1.Psychometrics of Health-Related Hardiness Scale for Korean Elderly.

The purpose of this study was to test psychometric properties Health-Related Hardiness Scale(HRHS) for Korean elderly. HRHS has been developed by Pollock to measure the effect of hardiness in individuals with actual health problems. Items were generated to measure dimensions based on theoretical definitions of health-related control, commitment, and challenge. In 1990, Pollcok and Duffy revised HRHS to be 34 items. In this study, 34-item HRHS(Pollock & Duffy, 1990) was translated into Korean and administered to 193 elderly involving in 95 hospitalized patients and 98 outpatients or healthy people. The subjects were 80 women and 112 men with an age range of 65 to 95 years(M=71 years) in whom 82.6% classified themselves as moderate or pretty active. Most subjects(80.2%) had education of elementary school or less. The 34-item HRHS consists of three subscales: control, commitment, and challenge, and was measured with 1(strongly disagree) to 6(strongly agree) Likert scale. The higher the score is, the higher people's hardiness. Factor analysis was performed to confirm whether the scale represents three sub-areas as suggested in the literature. Two factors were isolated, using principle components analysis. Two factor solution was accepted for the study since Pollock described the possibility of classifying sub-areas into two divisions. These two factors, commitment/challenge and control accounted for 31.9% of variance of the scale. Factor loadings were string, ranging from .41 to .77. The moderate correlation with task self-efficacy scale also supported the validity(r=.35, p<.05). The scale was found to be internally consistent, showing Cronbanch;s alpha of .90 for the total items with the range of .82 and .90 for three subscales. The findings revealed the potential applicability of HRHS to nursing intervention studies for elderly.

2.Pulmonary paragonimiasis: CT findings.

Though the incidence of paragonimiasis has been remarkably decreased since 1970, it is still not a rare disease in Korea. Major problems in the diagnosis of pulmonary paragonimiasis on chest radiography are its differentiation from pulmonary tuberculosis and lung cancer. Chest radiographic findings have been described in detail, but little have been reported on CT findings. We reviewed CT findings of 10 patients with pulmonary paragonimiasis. The characteristic CT findings were similar to those on chest radiography, such as air-space consolidation (70%), nodular mass (50%), pleural effusion (40%), cystic lesion (30%), small low density within the mass (30%), linear density (20%), pneumothorax(20%), and burrow track (20%). CT depicted the cystic lesions and the burrow tracks more clearly and showed the small worm-retaining cysts within the mass that were not detectable on chest radiography. In conclusion, all of these CT findings are useful in the diagnosis of pulmonary paragonimiasis especially when differentiation from tuberculosis or lung canceris difficult on chest radiography.

The purpose of this study is to develop the standards of care in rehabilitation nurse specialist. This study was a descriptive survey. The data were collected 143 nurses who were worked in rehabilitation unit at 4 university hospital and 3 rehabilitation center in Korea from Aug. to Nov. 2000. The questionnaire was consisted of 78 items developed by the standards of care in rehabilitation nurse specialist in A.R.N. and the practice contents of care at rehabilitation unit in Korea. Collected data were analyzed by frequency, percentage, mean, S.D. ANOVA The results are as follows 1. For the adequency of the practice contents of rehabilitation nursing, area of skin care is the highest score, and safety, elimination, emotion, respiration, was ordered. 2. The adequency of the practice contents of rehabilitation nursing according to age, educational level. position. clinical experience, clinical experience in rehabilitation unit were significant difference.

4.Analysis of the Effectiveness of Tai Chi Exercise for improving Balance.

PURPOSE: This study aims to analyze the effectiveness of Tai chi exercise for improving balance and its tendency and helps to identify directions for future research. METHOD: 19 articles from Medline search of foreign journals(1981-2003) and 5 from Korean nursing journals (1981-2003) were surveyed. The contents analyses were focused on outcome measures and relative factors regarding balance. RESULT: Variable measures for balance were used in Tai Chi studies relating to balance. They included the functional measures such as 14 single-leg stance, 7 walking in physiological measures and 7 laboratory-based balance measures were done with platform stability test. The measure of effective Tai Chi exercise depended on the health status of subjects and the methods. Health status of subjects were only on physically inactive older adults in 17 articles and arthritis patients in 7 articles in the Tai Chi study. The significant effects of Tai Chi exercise on balance was revealed after 10weeks duration. CONCLUSION: Considering the results, Tai Chi exercise is more effective than any other studies for walking balance. But the reports on the outcome are inconsistent with wide variations in the choice of balance measures. It is suggested that future studies analyze systemic change through meta analysis and have a wide variety of subjects who need postural control and balance control. The methods of measures should be distinct from the balance state of subjects for the effectiveness of Tai chi exercise.

5.The Educational contents of Rehabilitation Nursing.

The purpose of this study was to provide fundamental data to propose further directions of education on rehabilitation nursing by investigating the adequacy of the educational contents of rehabilitation nursing. This study was a descriptive survey study. The data collected at 25 universities and 24 junior colleges through questionnaires to answer the research questions from August 10 to September 30, 2000. The questionnaire was consisted of 24 items. The contents of rehabilitation nursing education were developed by consulting with the rehabilitation nursing professionals. The results are as follows Rehabilitation nursing was taught as an independent class in 15 universities and 9 junior colleges. Most professors majoring in adult nursing(66.8%) were in charge of teaching the courses. For the adequacy of the teaching contents of rehabilitation nursing, conceptual bases for rehabilitation was the highest score(4.0), and interdisciplinary rehabilitation team, activities of daily living, clients of rehabilitation, nursing process in rehabilitation nursing, functional evaluation, movement and mobility, physical therapy, occupational therapy, sensation and perception, communication/language, eating and swallowing, bladder elimination, community based rehabilitation nursing, sleep, rest &, fatigue, bowel elimination., historical perspectives of rehabilitation nursing, sexuality, pulmonary rehabilitation, pain, cardiac rehabilitation, skin integrity, family care was ordered.

