BACKGROUND: Ciclopiroxole.minsolution or cream is the topical in imycotic agent which has been known to have the ability to penetrate the horny skin and nail keratin. OBJECTIVES: The purpose of this study was to evaluate the pierapeutic effect of 1% ciclopiroxolamine solution on onychomycosis. METHODS: 1% ciclopiroxolamie solution was applied topically to hentire surface of t,he diseased nail at least twice a day for 12 to 36 weeks according to the verity of nail involvement and degree of improvement in 21 cases of onychomycoses. RESULTS: 1. With only topical application of 1% ciclopiroxolamine solutior, 4 out of 21 cases(67%) showed 25% or more reduction of the affected nail area, and 8 out of 21 cases(38%) showed 50% or more reduction of the affected nail area. 2. There was no significant difference in the degree of improvem of according to the duration of disease. 3. Therapeutic effect was relat.ively high on Candida albicans, while it was relatively low on Aspergillus species. 4. 3 out of 7 cases of mild(up to 30%) involvement showed a 7% or more reduction of the affected nail, while 4 out of 6 cases of severe(60-100%) involvement showed a 25% or less reduction of the affected nail. 5. As side effects during the topical application of 1% ciclopiroxol nr ne solution, periungualer ythema appeared in 3 cases, and a burning sensation in 4 of 21 cases. CONCLUSION: 1% ciclopiroir olaniine solution could be tried as an effctive therapeutic agent on onychomycosis for the patient who would not be tolerable to oral an ifiigal medication for associated systemic diseases or who does not want oral medication.

Herpes zoster is characterized by several groups of vesicles situateu unilaterally within the distribution of the cranial or spinal nerve coming from one posterior ganglion Biateral involvement and recurrence are rare, and zoster involves two widely separated regions at on time are even rarer. We report a case of bilateral eupes zoster. A 5-years-old female patient was presented with painful bilateral grouped vesicles on an erythematous base along the dermatome of Lt T2 and Rt T5. Tzanck smear revealed multinucleated giant cells, and histopathologic findings were consistent with herpetic infection. She was successfully treated by administration of acyclovir without any significant cornplications.

The purpose of this study was to develop a standardized Nursing Diagnosis/ Intervention Protocol through a literature review and validity testing. Seven nursing diagnoses were selected as major nursing diagnosis in the field of Nutritional and Eliminative problem. The nursing intervention list was made by an expert group's review of Nursing Intervention Classification(NIC) suggested nursing interventions. Nursing activities which were included in each nursing intervention were sorted to follow the nursing intervention process after review and revision. The expert group's validity testing was done twice using the Likert scale. As a result the Nursing Diagnosis/ Intervention Protocol for Nutritional and Eliminative Problems was made to include 7 Nursing Diagnoses, 51 Nursing Interventions and 631 Nursing Activities.

A clinico-pathologic study with an immunohistochemical examination for c-erbB-2 expression in 54 cases of ductal carcinoma in situ and 16 cases of Paget's disease of the breast was performed. c-erbB-2 oncoprotein overexpression was observed in 45% (24/54) and 88% (14/16) of ductal carcinoma in situ and Paget's disease, respectively. The overexpression of c-erbB-2 oncoprotein was significantly correlated with the nuclear grade of tumors and inversely with the status of the estrogen receptor. c-erbB-2 was positive in 4 out of 5 patients with metastasis to axillary lymph nodes and 3 out of 4 patients who died of the disease. Prognostic significance of c-erbB-2 oncoprotein in ductal carcinoma in situ was highly suggested. The expression of c-erbB-2 oncoprotein in Paget's disease was well correlated with coexisting infiltrating or in situ ductal carcinoma. The high positive rate of c-erbB-2 oncoprotein in ductal carcinoma with Paget's disease could be understood with a recent hypothesis that c-erbB-2 oncoprotein is involved in promotion of cell motility and the spread of carcinoma cells.

Trichophyton rnbrum is a.nthropophilic, and the most common and widely distributed dermato phyte of man. Characterisic lions are circular, usually sharplyn ginated, slightly erythematous, dry, scaly patches with a raised edge. Progressive central clearin produces annular outlines. Other variants are reported, such as granulomatous, bullous or verucius. We reported a case of Trichophyton rubrum infection showing an unusual clinical manifest tin. The skin leison showed multiple discrete brownishouni macules with a fine scale on both hi lower legs, dorsum of feet and buttocks. Central clearing and inflammed border were not not. On KOH mount, fungal hyphae were noted and Techophyton rubrum was isolated on the funu culture.

