1.International Comparisons of Economic Burden of EndStage Renal Disease to the National Healthcare Systems

The global prevalence of chronic kidney disease (CKD) is between 11 to 13%. Renal replacement therapies
(RRT) – which include dialysis and renal transplantation – consume a significant portion of a country’s health
resources even though only 0.1% of all CKD patients are at stage 5, also known as end-stage renal disease
(ESRD). The aim of this review was to compare the magnitude of the economic burden of ESRD on national
healthcare systems of selected countries with high prevalence of ESRD including Malaysia. The quantity of
interest in this review were total ESRD expenditures and its proportions to the national health systems'
expenditure. It was identified that total ESRD expenditure contributes between 0.91% to 7.1% of national
health system expenditure in countries selected for this review. In Malaysia, the public sector - through its
various agencies at the federal and state levels, accounted for almost 70% of dialysis funding; the remaining
30% came from the private sector and out-of-pocket payments. The ESRD expenditures in Malaysia
constitutes 4.2% of total health expenditure by the public sector, relatively high compared to other
countries. This review will summarise findings of the currently available economic evaluations of RRT in
Malaysia. Based on available evidence, estimated weighted cost of treating ESRD patient in Malaysia is
MYR39,346 per patient per year (USD26,648, PPP 2016).
2.Minimal Invasive Plate Osteosynthesis (MIPO) Technique Using Anterolateral Approach for Treating Closed Proximal Humerus Fracture

Proximal humerus fracture is the second most common
fracture of the upper extremity and presents several unique
problems such as anatomical complexity, high risk of
avascular necrosis, minimal bone stock for purchase,
significant morbidity, and lack of a universally accepted
treatment. Recent treatments for proximal humerus fractures
include use of minimally invasive plate osteosynthysis
(MIPO). The aim of this cross-sectional study was to
evaluate the outcomes of our less invasive technique using a modified anterolateral approach for treatment of proximal
humerus fractures. Ten such operative procedures were
performed in patients of varying age and with varied
mechanism of injury from 2002-2011. All cases were
conducted in an acute setting. There were no cases of
infection and the functional outcome scores were good. This
approach represents an alternative treatment for closed
proximal humerus fracture but more extensive studies are
needed.
3.Temperature related storage evaluation of an RT-PCR test kit for the detection of dengue infection in mosquitoes.

The rapid detection of dengue infection in mosquito vectors is important for early warning to forestall an outbreak. Reverse Transcriptase-Polymerase Chain Reaction (RT-PCR) provides a rapid method for dengue detection in man and mosquitoes. An RT-PCR kit developed by the Medical Entomology Unit, Institute for Medical Research to detect dengue infection in mosquitoes, was tested for its shelf life at 3 storage temperatures: room temperature, refrigerator and freezer. Test kits were tested once every 3 days for kits stored at room temperature, and once every week for those stored at refrigerator and freezer temperatures. The results showed that the test kit could only be stored above its recommended storage temperature of -20 degrees C for not more than 3 days. DNA 100 bp markers in the kits appeared to be stable at the tested temperatures and were usable up to the 20th day when stored at 2 degrees C and below.

4.Pesticide Application, Dermal Exposure Risk And Factors Influenced Distribution On Different Body Parts Among Agriculture Workers

Agriculture sector accounts significant numbers of injuries and fatalities in the workplace particularly related to pesticide management. Among three main pathways of pesticide exposure, dermal contact is the most common route, which exposure usually occurs during pesticide mixing/loading, application, harvesting and other farming activities. This review aims to present and discuss several vital components of pesticide dermal exposure among agriculture workers, as well as pesticide application in agriculture sector in Malaysia involving different commodity agriculture sub-sectors. Pesticide exposure was discussed from perspective of three pesticide management activities (i.e. preparation, application and cleaning) that contribute to the risk of exposure through three routes (i.e. emission, deposition, transfer). Moreover, this paper also discussed pesticide dermal exposure risk assessment methods which can be defined into exposure assessment and effect assessment. The exposure rate was affected by various factors such as application equipment, application rate and duration, type of pesticide formulation, pesticide management stage, usage of personal protective equipment, training and aptitude of the applicator as well as environmental factors (i.e. temperature, humidity, wind speed and direction). The factors mention earlier have been used to explain the exposure distribution over different parts of the body and support the fact that pesticide type was not a major factor in total exposure.

