoprecipitation. Serum 17beta-estradiol was determined by radioimmunoassay. Type and molecular weight of serum IGFBP did not changed during normal ovulatory menstural cycle. No significant variation in the relative proportion and level of each IGFBP was found throughout normal ovulatory menstural cyle. Also, the relative proportion and level of each IGFBP did not correlated with serum 17beta-estradiol level. There was no significant difference in the relative proportion and level of each serum IGFBP between on the day of ovulation in normal ovulatory menstrual cylce and on the day of aspiration of oocyte in controlled hyperstimulated cycle. Our data indicate that IGFBPs have regulatory functions in ovary through an paracrine and autocrine rather than endocrine mechanism during normal ovulatory menstural cycle.
