1.Two Cases of Acute Febrile Neutrophilic Dermatosis (Sweet's Syndrome).

We present two cases of acute febrile neutrophilic dermatosis developed in women aged 56 and 52. One of the two patients deveIoped the lesions during longterm antituberculosis chemotherapy for her advanced pulmonary tuberculosis, The other case developed the lesions, initially at the site of acupuncture done for relief of her arthralgia, followed by the involvement of the other sites. The etiology of acute febrile neutrophilic dermatosis is not clear, however, hypersensitivity and association with systemic diseases such as upper respiratory infection, malignancies, ulcerative colitis and pyoderma gangrenosum are postulated.

2.A Case of Xanthoma Tuberosum.

A 43 year-old Korean male has had multiple, grouped, asymptomatic, yellowish brown flat papules and nodules on the knees, elbows, buttocks, palms and soles for 3 years. Small yeIlowish, discrete papules first appeared on the right palm and left elbow, and gradually increased in size and number and spread to knees, buttocks and soIes. Physical examination revealed normal except for the skin lesions and no familial occurrence was noted. Gross finding of the serum was turbid, more or less creamy and paper electrophoresis revealed marked increase of prebetalipoprotein, increase of betalipoprotein and alphalipoprotein and absence of chylomicron, suggesting type III or type IV hyperlipoproteinemia. Serem choleterol was 200mg% and fasting blood sugar was 115.0mg%. with normal glucose tolerance test. Histopathology showed many aggregates of foam cells on H-E stain and many lipid droplets in the dermis on Sudan red stain. Based on the clinical and histopathological findings and lipoprotein analysis by the paper electrophoresis, the patient was diagnosed as type IV hyperlipoproteinemia with xanthoma tuberosum.

4.Perceptions of Medical Personnel toward Burnout using Q Methodology.

PURPOSE: This exploratory study was done to categorize medical personnel's perceptions of burnout and analyzed the characteristics. METHODS: Q methodology was applied using a 45 Q-sample categorized on an 11-point scale which was completed by nurses and doctors working at a university hospital in Seoul, Korea. Collected data were analyzed using the PC-QUANL program. RESULTS: Medical personnel's perceptions of burnout were categorized into three types; 33.83% of the total variance was explained. The first type was 'functionally deteriorating pressure' the second type was 'daily powerlessness' and the third was 'achievement-oriented re-energization'. CONCLUSION: There is a need to establish and apply intervention strategies for each type to alleviate medical personnel burnout and increase work efficiency to qualitatively improve medical services.

5.Pulmonary paragonimiasis: CT findings.

Though the incidence of paragonimiasis has been remarkably decreased since 1970, it is still not a rare disease in Korea. Major problems in the diagnosis of pulmonary paragonimiasis on chest radiography are its differentiation from pulmonary tuberculosis and lung cancer. Chest radiographic findings have been described in detail, but little have been reported on CT findings. We reviewed CT findings of 10 patients with pulmonary paragonimiasis. The characteristic CT findings were similar to those on chest radiography, such as air-space consolidation (70%), nodular mass (50%), pleural effusion (40%), cystic lesion (30%), small low density within the mass (30%), linear density (20%), pneumothorax(20%), and burrow track (20%). CT depicted the cystic lesions and the burrow tracks more clearly and showed the small worm-retaining cysts within the mass that were not detectable on chest radiography. In conclusion, all of these CT findings are useful in the diagnosis of pulmonary paragonimiasis especially when differentiation from tuberculosis or lung canceris difficult on chest radiography.

6.The Experience of Decision Making to Donate Organs.

PURPOSE: The aim of this study was to identify the process of experience toward decision making to donate organs. METHOD: Eleven subjects who registered at the Association for Organ Sharing in Korea were recruited. Using in-depth interviews and tape-recordings, data was collected from Jun. 2001 to Feb. 2002 and the contents of these were analyzed by Glaser's grounded theory analysis method. RESULT: Basic Social- Psychological Problem(BSPP) of subjects were 'attachment to body' and the core theme, that is Basic Socialization Progress(BSP) were discovered to 'find out what is the meaning of life'. Also it consisted of four steps: 1st 'hesitate', 2nd 'look into self', 3rd 'realize the tie up to self' and last 're-find the meaning of life'. CONCLUSION: These findings indicate the Korean's Basic Socialization Progress of decision making regarding organ donation, so possible development of promoting decision making is necessary strategies for people who is having his/her potential of organ donation in mind.

7.Localized Pseudopolyposis of the Ascending Colon Associated with Granulomatous Colitis: A case report.