5.Adverse Drug Reaction with Midazolam Use in Emergency Department

Midazolam is one of the most commonly used drugs for sedation in Emergency
Department (ED). This was a retrospective study conducted on 380 patients from December 2012 to May 2014 in ED of Universiti Kebangsaan Malaysia Medical
Centre (UKMMC). The objective was to elicit the frequency of side effects and
correlation to various factors i.e. socio-demography, co-morbidities, age groups
and underlying illnesses. Out of 380 patients, 35 patients experienced side effects
(20 patients with midazolam alone, 15 patients with combination of drugs). The
average age was 42 years and the average dose of midazolam was 3.5mg. The
most common other drug combined was fentanyl. The overall complication rate
for midazolam was 5.3%. The most common side effect recorded was excessive
somnolence (1.6%). Other side effects included local skin reactions (1.1%), vomiting
(0.8%), headache (0.8%) and hypotension (0.5%). There was no significant
association between the socio-demographic factors and drugs combination with
the side effects of midazolam on patients. It was concluded that midazolam was a
safe drug due to absence of any life-threatening side effects. There are possibilities
that most side effects recorded could be caused by other comfounding factors e.g.
underlying injuries or disease and combination with other drugs.

6.Comparison of dexmedetomidine and dexamethasone for prevention of postoperative nausea and vomiting after laparoscopic cholecystectomy.

BACKGROUND: Postoperative nausea and vomiting (PONV) are common following laparoscopic cholecystectomy (LC). Dexamethasone has been reported to reduce PONV. However, there is insufficient evidence regarding the effect of dexmedetomidine in decreasing PONV. This study was designed to compare the effects of a single dose of dexmedetomidine to dexamethasone for reducing PONV after LC. METHODS: Eighty-six adult patients scheduled for LC were randomized to receive either single dose 1 microg/kg of dexmedetomidine (Dexmed group, N = 43) or 8 mg dexamethasone (Dexa group, N = 43) before skin incision. During the first 24 h postoperatively, the incidence and severity of PONV were assessed. Pain and sedation scores were assessed on arrival in the recovery room and early postoperatively. Analgesic and antiemetic consumption during the 24 h after surgery were calculated. Intra-operative and postoperative hemodynamics were recorded. RESULTS: Twenty-one percent of the patients in the Dexmed group developed PONV compared to 28% in the Dexa group (P = 0.6). Severity of PONV was similar between the two groups (P = 0.07). Early postoperatively, pain severity was significantly lower in the Dexmed group, but sedation scores were significantly higher. The first analgesic request was significantly delayed in the Dexmed group (P = 0.02). The total amounts of intraoperative fentanyl and postoperative tramadol administered were significantly lower in the Dexmed group. No difference in ondansetron was noted between the two groups. Mean arterial pressure and heart rate were significantly lower in the Dexmed group after administration of dexmedetomidine. No major side effects were reported. CONCLUSIONS: Dexmedetomidine reduces the incidence and severity of PONV, similar to dexamethasone. It is superior to dexamethasone in reducing postoperative pain and total analgesic consumption during the first 24 h after LC.

