3.Knowledge, Health Belief, and Vaccination Behavior on Hepatitis A among University Students.

OBJECTIVE: This study was to investigate the knowledge, health belief, and vaccination behavior on hepatitis A among university students. METHODS: A self-administered questionnaire survey was conducted from 3(rd) to 25(th) March, 2014 and 197 subjects were enrolled in statistical analysis. RESULTS: The result showed that the score for knowledge of the subjects on hepatitis A was 4.59±3.06 out of 15 points, for health belief 2.39±0.28 out of 4 points, and the vaccination rate of the subjects was 12.7%. There was significant difference in hepatitis A knowledge score according to experience of hepatitis A check-up and hepatitis A vaccination history of family members, and in health belief according to gender. For hepatitis A vaccination there was significant difference according to experience of hepatitis A check-up, family history of liver disease, hepatitis A vaccination history of family members, and education history of hepatitis A. As a result of logistic regression analysis experience of hepatitis check-up and hepatitis A vaccination history of family members were significant factors for hepatitis A vaccination. CONCLUSIONS: The level of knowledge, health belief for hepatitis A of the subjects was low and vaccination rate also low. The experience of hepatitis A check-up and hepatitis A vaccination history of family members were factors affecting hepatitis A vaccination. It would be necessary to develop programs for improving level of knowledge and health belief and raising the rate of hepatitis A vaccination for the university students in Korea.

5.The Effects of Combination of Fentanyl with Morphine in Intravenous Patient-Controlled Analgesia.

Background: The highly lipid soluble opioid, fentanyl, has a rapid onset and short duration of action. The present study was designed to examine the analgesic efficacy and side effects of the combination of fentanyl with morphine in patients using intravenous PCA. Methods: Patients were randomly assigned to receive one of three PCA regimens: M4 group (40 mg morphine+90 mg ketorolac+1.5 mg dorperidol), M2F2 group (20 mg morphine+200 ug fentanyl+90 mg ketorolac+1.5 mg dorperidol), or M2F4 group (20 mg morphine+400 ug fentanyl+90 mg ketorolac+1.5 mg dorperidol). All patients were given initial loading dose of 0.1 mg/kg morphine plus 1 mg droperidol at the end of surgery. Pain score, side effects, and overall satisfaction were assessed at 30 min, 1 hr, 8 hr, 24 hr, and 48 hr postoperatively. Results: The pain score was significantly higher in the M2F2 group than in the M4 group and M2F4 group during 1 hr and 8 hr postoperatively. The total opioid consumption was significantly greater in the M2F4 group than in the M4 group. Patient satisfaction was better in the M2F4 than other two groups. There were no differences in the overall incidence of side effects among three groups. Conclusions: The present results suggest that the combination of fentanyl with morphine for intravenous patient-controlled analgesia is a useful method, and the double dose of fentanyl in comparison with the equipotent morphine dose is recommended in the early postoperative period.

6.Allelic Loss at the BRCA1 and BRCA2 Loci in Sporadic Breast Carcinoma Using Paraffin Embedded Tissue .

BACKGROUND: Germline mutations in the breast cancer-associated genes BRCA1 and BRCA2 confer susceptibility and a lifetime risk of breast. Several morphological and clinical features have been attributed to hereditary tumors. However, in sporadic breast cancer, the interrelationship between the loss of heterozygosity (LOH) of these loci and clinical features remains to be fully elucidated. METHODS: Microdissected paraffin-embedded tissue blocks of 48 cases of surgically resected breast carcinoma were investigated to identify the LOH of BRCA1 and BRCA2 using microsatellite markers. RESULTS: Of 48 cases, 22 (45.9%) exhibited LOH at BRCA1 locus while in 29 out of 48 (60.4%) cases LOH was observed for the BRCA2 region. There was no significant correlation between LOH at BRCA1/2 and the patient's age, tumor size, histologic grade or lymph node metastasis. When comparing the frequency of LOH with the expression of several prognostic factors, such as p53, c-erb B2 protein, estrogen and progesterone receptor using immunohistochemical stain, there was only correlation with LOH at BRCA2 and the progesterone receptor. CONCLUSIONS: Our results suggest that allelic deletion play a role to the development of sporadic breast cancers.

7.Recurrent Herpes Zoster During the Treatment of systemic Lupus Erythematosus With Immunosuppressive Drugs.