BACKGROUND: Squamous cell carcinoma(SCC) is the most common skin cancer in Korea and its incidence is increasing. OBJECTIVE: Our purpose was to evaluate the clinical and histopathological characteristics of SCC. METHODS: Forty cases of cutaneous SCC slides were retrieved from the files of the surgical pathology division of the National Medical Center. Hematoxylin and eosin stained sections were re-examined microscopically by two dermatologists independently. For the cases of reconfirmed SCC, clinical characteristics including incidence, age and sex distribution, location and underlying conditions and histopathologic grading by Broder's grade system were evaluated. For 3 control cases controls and 17 cases of the SCCs, cytokeratin expression patterns on immunohistochernical staining using monoclonal antithodies MNF 116, RCK 102 and NCL 5D3 were observed. RESULTS: 1. Of 40 specimens, 33 specimens(73.3%) were histologically reconfirmed as true cutaneous SCCs. 2. The most commonly involved age group was the fifth decade of life and the median age when diagnosis was made was 57.3. 24 male patients and 9 female patients were involved and the ratio of male to female was 2.67: 1. 3. Most commonly involved sites were face and lower extremities and the incidence of occurrence on sun exposed areas increased in accordance with an increase of the age. 4. Underlying conditions such as burn scars, osteomyelitis or atomic bomb exposure were associated in 455% of SCCs. 5. Histopathologic grading of 33 cases of SCCs by Broder's grade was performed as follows.:23 cases(67.7%) of grade I, 8 cases(24.2%) of grade II and 2 cases(6.1%) of grade III. 6. The cytokeratin expressio pattern on immunohistochemical staining for 3 of the control cases and 17 cases of SCCs showed some differences between the highly and poorly differentiated groups of SCCs. CONCLUSION: We evaluated the clinical and histopathological characteristics of primary cutaneous SCCs in the National Medical Center from september 1978 to August 1993.

PURPOSE: The purpose of this study was to construct and test a structural equation model for pressure ulcer prevention action by clinical nurses. The Health Belief Model and the Theory of Planned Behavior were used as the basis for the study. METHODS: A structured questionnaire was completed by 251 clinical nurses to analyze the relationships between concepts of perceived benefits, perceived barriers, attitude, subjective norm, perceived control, intention to perform action and behavior. SPSS 22.0 and AMOS 22.0 programs were used to analyze the efficiency of the hypothesized model and calculate the direct and indirect effects of factors affecting pressure ulcer prevention action among clinical nurses. RESULTS: The model fitness statistics of the hypothetical model fitted to the recommended levels. Attitude, subjective norm and perceived control on pressure ulcer prevention action explained 64.2% for intention to perform prevention action. CONCLUSION: The major findings of this study indicate that it is essential to recognize improvement in positive attitude for pressure ulcer prevention action and a need for systematic education programs to increase perceived control for prevention action.

Fusarium species are known as ubiquitous soil saprophytes and human skin comtaminants. Occasionally, however, they have been reported to cause a variety of infections in humans. We report herein a case of cutaneous infection by Fusarium sokmi in a patient with Burger's disease involving his right lower leg. He was treated with itraconazole 200 mg p.o. per day for 5 weeks and the skin lesion resolved without any noticeable side effects. Furthermore, there has been no evidence of recurrence of the disease 9 months after discontinuing the itraconazole. This is the first reported case of cutaneous infection by Fusarium sokmi in a patient with Burger's disease and itraconazole was an effective drug in our case.

PURPOSE: This study was done to explore the relationship of social support and meaning of life to suicidal thoughts among patients with cancer. METHODS: Data were collected by questionnaires from 138 patients who had been in cancer treatment at medical clinics and 8 patients who were members of an internet cancer association. The data were collected between August and November 2009 and analyzed using SPSS 12.0. RESULTS: Of the participants, 47.3% reported having had suicidal thoughts and 16.4% had attempted suicide since the diagnosis of cancer. The study participants received most support from family members, but 73.3% reported experiencing an existential vacuum. The suicide attempt group had significantly higher scores according to gender, age, level of education, diagnosis, treatment modality, level of activity, caregiver and social support compare to the suicide thought group. Suicidal thoughts were negatively related to social support and meaning of life was positively associated with social support. Support from family and friends and diagnosis explained 50.0% of variance for suicidal thoughts with 36.0% of variance being explained by family support. CONCLUSION: Nurses should be able to identify risk factors for suicide in cancer patients. Prevention and intervention efforts need to be directed toward improving social support, family support in particular, and assisting patients finding meaning in life after a diagnosis of cancer.