6.Health Related Quality of Life of Women with Disabilities and Its Affecting Factors.

PURPOSE: The purpose of this study was to investigate the factors that influenced health related quality of life in women with disabilities. The other purpose was to examine the correlations of activities of these women's daily life, self esteem, and health promoting behavior with health related quality of life. METHOD: The subjects consisted of120 adult disabled women. Data was collected from February to June2004 using structured questionnaires. Spearman's correlation, and multiple regression were used to test the quality of life with K-ADL, self esteem, and health promoting behaviors by using SPSS 12.0 for windows. RESULT: The health related quality of life of women with disabilities was related to educational level, marital status, residential type, cause of acquired disability and disability of spouse. In addition, it was related to self esteem (r=.385, p=0.00), health promoting behaviors(r=.428, p=0.00), K-ADL(r=-.419, p=0.00) and K-IADL(r=-.439, p=0.00). Activities of daily life, self esteem and health promoting behaviors were significant predictors 37.3% (=0.212, p=0.000) to explain quality of life in disabled women. CONCLUSION: The more self esteem, health promoting behaviors, K-ADL and K-IADL are positive, the more the health related quality of life is good. Further studies need to be done to investigate additional effects of health related quality of life and to investigate nursing approaches to improve their health related quality of life with significant predictors.

7.Use of Polymyxin B Hemoperfusion in a Patient with Septic Shock and Septic Cardiomyopathy Who Was Placed on Extracorporeal Membrane Oxygen Support.

Although shock in sepsis is usually managed successfully by conventional medical treatment, a subset of cases do not respond and may require salvage therapies such as veno-arterial extracorporeal membrane oxygenation (VA ECMO) support as well as an attempt to remove endotoxins. However, there are limited reports of attempts to remove endotoxins in patients with septic shock on VA ECMO support. We recently experienced a case of septic shock with severe myocardial injury whose hemodynamic improvement was unsatisfactory despite extracorporeal membrane oxygenation (ECMO) support. Since the cause of sepsis was acute pyelonephritis and blood cultures grew gram-negative bacilli, we additionally applied polymyxin B direct hemoperfusion (PMX-DHP) to the ECMO circuit and were able to successfully taper off vasopressors and wean off ECMO support. To the best of our knowledge, this is the first adult case in which PMX-DHP in addition to ECMO support was successfully utilized in a patient with septic shock. This case indicates that additional PMX-DHP therapy may be beneficial and technically feasible in patients with septic shock with severe myocardial injury refractory to ECMO support.

8.Health-related Quality of Life and Depression after Radical Prostatectomy or Hormonal Therapy.

PURPOSE: The aim of this study was to compare the health-related quality of life (HQOL) and depression in prostate cancer patients with radial prostatectomy or hormonal therapy more than 6 months ago. METHODS: A total of 116 patients participated in the study (83 radical prostatectomy patients and 33 hormonal therapy). Data were collected from 13th September to 13th November 2012, using two instruments-translated into Korean: Expanded Prostate Cancer Index Composite and Geriatric Depression Scale Short Form. RESULTS: Hormonal therapy participants reported significantly higher scores of HQOL than radical prostatectomy participants did (p=.002). The HQOL subscales with significant differences were urinary function (p<.001) and incontinence (p<.001) under urinary domain and sexual bother (p<.001) under sexual domain. The level of depression was not significantly different between the two treatment groups. There were moderate negative correlations between HQOL and depression in both groups. CONCLUSION: Many of prostate cancer patients reported treatment-related functional deteriorations in urinary and sexual domains and they were depressed even more than 6 months after the treatment completion. To manage prostate cancer patients' HQOL and depression, clinicians including nurses should evaluate patients' complaints according to their treatment modality and intervene accordingly.

9.A Case of Glucagonoma Syndrome Diagnosed as Necrolytic Migratory Erythema.

Necrolytic migratory erythema (NME) is a typical cutaneous manifestation of glucagonoma syndrome. The entire syndrome consists of NME, glucose intolerance, weight loss, anemia, glossitis, diarrhea, and increased glucagon levels. We herein report a patient with glucagonoma syndrome who was diagnosed as having NME. A 48-year-old male presented with a 2-month history of painful erythematous, desquamative, erosive papules and plaques on both lower extremities. Histological examination revealed an intraepidermal cleft, the presence of vacuolated, pale epidermal cells, and necrosis in the upper epidermis. His glucagon level was 2650 pg/ml, with the upper limit of a normal range being 250 pg/ml. The patient was treated with octreotide, and showed an improvement of the skin eruption with normalization of the glucagon level within 4 weeks.

10.Direct coronal computed tomography of the body

Direct coronal CT scans of the body have been carried out in 78 cases of the thoracic and abdnominal diseases. The results are as follows: 1. Among total 78 patients, 66 cases were successfully scanned by the direct coronal CT. 2. Direct co ronal CT scan most accurately detected retroperitoneal diseases, especially renal diseases. 3. In comparison with multiplanar reformating (MPR) image, direct coronal CT of the body has three advantages: (a) improved image quality: (b) improved definition of the coronal anatomy : (c) shorter examination time. 4. Direct coronal CT has advantages in evaluation of the mass nature and relationship with other organs. 5. Disadvantages or limitations of the direct coronal CT are as follows: in creased radiation exposure and insufficient positioning for disabled patients. 6. In our experience, direct coronal CT is considered as helpful method for presurgical evaluation and the rapeutic management of various diseases, especially gross tumor of the retroperitoneum.
