Laureth 9 is a nonionic surfactant and it is used as a sclerosing agen; in the treatment of esophageal and gastric varices and a local anesthetics. We report a 20-year-old male patient presenting with features of alergic contact dermatitis due to laureth 9 used in the oxoralen, ointment.

2.Rapid Detection of Rifampin Resistant Mycobacterium tuberculosis Using the Line Probe Assay.

BACKGROUND: Tuberculosis continues to be a major threat to health throughout the world, with an estimated 8 to 10 million new cases and 3 million deaths annually. And control of the disease is further threatened by the emergence of drug resistance. Recent major advances have been made in unravelling the molecular basis of M. tuberculosis resistance to isoniazid, streptomycin, quinolones and rifampin. And rifampin resistance is the useful indicator for the occurance of the multi-drug resistance. Hence, the rapid detection of rifampin resistant strain of M. tuberculosis is the key to have successful anti-tuberculosis therapy. Here we present our experience using PCR and line probe assay (INNO-LiPA) for easy and rapid detection of rifampin resistance of M. tuberculosis. METHODS: Thirty rifampin resistant and twenty susceptible strains of M. tuberculosis were collected from the routine culture and analyzed with INNO-LiPA. And results were compared with conventional antibiotic susceptibility testing. After amplification of the region of the RNA polymerase(rpoB), the amplified product is hybridized with a set of 10 oligonucleotides immobilized onto a membrane strip. From the pattern obtained the presence or not of rifampin resistance M. tuberculosis can be assessed. RESULTS: Ninety three percent of patients who had rifampin resistant strain revealed the multidrug resistance while only two showed resistance to rifampin only. The INNO-LiPA test results were generally agreeable with that of the conventional susceptibility testing(90%). The mutations in codon 531 (absence of 55 probe) were most commonly observed. In 55.2% of the 31 rifampin resistance M. tuberculosis confirmed on mutation by R-probes on the INNO-LiPA strips. CONCLUSIONS: The line probe assay after polymerase chain reaction is a fast and convenient method to detect both presence of M. tuberculosis complex strains and its resistance to rifampin in clinical specimens. We have suggested that detection of rifampin resistance may play a key role in monitoring multi-drug resistance. Consequently, the INNO-LiPA test may constitute an important tool for the control of tuberculosis.

3.A clinical and histologic study of 71 cases of basal cell carcinoma.

A clinical and histologic survey was done on 71 cases of basal cel carcinoma(BCC) filed at Department of Clinical Fathology, Kang Nam, Han Kang, and Kang Dong Sacred Heart Hospital during past 13 year s from 1979 to 1991. The results were summerized as follows: 1. Of all malignant skin tumors BCC accounted for 31.6%. The frequency of BCC in the total number of outpatients visiting the Department of Dermatology was 0.06%. 2. The ratio of men to women with HCC was 1:1.03. 83.8% of the pat.ients with BCC were older than the age of l0 years with the mean age of 56.9 years. 3. 83.8% of the BCC appared in the face, especially on the nose(25.4%), eyelid(20.3%). 4. On the basis of classification of Lever et al, the solid type(66.7% ) was the most com mon histologic pattern, followed by the adenoid(8.8%), pigmented(8.8%), and etc. By the classification of Farmer et al, the nodulocystic type(35.1% ) was the most common histologic pattern followed by infiltrative(31.6%), adenoid(8.8%), pigmented(8.8%), and etc. By the classification on Sexton et al, in the order of decreasing frequency, liistologic subtypes were the nodular(43.9%), mixed (22.8%), infiltrative(19.3%), supeficial(3.5%), morpheic(3.5%), and micronodular(3.5%).

4.Influence of Resilience and Job Embeddedness on Turnover Intention in General Hospital Nurses

PURPOSE: The purpose of this study was to identify the effect of resilience and job placement on general hospital nurse's intention to relocate, prepare a practical transfer management plan, and provide basic data on effective personnel management. METHODS: Data were collected from 333 nurses in 2 general hospitals with more than 500 beds in P-city. Data analysis was performed using hierarchical regression with the SPSS/WIN 25.0 program. RESULTS: The significant factors influencing turnover intention of hospital nurses were sacrifice, fit, link, interpersonal relationship, followed by experience of turnover. Explanatory power was 42% in the regression model. CONCLUSION: Resilience and job embeddedness have a positive effect on turnover intention in general hospital nurses. Therefore, efforts to improve resilience, job embeddedness, and organizational support are required to decrease turnover intention.

5.Percutaneous Transhepatic Biliary Drainage Using Large Needle: Complications and Usefulness.

PURPOSE: To analyse complications and to review usefulness of large needle(18G) in percutaneous transhepatic biliary drainage(PTBD). MATERIALS AND METHODS: 46 patients underwent PTBD in 52 occassions using large needle. 44 of 46 patients had either definite or suspicious malignant biliary obstruction, and most patients were poor in general dondition. Complications were classified as acute type if developed within 30 days and as delayed type if developed after 30 days. Acute type was subdivided into severe and mild forms. RESULTS: Severe forms of complications were death(5.8%), septicemia(3.8%), and bile peritonitis(1.9%). There were not different from the rate of complication in PTBD using fine needle but the procedure was much more simple. CONCLUSION: We PTBD using large neddie is a simple and safe procedure for prompt bile decompression.

7.Incidence and Contributing Factors of Malpositioning of the Endotracheal Tube after Endotracheal Intubation with Chest X-ray.

