2.A case of preoperative angiographic uterine artery embolization for the conservative treatment of cervical pregnancy.

A 35 year old woman, gravid 5, multiparous, was admitted to our department at 6 weeks and 5 days of gestation after being diagnosed with cervical pregnancy. Before the evacuation, the uterine arteries were embolized using angiographic techniques. As a result, dilatation and curettage were performed with minimal hemorrhage. We report one case of cervical pregnancy managed, successfully with preoperative uterine artery embolization and evacuation.

Sjogren's syndrome is a systemic disorder and characterized by keratoconjunctivitis sicca, xerostomia, and connective tissuedysfunction(arthritis). A 42 year-old Korean iemalehad progressive loss of visual acuity in both eyes. In addition she has dry eyes, dry mouth and arthritis of 8 years duration. Clinical findings include cornea punctate, filamentary karatitis and decreased lacrimal secretions of both eyes. A diagnosis of Sjogren's syndrome was made. The karatoconjunctivitis siccawas treated with artificial tear and soft contact lens.

7.Is There a Role of RigiScan(R) in the Measurement of Rigidity after Intracorporeal Injection of Prostaglandin E1?.

PURPOSE: To evaluate whether radial rigidity measured using RigiScan(R) represents the intracorporeal pressure effectively. MATERIALS AND METHODS: From January 1998 to May 1999, total of 23 patients with erectile dysfunction were evaluated by RigiScan(R) and duplex ultrasonography after the intracorporeal injection of prostaglandin E1. Peak systolic velocity and end diastolic velocity were measured by duplex ultrasonography and then the resistance index was calculated as (peak systolic velocity-end diastolic velocity)/(peak systolic velocity). Radial rigidity of penile tip and base was measured by RigiScanR . The results were analyzed statistically by PC-SPSS version 7.5. RESULTS: There were statistically significant correlations between radial rigidity of penile tip and base and the resistance index by Spearman's correlation analysis, respectively (r=0.680, p<0.001)(r=0.703, p<0.001). When radial rigidity of penile tip and base exceeded 60% of maximum, radial rigidity of penile tip and base again correlated well with the resistance index, respectively (r=0.659, p=0.020)(r=0.759, p=0.011). Based on clinically determined degree of erection, radial rigidity of penile tip and base represented the intracorporeal pressure effectively. CONCLUSIONS: Radial rigidity measured by RigiScan(R) represents the intracorporeal pressure effectively.

9.Psychometrics of Health-Related Hardiness Scale for Korean Elderly.

The purpose of this study was to test psychometric properties Health-Related Hardiness Scale(HRHS) for Korean elderly. HRHS has been developed by Pollock to measure the effect of hardiness in individuals with actual health problems. Items were generated to measure dimensions based on theoretical definitions of health-related control, commitment, and challenge. In 1990, Pollcok and Duffy revised HRHS to be 34 items. In this study, 34-item HRHS(Pollock & Duffy, 1990) was translated into Korean and administered to 193 elderly involving in 95 hospitalized patients and 98 outpatients or healthy people. The subjects were 80 women and 112 men with an age range of 65 to 95 years(M=71 years) in whom 82.6% classified themselves as moderate or pretty active. Most subjects(80.2%) had education of elementary school or less. The 34-item HRHS consists of three subscales: control, commitment, and challenge, and was measured with 1(strongly disagree) to 6(strongly agree) Likert scale. The higher the score is, the higher people's hardiness. Factor analysis was performed to confirm whether the scale represents three sub-areas as suggested in the literature. Two factors were isolated, using principle components analysis. Two factor solution was accepted for the study since Pollock described the possibility of classifying sub-areas into two divisions. These two factors, commitment/challenge and control accounted for 31.9% of variance of the scale. Factor loadings were string, ranging from .41 to .77. The moderate correlation with task self-efficacy scale also supported the validity(r=.35, p<.05). The scale was found to be internally consistent, showing Cronbanch;s alpha of .90 for the total items with the range of .82 and .90 for three subscales. The findings revealed the potential applicability of HRHS to nursing intervention studies for elderly.

10.Dosage and Plasma Concentration of Local Anesthetics in Epidural Anesthesia for Cesarean Section.

Epidural anesthesia for cesarean section requires sensory blockade up to T4 level. 18 to 20 ml of 0.5% bupivacaine, 1.5 to 2.0% lidocaine, or 3.0% chloropmcaine usually produces an adequate sensory blockade to T4 level. However, when we used that amount of local anesthetics, most of the patients often complained of mild to moderate pain or discomfort during delivery of baby, manipulation of peritoneum or the uterus. So we feeled the need of increase in dosage of local anesthetics and we used 26 ml of local anesthetics or local anesthetic-fentanyl mixture. This study was undertaken to measure plasma concentration of lidocaine and observe the possible systemic toxicity of local anesthetics with the total dosage of beyond maximum recom-mended dose. Epidural anesthesia were performed with a 17 gauge Tuohy needle and local anesthetics were injected as follows. Group 1 (n;17): 0.5% bupivacaine 100 mg+2% lidocaine 80 mg+fentanyl 100 ug (50 ug/ml) Group 2 (n; 15): 2% lidocaine 520 mg without epinephrine Group 3 (n; ll): 2% lidocaine 480 mg + fentanyl 100 ug All groups were received 26 ml of local anesthetics or local anesthetic-fentanyl mixture. We checked the level of anesthesia, length of spinal column, cardiovascular changes, plasma concen- tration of lidocaine, toxic systemic reactions and patient's complaints. The plasma concentration of lidocaine were measured by immunofluorescence assay at 2, 5, 7, 10, 12 and 15 minutes after injection of local anesthetics. The results were as follows; 1) The peak plasma concentration of lidocaine were measured 12 minutes after administration of local anesthetics in the group 2 and 3. 2) Sensory blockade up to T4 level could be accomplished within 10 minutes after epidural anesthesia in the group l. 3) In the group 2 and 3, onset of action was more rapid. These groups had a tendency to fall in blood pressure than the group 1, but recovered soon with small dose of ephedrine. 4) In one patient, maximum peak plasma concentration of hdocaine was 6.8 ug/ml, but no adversereaction was observed. Above results suggested that 26 ml of local anesthetics in all groups could be used for appropriate anesthesia for cesarean section with minimal camplications and patient's satisfaction.
