2.A Clinical Study on the Treatment of Medical Collateral Ligament Injuries

In military training, the knee joint is more susceptible to trauma than any other joint in the body. Chronic knee instability is a difficult problem for both the patient and the orthopaedic surgeon who must correct the disability. To achieve the best possible result, the surgeon must first make the correct diagnosis and be aware of the type or types of instability that exist. There may be associated instability in a rotatory plane as described by Slocum and Larson, and the surgeon must examine carefully for this. Various surgical techniques have been used in the past to repair the knee static stabilizers such as ligaments and capsule. Since the introduction of the dynamic concept of rotatory instability and the need for muscular reinforcement of static repair, most orthopaedists have attempted to back up their reapirs with various types of dynamic stabilization such as the pes anserinus transfer and advancement of semimembranosus. 39 cases of medial instability of the knee joint were analysed clinically in the department of orthopaedic surgery of Busan Armed Froce General Hospital from Feb. 1977 to Feb. 1980 and following results were obtained. 1. All of the cases were military personnels, most of them were privates and P.F.C. showed most frequent incidence in 19 cases (49%). 2. Sports injury was a main causes of the knee instability. 3. The highest incidence in severity of the lesion was grade III in 21 cases (54%). 4. Among 39 cases of ligamentous injuries of medial side, rupture of collateral ligament alone were 30 cases, and mixed injuries with cruciate were 9 cases. 5. The pes transfer was performed in all of the cases with acute medial collateral rupture, and even in the cases with instability after primary repair, good results were obtained by carrying out dynamic muscle or tendon transfer. 6. In some cases, the patient did not feel much discomfort for the remaining static instability in valgus stress test after dynamic reconstruction. This was thought to be overcome by dynamic force during use of the leg. 7. For the purpose to prevent further stretching of repaired ligaments, derotation brace were recommended in 5 cases of dynamic reconstructive surgery.

3.Septic Arthritis of the Knee Following Arthroscopic Anterior Cruciate Ligament Reconstruction.

PURPOSE: Septic arthritis of the knee is a relative rare complication of the arthroscopic anterior cruciate ligament reconstruction, but inadequate treatment of this complication may lead to a devastating conse- quence of a chronic infection. Moreover, no guidelines have been proposed for the treatment of patients with knee infections after the anterior cruciate ligament reconstruction. To determine the optimal diagno- sis and treatment recommendation, we performed this study. MATERIALS AND METHODS: We performed retrospective study of the knee joint infections after the arthroscopic anterior cruciate ligament reconstruction between August 1993 and August 1998, Of the 420 patients who reviewed arthroscopic ACL reconstruction during this period from one surgeon, we experi- enced 4 cases(0.95%) of postoperative deep infections of the knee. RESULTS: All 4 patients were male, and the average age was 32 years(range from 19 to 43). Two patients had acute(< 2 weeks), two patients had subacute infections(2 weeks to 2 months). The acute cases had more severe symptoms, and positive cultures from knee joint aspirates. All patients underwent immediate open(1 case), or arthroscopic irrigation and debridement. One patient underwent repeat irriga- tion and debridement. The graft was removed from all patients in acute cases. CONCLUSION: The clinical symptoms, and the infected organism were more helpful than the peripheral blood counts in the diagnosis of a deep infection of the knee. Arthroscopic lavage, and debridement com- bined with intravenous antibiotics were effective treatment options. Clinical symptoms were more severe in acute cases, and early graft removal is an adequate method of surgical treatment.

6.Hypertensive Arterial Change in Hindlimb Unloading Rats.