BACKGROUND: Malpositioning of the endotracheal tube within airway after intubation may results in serious complications, such as accidental extubation and inadvertent endobronchial intubation. Therefore, early detection of malposioning of the endotracheal tube is very impotant for deciding patient's prognosis. METHODS: We assessed the distance from the tip of the endotracheal tube to the carina according to the patient's age, sex, and the time, the location, and the route of intubation. The chest x-ray was taken for 333 patients (men, 226: women, 107) who were admitted to the intensive care unit of Wonju Christian Hospital from march 1, 1995 to February 28, 1996. Chest x-ray was obtained after intubation to verify endotracheal tube position. Appropriate endotracheal tube position on chest x-ray was defined as between 2 and 6 cm above the carina. RESULTS: Of the 333 intubations, 106 (31.8%) endotracheal tubes were inappropriately placed according to the chest x-ray. The percentage of malpositioned endotracheal tubes (<2 cm) was higher in women than in men (11.2% vs. 5.3%, p<0.05), with higher in night (7 PM to 7 AM) than in day (7 AM to 7 PM) (42% vs. 24%, p<0.05). Distance from the carina to the tip of endotracheal tube is 4.34 +/- 1.77cm in women and 5.23 +/- 1.64cm in men. Thus, position of the endotracheal tube in women is deeper than men (p<0.001). CONCLUSIONS: We conclude that the chest x-ray for confirmation of endotracheal tube position after endotracheal intubation may remain the standard of practice. And endotracheal tube position should be carefully assessed immediately after tracheal intubation, particularly in women and at night.

8.Relationship of Body Weight, Anxiety Depression, and Quality of Life in Patients with Radical Gastrectomy according to Perioperative Period.

PURPOSE: The purpose of this study was to identify the relationship of quality of life (QoL) with body weight, anxiety, and depression according to perioperative period. METHODS: Data were secondarily analyzed from 30 gastric cancer patients. RESULTS: There were positive relationships of anxiety with depression (r=.42, p=.019) and appetite symptoms of QoL (r=.41, p=.023) at preoperative day one. There were negative relationships of body weight with physical function of QoL (r=-.40, p=.026), and positive relationships of anxiety with depression (r=.37, p=.044) and constipation symptom of QoL (r=.38, p=.034) at postoperative day 7. Also, at postoperative one month, there were positive relationships of body weight with physical function (r=-.45, p=.011) and constipation symptom (r=.37, p=.039). There was a negative relationship of anxiety with emotional function (r=-.39, p= .035), a negative relationship of depression with physical function (r=-.43, p= .018), and a positive relationship of depression with finance (r=.39, p=.034). CONCLUSION: Oncology nurses should consider diverse factors during the early postoperative period to increase QoL. Also, appropriate and timely interventions should be provided to minimize the negative influence on QoL.

9.A Structural Equation Model of Pressure Ulcer Prevention Action in Clinical Nurses.

PURPOSE: The purpose of this study was to construct and test a structural equation model for pressure ulcer prevention action by clinical nurses. The Health Belief Model and the Theory of Planned Behavior were used as the basis for the study. METHODS: A structured questionnaire was completed by 251 clinical nurses to analyze the relationships between concepts of perceived benefits, perceived barriers, attitude, subjective norm, perceived control, intention to perform action and behavior. SPSS 22.0 and AMOS 22.0 programs were used to analyze the efficiency of the hypothesized model and calculate the direct and indirect effects of factors affecting pressure ulcer prevention action among clinical nurses. RESULTS: The model fitness statistics of the hypothetical model fitted to the recommended levels. Attitude, subjective norm and perceived control on pressure ulcer prevention action explained 64.2% for intention to perform prevention action. CONCLUSION: The major findings of this study indicate that it is essential to recognize improvement in positive attitude for pressure ulcer prevention action and a need for systematic education programs to increase perceived control for prevention action.

10.Type I Allergy to House Dust Mite and Familial BACKGROUND of Respiratory Atopy in Patients with Atopic Dermatitis.

BACKGROUND: Recent studies have suggested that house dust mite allergy is an important cause of the atopic dermatitis(A.D). However, it is not clear that. what factors may be related to the development of the mite illergy in patients with AD. OBJECTIVE: This study was done to see whether the presence of a familial background of RA implies a relationship to the mite allergy in AD. METHODS: Skin prick test and fluoroallergosorbent, test (FAST) with house dust mit,e were performed in 47 patients wih AD. RESULTS: 1. In comparison the esult of prick test with that of FAST to house dust mite antigen, it showed a concordance ra!e of 82%. And the prick test was more sensitive than the FAST. 2. The prevalence of positive FAST reactions was significantly increased in the patients with AD who had a family hitory of RA than those in patients with AD who had neither family or personal history of RA. 3. The level of specific IgE against house dust mite did not relate to the presence of family cr personal history of RA. 4. The prevalence of positive prick test results did not relate to the presence of family or personal history of RA. 5. The prevalence of positive FAST reactions, the level of specific IgE, and the rate of positive prick test results did not relate to the severity of skin involvement. 6. The most common allergens which caused positive skin reactions were house dust, cat fur, Dermatophagoides farinac, Dermatophagoides pteronyssinus, et al. Antigen score of prick test to 55 common antigens wa. significant increased in the patients with AD who had a family history of RA, but did not relat to the severity of skin involvement or the presence of personal history of RA. CONCLUSION: We may conclude that type I allergy to the house dust mit,e is not directly related to AD perse. This type of allergy to the mite seems to occur predominantly in those patient s with AD who have a farilial background of RA.
