2.Clinical Application of EMA, CEA and LCA in Differential Diagnosis of Tumors.

To determine the therapeutic plan and to predict the prognosis, it is a pivotal and important task to differentiate the exact nature of the various undifferentiated neoplasms. In order to solve this problem, the electron microscopy has once been considered only method in certain cases. Recently, however, the immunoperoxidase staining method which is less expensive and easier to usethan electron microscopy has been developed and introduced. To evaluate the diagnostic value of epithelial membrane antigen(EMA), carcinoembryonic antigen(CEA) and leukocyte common antigen(LCA) immunoperoxidase method, the authors applied these staining in 15 cases of which exact diagnoses were difficult with conventional routine and special stains, using the formalin-fixed and paraffin-embedded tissue sections. The results are as follows: 1) EMA was helpful in confirming the diagnosis or revising to the proper diagnosis of undifferentiated carcinoma in 4 of 6 cases and in excluding the possibility of hepatocellular carcinoma and melanoma in 2 cases, respectively. The negative stainabilities in one case of choriocarcinoma and one case of malignant fibrous histiocytoma was useful to exclude the possibility of squamous cell carcinoma and the positive stainability in one case of giant cell carcinoma to exclude the possibility of sarcoma. 2) The diagnostic value of CEA was similar to that of EMA; CEA was useful to differentiate from malignant lymphoma in one case of epithelial tumor in which CEA was positive and EMA negative. 3) LCA was useful to differentiate one case of malignant lymphoma from undifferentiated carcinoma and to confirm Ewing's sarcoma from malignant lymphoma in one case.

3.Influencing Factors on Health Promotion Behavior of Fire Officers.

PURPOSE: The purpose of this study was to investigate the factors influencing health promotion behavior of fire officers in Korea. METHODS: It was a descriptive study. The subjects were comprised of 104 fire officers by self-reported questionnaires from January to February 2012. The instruments used for this study were a health belief, self-efficacy and health promotion lifestyle profile II. The data were analyzed using SPSS/WIN 14.0. RESULTS: The scores of health promotion behavior of fire officers were 2.76 (on a 4-point scale) and of health belief were 2.98 (on a 4-point scale), followed by perceived benefits at 3.45, perceived seriousness at 3.02, perceived sensitiveness at 2.82, perceived barriers at 2.23, and self-efficacy at 3.04 (on a 4-point scale). The health promotion behavior was very closely correlated with the health belief (r=0.49, p<.001). In the sub region, perceived benefit was correlated (r=0.28 p=.005) and the perceived seriousness (r=0.38, p<.001), the perceived sensitiveness (r=0.36, p<.001), and self-efficacy (r=0.55, p<.001) were correlated. Two significant variables influencing health promotion behavior of fire officer were self-efficacy and perceived seriousness (explained 41.0%). CONCLUSION: It suggested that self-efficacy and perceived seriousness should be considered when developing a fire officers's health promoting program.

5.The statistical observations for pediatric inpatients(1971-1990).

We observed the patients admitted to the department of pediatrics of Seoul Red Cross Hospital during the past twenty years form Jan. 1971 to Dec. 1990. The results were as follows: 1) The total number of pediatric inpatients during 20 years was 15,800, of which 9,683 were male, 6,117 were female and male to female ratio was 1.58:1. 2) On age distribution, neonates and infants less than 1 year of age were 6,224 cases (39.4%). 3) Monthly distribution of the inpatients showed the highest incidence in October. 4) Infectious and parasitic diseases were 4,759 cases (30.1%) and respiratory tract diseases, 3,556 cases (22.5%) and disease of these two group occupied more than one half of total admission. 5) Major 10 leading causes of hospitalization were diarrheal disease (14.2%), convulsion (7.0%), acute bronchiolitis (6.8%), pneumonia, tuberculosis, URI, neonatal hyperbilirubinemia, prematurity and low birth weight infant, acute glomerulonephritis and laryngitis in the order of frequency. 6) There was marked decrease in the number of the admitted patients with typhoid fever, bacillary dysentery, tuberculous meningitis, hepatitis, pyogenic meningitis, rheumatic fever, and acute glomerulonephritis. But the patients with URI, bronchial asthma, neonatal hyperbilirubinemia and Kawasaki disease have increased.

8.The gastrofiberscopic findings in pediatric patients.

We observed 240 cases ranging in age from 3 months to 15 years seen by gastrofiberoscopy in the department of pediatrics of Seoul Red Cross Hospital from March 1990 to October 1992 and following results were obtained. 1) The male to female ratios in total cases and in 89 patients complaining of abdominal pain for more than one month were 0.94:1 and 1.02:1 respectively. The most prevalent age group was 10~12 years in both groups. 2) The indication in the order of frequency were chronic abdominal pain (37.1%), acute abdominal pain (32.5%), vomiting (7.9%), hematemesis and melena (5.4%), follow-up study (6.3%), further evaluation (3.8%), chest discomfort (2.5%), foreign body removal (2.1%), nausea (1.2%), dysphagia (0.8%) and abdominal mass (0.4%). 3) The common endoscopic findings in the 225 cases excluding follow-up ones were gastritis (34.7%), duodenitis (14.2%), ulcer (11.6%), esophagitis (7.6%) and foreign body (2.2%). 4) The common endoscopic findings in cases of chronic abdominal pain were gastritis (42.7%), duodenitis (16.9%) and ulcer (7.9%). 5) There was no complication except mild sore throat.

10.Immunohistochemical Observation of Placental Form of Glutathione S-Transferase in Squamous Cell Carcinoma.

Glutathione S-Transferase (GST) is a conjugation enzyme in the metabolism of exogenous and endogenous lipophilic compounds for their excretion and detoxification. Acidic isozyme of GST, GST-Pi, has been recognized as a preneoplastic marker in the experimental hyperplastic nodules of liver in rats, and GST-Pi is abundant in the squamous cells of the skin, also. This histochemical study was carried out to evaluate the distribution and the relationship between the differentiation status of squamous cells in dysplastic or neoplastic epithelium in various organs. The human placental form of glutathione S-transferase (GST-Pi) were stained immunohistochemically with specific anti GST-Pi rabbit antibody in 23 cases of human squamous cell carcinomas. The patients consisted of 14 cases from the uterine cervix, 3 cases from the esopahgus, 3 cases from the lung and 3 cases from the larynx. The results obtained were as follows; 1. Basal cells in normal mucosa were stained negative for GST-Pi while superficial keratinocytes were stained moderately positive. Basal dysplastic cells were stained negatively or weakly positive. Carcinoma cells especially large cells either keratinizing or nonkeratinizing were stained moderately to strongly. Carcinoma cells surrounding keratin pearl were strongly reacted with GST-Pi than other carcinoma cells. 2. Differentiated cells of squamous cell carcinoma showed moderate to strong positive reaction to GST-Pi staining irrespective of its site of origin. 3. Therefore, Immunohistochemical staining pattern of GST-Pi in various squamous carcinoma cells showed similar immunohistochemical reaction to the GST-pi, which is closely correlated to the degree of differentiation, keratinigation and also suggested that squamous carcinoma cells had abundant GST-Pi related detoxifying system.