Herpes zoster is known to be more frequent and severe in patients during immunosuppressive treatment and various predisposing factors, such as Hodgkin's disease, advanced tumor stage, local X-ray irradiation, splenectomy and cutaneous anergy. This report presents the case of a 23-year-old woman who developed herpes zoater on two separate occasione during the treatment of systemic lupus erythematosus with immunosuppressive agents (corticosteroid and cyclophosphamide). In each occasion, it was involved by different and multiple dermatomes. Treatment with oral and intralesional corticosteroids administration proved to be beneficial without any complication, although some delayed recovery of the disease was noted.

8.A Case of Patau Syndrome with Congenital Ocular Anomaly.

Patau syndrome, a trisomy of number 13 chromosome, is a rare congenital chromosomal anomaly accompaing many abnormalities of cardiovascular and central nervous system, kidney and extremity, face and eye. We experienced a case of Patau syndrome in newborn infant who had bilateral microphthalmia, microcornea, corneal opacity, iris coloboma, cataract, dislocated lens, spherophakia, retianl fold and dysplasia, which are typical ocular anomalies of this syndrome previously described overseas but not reported in domestic. So, we report this case with a review of the literatures.

9.Does Lidocaine Mixture for Preventing The Pain on Propofol Injection Affect Anesthetic Induction and Hemodynamic Responses to Tracheal Intubation.

Background: We hypothesized that intravenous lidocaine mixed with propofol may have an influence on anesthesia induction and hemodynamic responses to propofol induction and endotracheal intubation as well as propofol-induced pain on injection. Methods: Seventy-five patients were allocated to group L1 (2% lidocaine 1.5 mg/kg, n=25), group L2 (2% lidocaine 2 mg/kg, n=25) or group C (normal saline 0.05 mL/kg, n=25) according to the lidocaine dosage mixed with propofol 2 mg/kg. The pain on injection was scored as none, mild, moderate, and severe. The site of pain and recall of pain were also recorded. Loss of verbal response was observed during induction. Mean arterial blood pressure (MAP) and heart rate (HR) were recorded before anesthetic induction (baseline value), immediately before and after endotracheal intubation, and every min until 5 min thereafter. Results: Ninety-two percent of patients reported pain upon injection in group C, whereas 8% of the patients in group L1 and no patient in group L2. Loss of verbal response before injection of total dose of propofol was observed in 44% in group L2, 36% in group L1 and 28% in group C. Lowered MAP caused by propofol increased significantly after endotracheal intubation in all three groups (p<0.05). HR increased immediately and 1 min after endotracheal intubation in all three groups (p<0.05). Conclusions: Our results indicate that intravenous lidocaine 1.5 mg/kg or 2 mg/kg mixed with propofol 2 mg/kg significantly reduces the incidence and the degree of pain, but does not affect anesthesia induction and hemodynamic responses to propofol and tracheal intubation.

10.Target Effect-site Controlled Infusion of Propofol by Schnider Model: Comparison of Gender and Age.

BACKGROUND: We investigated effect-site median effective concentration (EC50) of propofol for loss of consciousness and effect-site concentration at each anesthetic stage and awakening time to compare the differences by gender or age of patients using Schnider model. METHODS: Total 91 patients were allocated into 4 groups: male patients aged 18-34 year (Group 1), male patients aged 35-55 years (Group 2), female patients aged 18-34 years (Group 3) and female patients aged 35-55 years (Group 4). The effect-site EC50 for loss of consciousness was calculated and the effect-site concentration of propofol at each anesthetic stage and the awakening time of each group were recorded. RESULTS: The effect-site EC50 of propofol for loss of consciousness was 4.53 +/- 0.30microgram/ml, 4.60 +/- 0.25microgram/ml, 4.13 +/- 0.39microgram/ ml and 4.47 +/- 0.65microgram/ml in group 1, 2, 3, and 4, respectively. The effect-site concentrations in male patients aged 18-34 years were significantly higher than those in female patients. The awakening time of female patients aged 35-55 years was shorter than that of male patients. CONCLUSIONS: There were no significant differences of effect-site EC50 for the loss of consciousness of propofol with target effect-site controlled infusion by Schnider model among the groups. However the effect-site concentrations and the awakening time of propofol by Schnider model showed differences in patients' gender and age.