Pseudopolyps represent discrete areas of mucosal inflammation and regeneration that are seen in a variety of inflammatory bowel disease including ulcerative colitis and Crohn's disease. These polyps are typically short, measuring less than 1.5 cm in height. Rarely, localized giant pseudopolyposis can occur, i.e., a collection of larger inflammatory pseudopolyps giving rise to a mass lesion within the colon. The most serious problem concerned with pseudopolyposis is a confusion with carcinoma. We experienced a case of localized giant pseudopolyposis causing partial large bowel obstruction. Right hemicolectomy was done for a preoperative diagnosis of ascending colon carcinoma. The resected specimen contained a circumferential lesion, which was composed of numerous interconnecting cylindrical villi, measuring 12 cm in length and 3 cm in height. Microscopically, these polypoid lesions were inflammatory pseudopolyps. Several deep fissure-like ulcerations were noted with multifocal microabscess, lymphoid hyperplasia and an area of noncaseating granuloma.
8.Development of a Nursing Professional Values Scale.

PURPOSE: The purpose of this study was to develop an instrument to measure nursing professional values. METHOD: Forty preliminary items were selected by classifying 223 basic items extracted via a literature study and in-depth interviews of subjects and testing the relevance of their contents. In order to verify the reliability and relevance of the preliminary instrument, data was collected from 504 nurses in 3 general hospitals. RESULT: As a result of the item analysis, 29 items were selected from a total of 40 items. Five factors were extracted by factor analysis, and the total variance was 51.5%. For the explanation of variances by factors, the 1st factor, 'self-concept of the profession' accounted for 14.8%, the 2nd factor, 'social awareness' 12.1%, the 3rd factor, 'professionalism of nursing' 9.8%, the 4th factor, 'the roles of nursing service' 9.1%, and the 5th factor, 'originality of nursing', 5.6%. Cronbach's Alpha of those 29 items was .9168, which was high. CONCLUSION: This paper is meaningful in a way that it developed a tool capable of measuring nursing professional values, which reflects the characteristics of our country. In order to re-verify the relevance and stability of this tool, it is necessary that comparative studies should be conducted.

9.The Current Status and Propects of Community Nutrition Services: II. The Perception and Needs of Community Nutrition Services among the Residents of the Pilot Service Areas.

The purposes of this study were to investigate the perception and needs of community nutrition programs for 379 community residents of 23 health centers where the pilot community nutrition programs are intervening. The awareness rate of the nutrition programs was 54.3% and the reason of the awareness was mainly happened to know when visiting health centers'. More than 90% of the respondents responded that public health nutrition services are necessary. But the residents who experienced the nutrition services showed higher needs of the programs(97.3%) and improved the impression about the roles of health centers(93.6%). They also showed a higher rate of balanced dieting, stronger intentions to change their inappropriate eating style and a higher practicing rate. The more they believed in the provided nutrition information, the more they showed concerns about their diet and practicing rate of the advices from nutritionists. These results show the positive and successful impact of the pilot nutrition programs on the community residents. We need strategies for a more active improvement of the programs and to maintain more qualified public health nutritionists to carry out targeted community nutrition programs.

10.Major patterns of inflammatory sinonasal diseases on CT.

Paranasal sinus CT is known as the most effective imaging modality in the evaluation of inflammatory sinonasal diseases and can depict the distribution, causative lesions obstructing main drainge route, and associated findings. Recently, functional endoscopic sinus surgery has been widely used for the evaluation and treatment. Before operation, PNS CT has been routinely used to evaluate the paranasal sinuses and mucociliary drainage route. The authors analyzed the PNS CT findings of 3156 cases in 1578 patients with chronic sinusitis symptoms. Sinonasal inflammatory diseases were categorized into 5 patterns according to the obstruction sites. They were 1) infundibular (10%, 316/3156), 2) ostimeatal unit (41%, 1294/3156), 3) sphenoethmoidal recess (12%, 379/3156), 4) sinonasal polyposis (30%, 946/3156) , and 5) unclassifiable (6%, 190/3156) patterns. The main causes for infundibular obstruction in order of frequency were inflammatory mucosa, enlarged ethmoidal bulla and Haller's cell. With respect to the middle meatus obstruction, is main causes in the order of frequency were polypoid lesion, inflammatory mucosa and medially deviated uncinate process. In particular, sinonasal polyposis showed one or more of the characteristic associated findings of infundibular enlargement, air-fluid level, ethmoidal sinus was bulging and bony deossification or sclerosis as well as sinonasal polypoid change. In conclusion, the inflammatory sinonasal diseases were classified into five patterns, and the causative lesions or anatomic variations were efficiently detected by the PNS CT. Furthermore, it could provide a guidance for proper management of the sinusitis including functional endoscopic sinus surgery.