BACKGROUND: In a condition of microgravity, arteries of upper body part are exposed to relative hypertensive stress, which is induced by headward fluid shift. We already have reported the increase of monocyte adhesion and intercellular adhesion molecule-I (ICAM-I) expression in carotid artery and thoracic aorta in the simulated microgravity. The purpose of this study was to identify the evidence of hypertensive arterial change in hindlimb unloading rats. METHODS: A total of 16 Sprague-Dawley rats were divided into the 2 and the 4 weeks hindlimb unloading groups and their control groups, respectively. After experimental exposure, the carotid artery, the thoracic/abdominal aorta, and the femoral artery were obtained from each experimental and control groups. The degree of matrix metalloproteinase (MMP)-2, MMP-9, and tissue inhibitor of MMPs type 1 (TIMP-1) expression were analyzed by immunohistochemical stain. RESULTS: The increase of MMP-2, 9 expression was noted in the carotid artery and the thoracic aorta from the 4 weeks hindlimb unloading group. On the other hand, the MMP-2, 9 expression was negligible in the abdominal aorta and the femoral artery of this group. In general, the findings of the 2 weeks hindlimb unloading group were similar to those of the control group. The TIMP expression was not found in all groups. CONCLUSION: We found the evidence of hypertensive arterial change by increased shear stress on upper body part's arteries in simulated microgravity. Considering the possible development of this change into hypertensive arterial damage with other cardiovascular risk factors, our findings can be highlighted as an important medical concern, especially in a long-term spaceflight.

8.Arthroscopic Ankle Arthrodesis.

Eleven cases of arthroscopic ankle arthrodesis were reviewed. Average age of patients was 59 years, 4 male and 7 female. Underlying diseases were osteoarthritis in 7 cases, rheumatoid arthritis in 3 cases and one post-traumatic arthritis. At the mean follow up of 15 months, fusion rate was 100%, mean time to complete union was 9.5 weeks. There were no complications such as nonunion, infection and skin problems. All patients stayed at the hospital just overnight after the surgery. Patients satisfaction was high and the functional clinical grade was excellent in 9 patients, good in 1 patient and fair in 1 patient. We concluded that arthroscopic technique was a simple and effective method for ankle arthrodesis in the selected patients and had significant advantages over the open technique.

9.Tram Track Lesion of the Talar Dome.

PURPOSE: We report a distinctive lesion in the articular cartilage of the talar dome in anterior bony impingement syndrome of the joint. MATERIALS AND METHODS: Arthroscopic debridement was done in 68 ankles under the diagnosis of anterior impingement syndrome of the ankle joint between October 1994 and June 1996. There were 6 tram track lesions of talar dome of the 68 ankles. We evaluated arthroscopic findings of tramtrack lesion, injury mechanism, and treatment results according to the criteria of Ogilvie-Harris DJ et al. RESULTS: All six male patients were professional or collegiate soccer players. Average age was 29 years (26 to 34). All 6 cases were dorminant ankles. Trauma history was not definite in all 6 cases. All 6 patients were anterior impingement syndrome of the ankle and treated by arthroscopic removal of osteophytes. The cartilage lesions were full thickness defects, located in the anterior half of the medial aspect of the talar dome and were longitudinal with variable widths resembling a tram track, thus named ""tram track lesion"". Tram track lesion and osteophytes were contacted by ankle dorsiflexion. Overall, good and excellent results were achieved in five patients at a mean follow up of twenty-seven months. Two cases had recurrences of osteophytes. CONCLUSIONS: Tram track lesion of talar dome occurred by repeated scratching of the osteophyte of distal tibia and treated effectively by arthroscopic removal of osteophyte and debridement.

10.Detection of Antibodies against Shope Fibroma Virus and Formation of Fibroma in the Korean Domestic Rabbits.

In our preliminary study to find antiviral or antitumor agents from Korean natural products, we found that the Shope fibroma virus (SFV) induced fibromas reaching maximum size at 5~6 days with spontaneous disappearance at 15~20 days after SFV intracutaneous inoculation into Korean domestic rabbits. However, the sizes of fibromas of rabbits at day 5 after virus inoculation were significantly different individually. Assuming that the variation of tumor size was due to either susceptibility or the preexisting antibodies against SFV in the Korean domestic rabbits, the rabbits were checked for the antibodies against SFV by IFAT using SFV infected RKl3 cells. The antibody positive rate of normal Korean domestic rabbits was 32.8% and the sizes of the fibromas of the positive rabbits were significantly smaller than those of negative rabbits (p<0.0001). The fibroma sizes were dependent on the antibody titers of rabbits to SFV. The sizes of fibromas after inoculation of SFV into immunized rabbits were about one tenth of those by the first inoculation into normal rabbits. This is the first report on the antibody prevalence against SFV among normal Korean domestic rabbits and it suggest the existence of a wild fibroma virus or related virus in Korea.
