1.Flexible Bronchoscopic Observation on Endobronchial Tuberculosis.

Endobronchial tuberculosis is frequently associated with lymphadenopathy, bronchial stenosis, atelectasis which needs to be differentiated with other pulmonary disease. Authors are presenting 25 cases of endobronchial tuberculosis which were confirmed boy bronchoscopic biopsy and AFB smear and/or culture. The study was done on patients who visited Yeungnam University Hospital from May 1983 to July 1985. The following results were obtained: 1. Of the 25 cases, 8 cases were male and 17 cased were female (male:female=1:2). 32 percent of the cases was older than 60 years of age. 2. Abnormal findings on chest x-ray were seen on 22 cases of 25 cases (88%) and its most predilective site was right upper lobe (24%). 3. Symptoms were coughing (56%), hemoptysis, hoarseness, chest pain, dyspnea and fever in orders. 4. The positive results were obtained in 73 percent of sputum AFB smear (11 cases of 15 cases). 60 percent of AFB culture (3 cases of 5 cases) and 58 percent of bronchoscopic biopsy (14 cases of 24 cases). 5. Complete pulmonary function test was done on 19 cases and showed normal result in 6 cases, restrictive pattern in 8 cases and small airway dysfunction in 5 cases. No case of obstructive airway disease was detected. 6. Associated disease were hypertension (2 cases), tuberculous meningitis (1 case), diabetes mellitus (1 case), and cataract (1 case). 7. The site of bronchoscopic lesion on bronchoscopic examination were as follow; 8. All 25 patients were treated with anti-tuberculosis therapy and 6 patients were treated with additional steroid therapy because of severe respiratory symptom and showed dramatic improvement of symptom as well as lung function.

3.Separation of Unfused Olecranon Epiphysis in an Adult Baseball Pitcher: A Case Report

Unfused olecranon epiphysis has been reported rarely. We found an unfused olecranon tip within joint and a few small spurs on the apposing olecranon fossa and removed it successfully through the arthroscope. We think that it is a part of secondary ossification center of the olecranon and tbat it was separated through the epiphyseal plate by repetitive impaction of the tip of the olecranon into the olecranon fossa during pitching. In conclusion, fusion of the intra-articular part(articular center) of the olecranon epiphysis can not occur by nature. Repetitive extension overload during pitching may cause symptoms, and could be treated with arthroscopic control.

7.Nonunion of Clavicle

Although the clavicle is probably the most commonly fractured bone, nonunions are rare. We experienced 19 cases of clavicle nonunions, trested at the Depatment of Orthopedic Surgery of Wonju Christian Hospital, between January 1980 and June 1988(seven years and six months). This study focused on predisposing factors in relation to nonunion of clavicle and treatment. The factors predisposing to nonunion were middle and lateral 1/3 of clavicle, primary open reduction, severe trauma with associated injury and persistent gross displscement of fregment. Among the 19 patients, 16 patients were treated with semitubular plating and iliac bone grafting, 2 patients with the resection of the distal fregment and one patient was treated with treated with K-wire fixation and bone graft. All patients achived good union by the postoperative 10.7 week on average and symptomes disappeared. It was concluded that symptomatic nonunion of the clavicle could be trested by operation, and the procedure of choice seemed to be rigid internal fixation with plate applied in compression and bone graft.

8.A Case of Atypical Sessile Dermatofibroma.

We report a case of a morphologic variant of dermatofibroma. A coin sized, brownish, shinny, somewhat fibrotic pedunculated tumor in a 39-year-old male was diagnosed as a sclerosing hemangioma, a type of dermatofibroma histopathologically. Its dome shape morphology was unusual in comparison with the typical morphology of dermatofibroma that we know.

9.Long Term Follow-up of the Stable Fractures Around Thoracolumbar Spinal Junction (Conservative Management vs. Operative Intervention).

STUDY DESIGN: The treatment of a stable fracture around thoracolumbarjunction is a controversial subject. OBJECTIVES: To evaluate the result from long term follow-up of the stable spinal fracture around thoracolumbar junction according to the operation, osteoporosis and in situ extension bar, respectively. SUMMARY OF BACKGROUND DATA: The thoracolumbar junction is a transitional zone in which the kyphotic angle of thoracic spine is continued to the lordotic angle of lumbar splne. So it is prone to be injured biomechanically, and the stability of the spine around thoracolumbar junction is not determined yet. METHODS: We studied retrospectively 105 cases of the stable fracture around thoracolumbar junction from January 1985 to December 1995. The criteria of the stability were a compression fracture by Denis F., a bursting fracture without involvement of posterior column, a neural enchroachment of bony fragment below 30%, fracture without subluxation or dislocation in spinal junction. RESULTS: The most common cause was traffic accident and, the first lumbar vertebrate was common site. Below 30% of initial compression, the progression was not exceded 5% in operative group. But in nonoperative group, anterior compression was more progressed than initial compression over 5%. The degree of anterior compression was affected by osteoporosis and in situ extension bar just after injury. CONCLUSIONS: We suggest an anterior compression abode 30% in spinal body of thoracolumbar junction for an additional operative indication. The prognosis may be affected by osteoporosis and in situ extension bar.

10.Dual Plate Fixation Compared with Hybrid External Fixator Application for Complex Tibial Plateau Fractures .

PURPOSE: To cmpare the clinical results of complex tibial plateau fractures treated by hybrid external fixation and dual plate fixation. MATERIALS AND METHODS: We studied forty patients with Schatzker type V and VI fractures respectively and selected twenty seven patients who were followed at least one year between January 2000 and December 2005. We evaluated the clinical results in which fourteen fractures with hybrid external fixation were compared with thirteen fractures with dual plate fixation. The clinical results were evaluated according to Knee Society Clinical Rating System and the statistical analysis was performed by Student t-test. RESULTS: There were no significant differences in terms of bone union time (average union time: dual plate fixation 13.8 weeks, hybrid external fixation 14.2 weeks). The quality of osseous reduction was superior in the fractures with dual plate fixation than those with hybrid external fixation. There were significant differences in functional score (average functional score: dual plate fixation 73, hybrid external fixation 62), but not in average knee score. CONCLUSION: The hybrid external fixation can be a useful modality for treatment of complex proximal tibial plateau fractures. But the good quality of the fracture reduction by dual plate fixation may be a indicator for favorable prognosis for satisfactory knee function.
