2.Comparative Study on the Epithelial Regeneration of the Corneal Ulceration after Cryocautery and Iodine Cautery.

Recently cryogenic therapy is widely used for the treatment of herpes simplex keratitis. We had experimental studies on the effect of iodine cautery and cryocautery upon the regeneration of corneal ulceration. On 10 adult albino rabbits, one eye was treated by iodine, and the other by cryocautery in area of 0.6cm X 0.6cm. The epithelium in this area was completly removed by cotton applicator. There after, the rate of epithelial regeneration was observed every 12 hours. The results were as follows: (1) The average period of complete epithelial regeneration was 48 hours in animal treated with cryocautery and 78 hours in animals treated with iodine cautery. This difference is statistically highly significant. (2) Corneal opacity was not demonstrated in eyes treated with cryocautery. Six eyes treated with iodine, however, showed various degree of corneal opacities. These results support the view that the healing time in human herpes simplex keratitis is shortened in patients with cryocautery as compared with iodine.

5.Legislation Direction for Health Information Privacy in the Telemedicine Era.

The Korean law on medicine was amended in 2003 to introduce new provisions for telemedicine, electronic medical record, and electronic prescription. However, this has not satisfied the realities of the increased demand on medical services and related technology development, resulting in calls for a legislative amendment. In this regard, recent active promotion projects and the advance notice proposing a new amendment by the Ministry of Health, Welfare, and Family Affairs have raised the need for active discussions on telemedicine because the current law on medicine and its amendment do not adequately address the individual privacy protection aspect in the telemedicine environment. In this regard, this study examines current domestic and foreign legal systems on telemedicine and privacy protection, drawing and reviewing subjects to be discussed for individual privacy protection in telemedicine, and proposes plans that may improve Korea's legal system. The domestic and foreign literature on telemedicine and privacy protection was reviewed, recent legislations on telemedicine and views of interest groups were considered, and expert opinions were collected. In addition, the main discussions on privacy protection in telemedicine were identified and reviewed, including information ownership, the scope of privacy protection, the right to review and request correction, and privacy protection matters related to foreign patients. The recent amendment to tele medicine contains a provision only on penalties for privacy protection violations. The main discussions in this study on privacy protection are expected to be reflected in future amendments to enforcement rules and sub-ordinances such as the enforcement ordinance.

7.Pain, Family Support and Quality of Lefe in Patients with Ankylosing Spondylitis.

This study was designed to identify the relationship between perceived pain, family support and quality of life in patients with ankylosing spondylitis. The purpose of this study was to contribute to the theoretical understanding of the relationship of these three variables and eventually to more effective adaptation of patients to their situation. The subjects for this study were the 68 patients who had been diagnosed with ankylsing spondylitis and registered as out-patients in the Rheumatism Center of one university hospital in Seoul. The data were collected during the period from October 10, 1997 to December 20, 1997. Pain was measured using the perceived pain scale(VAS: Visual Analog Scale)developed by Calin(1993), family support using the scale developed by Kang Hyun Suk(1985) and Quality of Life using the scale developed by Ro Yoo Ja(1988). The data were analyzed by descriptive statistics, Frequencies, Pearson correlation, using the SPSS program. The results of this can be summarized as followings. 1. The mean perceived pain score was 5.13 with a range of 2 to 10. 2. The mean perceived family support score was 41.08+/-5.34 with a range of 20 to 50. 3. The mean perceived quality of life score was 134.07+/-19.82 with a range of 83 to 176. 4. Significant statistical difference was found between family support and quality of life(r=.331, p<0.001). A significant negative statistical difference was found between the family support and quality of life and pain(r=-.250, p<0.05, r=-.460, p<0.001). 5. General characteristics related to pain were exercise(t=4.72, p<0.0006). 6. General characteristics related to family support were age(F=2.65, p<0.0246), educational level(F=2.84, p<0.0282) and exercise(t=3.24, p<0.0452). 7. General characteristics related to quality of life were educational level(F=3.03, p<0.0392) and exercise(t=3.12, p<0.0465). It was found that the higher the level perceived pain, the lower the degree of perceived family support and the quality of life. It was also fund that the higher the degree of perceived family support the higher the degree of perceived quality of life. Accordingly, the conclusions from this study are that reduction of pain is achieved through the family support. Therefore, it is proposed that family support is an appropriate nursing intervention to improve the quality of life of patients with anklosing spondylitis.

10.Cell Death and Cell Proliferation during Histogenesis in the Rat Retina after Birth.

During development of central nervous system, cell proliferation, cell migration, cell differentiation and cell death are required. It has been reported that a number of cells are dying during development in the mammalian retinae examined so far, but the pattern of cell death has not been clarified yet. In addition. little has been studied on cell proliferation after birth. This study was conducted to identify histogenesis, cell death and cell proliferation in the retinae of the developing rats by light and electron microscopic methods as well as by immunohistochemical method using anti-proliferating cell nuclear antigen [PCNA] antiserum. The results were as follows : 1. In the developing rat, from postnatal 0 through 7 days, retina consisted of ganglion cell layer, inner plexiform layer and neuroblast layer. Neuroblast layer could be subdivided into three sublaminae : sublamina a, sublamina b and sublamina p, from postnatal 3 through 7 days. 2. From postnatal 10 days, retina consisted of ganglion cell layer, inner plexiform layer, inner nuclear layer, outer plexiform layer and outer nuclear layer. 3. Cells undergoing degeneration were observed from postnatal 0 to 13 days, and patterns of cell death were apoptosis, cytoplasmic degeneration and autophagic degeneration. 4. PCNA-immunoreactivity was seen in the cells located in sublaminae b and p of the neuroblast layer at postnatal 0 and 1 days. From postnatal 3 days PCNA immunoreactivity decreased. At 7-day-old rat, PCNA-Immunoreactive cells scattered in the distal part of sublamina p of the neuroblast layer.No immunoreactivity was observed from postnatal 10 days. These results demonstrate that retinal cell proliferation ends at postnatal 7 days, and histogenesis of retina is completed at postnatal 10 days, and superfluous cells during retinal development are eliminated by apoptosis, cytoplasmic degeneration and autophagic degeneration.
