3.Studies on Alkaline Phosphatase Isoenzyme in the Serum and Organs of the Rat.

Isoenzymes of alkaline phosphatase from purified extracts of liver, intestine, pancreas and bone of rats were determined by their isoelectric points and compared with those from serum. 1) The extracts obtained from homogenized tissues were centrifuged at 65,000xg and filtered through an Ultrogel AcA 34 column. Among the three major peaks obtained by gel filtration, the second peak fractions were further separated by isoelectric focusing. Isoenzymes of alkaline phosphatase were found only in the second peak. 2) Isoenzymes of alkaline phosphatase were distinguishable with pH 3.5-10 ampholytes. When pH 3-6 ampholytes were used, isoenzymes were more clearly separated, e.g., 4in serum, 5 in intestine and 2 each in the liver, pancreas, and bone. 3) Comparing the bands of the isoenzymes of alkaline phosphatase to those of serum, only the band with 5.04 pI was the same between serum and intestine. These results indicate that several forms of alkaline phosphatase, even though all are from the rat, may exist; and some of the isoenzymes of alkaline phosphatase found in the serum originated from the intestine.

4.Effects of Sulfur Dioxide on Lactic Dehydrogenase-Isozyme.

Alteration of H-and M-isozymfs of Lactic Dehydrogenase(LDH) were observed in the various tissues after exposing the rats to 50ppm and 250ppm of sulfur dioxide. These isozymes of the respective tissue were separated by Diethlaminoethyl(DEAE)-cellulose from tile tissue homogenates of brain, lung and muscle, presenting the activities by rate of reduction of nicot inamids-adenine-dinucleotide (NAD+). Pure LDH and the coenzyme NAD (NAD) were directly treated with sulfur dioxide in vitro order to find out the direct to sulfur dioxide on LDH and NAD+ and the results were as follow. 1. In the normal tissues, the H-isozyme activity was dominant in the brain and heart, and tole M-isozyme in the muscle. 2. In the lung tissue of normal rats, there was no different between tole activity of H-and M-type of LDH. 3 When rats inhale sulfur dioxide gas in concentration of 50ppm and 230ppm, it appeared that the H-type tend to be suppressed in aerobic tissues and the M-type in anaerobic tissues. 4. In the lung tissue exposed to sulfur dioxide, both the LDH activities were suppressed. 5. It seems that LDH and the coenzyme (NAD ) are not directly affected by exposing in sulfur dioxide gas.

5.Postnatal changes of creatine kinase and isoenzyme activities in normal and asphyxiated newborns.

This study was conducted to evaluate the postnatal changes of serum creatine kinase (CK) and its isoenzymes in normal and asphyxiated newborns. In normal newborns total CK, CK-MM and CK-MB reached a plateau between 2 and 24 hr after birth whereas the serum CK-BB remained stable after birth. CK-MM, CK-MB and CK-BB respectivily composed 90~94%, 5~7% and 1~5% of total CK during study period. In asphyxiated newborns total CK, CK-MM and CK-MB reached maximal value at 12 hr after birth whereas CK-BB decreased after birth. There were no significant differences between normal and asphyxiated newborns in total CK, CK-MM and CK-MB during study period (up to 24 hrs after birth). But CK-BB of asphyxiated newborn was elevated significantly (p<0.05) within 6 hrs after birth compared to normal newborns. There were no significant differences of CK and its isoenzymes between patients evaluated by 5 min Apgar scores(0~3, 4~6 and 7~10) or degree of HIE (HIE stage 0~I, II and III). According to these results, the serum CK-BB is elevated in asphyxiated newborns during 1st 6hrs after birth but has no predictive values of the extent of cerebral damage.

PURPOSE: This study was conducted to analyze the clinical manifestation and natural course of benign transient hyperphosphatasemia (BTH) in children. METHODS: A total of 17 children diagnosed with BTH between June 2006 and July 2007, were included in this study. Clinical records and laboratory tests were analyzed retrospectively for all children. RESULTS: The mean age of the patients was 26.9+/-13.8 months (range: 9 to 49 months). Nine of the 17 BTH cases (52.9%) occurred in patients that had previously suffered from infectious diseases. While the patients were suffering from BTH, the mean serum level of alkaline phosphatase (ALP) was 2,500.2+/-1,165.1 U/L, however, these elevated serum ALP levels normalized within 7 weeks of recovery in all cases. In addition, a seasonal peak in the onset of BTH was observed from September to December. Further, the measurement of ALP isoenzymes in 7 of the patients with BTH, revealed a marked elevation of bone fraction. CONCLUSION: It is important for pediatricians to understand the clinical manifestation and benign course of BTH to avoid unnecessary diagnostic evaluation in children showing a transient increase in ALP activities.

8.Clinical Evaluation of Traumatic Sternal Fracture.

Sternal fractures, once thought of as an uncommon phenomenon, have occurred with an increasing frequency, paralleling the incidence of motor vehicle accidents. The tremendous force necessary to cause sternal fracture and this bone's prominent position overlying major intrathoracic and mediastinal structures, have important implications in the assessment and treatment of patients. This evaluation is based on the review of 72 patients of traumatic sternal fracture treated at the Department of Thoracic and Cardiovascular Surgery, Seoul Adventist Hospital during the last 4 years from March 1993 to February 1997. The frequency was 12.2% of nonpenetrating chest trauma and average age was 43.2 years old. Automobile accidents (84%) and sternal body fractures (95.8%) with anterior displacements (19.4%) was the most common cause and fracture site. Increase of cardiac isoenzymes was more frequent and higher in sternal fracture than chest contusion but there was no relationship between the time to take normalization of them and the mode of trauma.

9.Regional Distribution of Lactate Dehydrogenase Isozymes in Rabbit Brain.

The resolution of lactate dehydrogenase isozymes in tissue samples obtained from various regions of the rabbit brain was carried out by cellulose acetate electrophoresis. 1) Most regions of the brain showed an H-type isozyme pattern. 2) Five clear1y differentiated patterns of isozyme activity were found throughout the entire cerebral cortex with no difference between the lobes of the cerebral cortex. 3) All 5 patterns were found in the upper brain, while 4 patterns-LDH-1, 2, 3 and very 1ow activity of LDH-4 were found in the lower brain.

10.Changes of serum CK and LD Isoenzymes at Tourniquet Application.

Serum creatine kinase (CK) and lactic dehydrogenase (LD) isoenzyme concentrations were studied in 44 patients during orthopedic operations that required the application of a pneumatic tourniquet. CK and LD isoenzymes were measured at preoperation, during tourniquet, and after tourniguet release. In the general anesthesia patients, LD isoenzyme values were not significantly changed during and after touniguet release. CK-BB was more significantly increased (p<0.05) after tourniguet (2.65+/-5. 40IU/l) than at preoperation (0.36+/-1.17IU/l). In the regional block patients, LD isoenzyme values were not significantly changed during and after tourniguet application. CK-BB was more significantly increased (p<0.05) at after tourniguet (0.90+/-1.95IU/l) than at preoperation (0.12+0.48IU/l). But regardleas of tourniguet application time, the values of LD isoenzymes were not significantly changed. CK-BB values were more significantly (p< 0.05) increased after tourniguet release than at preoperation, regardless of tourniguet application time.