7.Physical Activity and Sedentary Behaviour among Adolescents in Petaling District, Selangor, Malaysia

Physical inactivity is strongly associated with obesity and an increased risk of cardiovascular disease in
children and adolescents. A cross-sectional study using multistage random sampling was conducted to
determine associations between demographic characteristics, sedentary behaviours and physical activity
among adolescents. Data were collected from 785 (414 males and 371 females) Form four students
attending 15 schools in Petaling District, Selangor using an adapted self-administered questionnaire.
Results showed that more females (50.1%) were physically inactive compared to males (39.6%) (Adjusted
odds ratio (OR): 1.55, 95% confidence interval (CI): 1.12-2.15). Physically inactive adolescents were
less likely to participate in intramural/house league sports (OR: 1.71, 95% CI: 1.19-2.44), school team
sports (OR: 1.45, 95% CI: 1.03-2.04) and individual physical activities outside school (OR: 1.53, 95%
CI: 1.11-2.12) compared to their physically active counterparts. Physically inactive adolescents were
also less engaged in sedentary activities, such as television watching (OR: 0.69, 95% CI: 0.50-0.94),
playing computer/video game (OR: 0.44, 95% CI: 0.28-0.72), talking on the telephone/mobile phone
text messaging (OR: 0.47, 95% CI: 0.32-0.69) and reading (OR:0.45, 95% CI: 0.24-0.86) compared
to those who were physically active. In this study, physical activity coexists with sedentary behaviour
in adolescents. Sedentary activities may not necessarily displace physical activity among youth. In
addition, these data suggest that promoting organised sports in school and outside the school among
youths may be a potential strategy for increasing physical activity in this population.

Child birth is associated with severely painful experience for the parturient, and often exceeds one’s expectations. Even though, severe pain is non life-threatening condition in healthy parturient women, it may lead to undesired neuropsychological consequences. When no analgesia was used, postnatal depression may be more common, and this labour pain leads to the development of post-traumatic stress disorder. Epidural analgesia is now considered gold standard for effective pain relief during labour. We here report a case of a 37-year-old G1 P0 patient at term gestation who successfully used epidural analgesia for labour pain management.
9.The Prevalence and Risk Factors of Sexual Dysfunction in Gynaecological Cancer Patients

Sexual dysfunction is highly prevalent in gynaecological cancer patients. Most of the time, sexual dysfunction in gynaecological cancer is underdiagnosed as there is overlapping of symptoms with other psychological morbidities, interplaying of multiple risks, patients’ reluctance to complain or incompetence of health care
provider to assess. Determining the risk factors of sexual dysfunction in cancer patients enables us to pay more attention to those who are vulnerable and to device
strategies for early detection, prevention and treatment of sexual dysfunction in them. The main aim of the study was to determine the prevalence of sexual dysfunction and
its risk factors in gynaecological cancer patients in Hospital Sultanah Bahiyah, Alor Star, Malaysia. Sexual function of eighty-three gynaecological cancer patients who
were married were assessed with self-rated MVFSFI (Malay version Female Sexual Function Index). Self-rated WHOQOL-BREF (World Health Organization- Quality of Life- 26) which assessed the domains of quality of life was used while MINI (Mini International Neuropsychiatry Interview) was used for diagnosis of major depressive disorder. The prevalence of sexual dysfunction among the married gynaecological cancer patients was 65% (54/83). Sexual dysfunction was significantly associated with low education level (OR 3.055, CI 1.009-9.250), shorter duration of cancer(OR 0.966, CI 0.966- 0.998), ongoing chemotherapy (OR 3.045, CI 1.149-8.067),pain perception (OR 3.230, CI 1.257-8.303), absence of sexual intercourse for more than one month (OR 1.862) and three domains of quality of life such as physical health, psychological health and social relationship (OR 0.942, CI 0.908-0.978; OR 0.955, CI 0.916-0.995; OR 0.933, CI 0.894-0.973, respectively). However, sexual dysfunction was not associated with major depressive disorder (χ2
² = 1.224, p = 0.268). The prevalence of sexual dysfunction in gynaecological cancer patients was
comparable to other studies of similar population. Since, the risk factors of sexual dysfunction in gynaecological cancer patients are multidimensional, the process of assessment and management need to be holistic and patient-oriented.

10.Comparison of intrathecal versus intra-articular dexmedetomidine as an adjuvant to bupivacaine on postoperative pain following knee arthroscopy: a randomized clinical trial.

BACKGROUND: Postoperative pain is a common, distressing symptom following arthroscopic knee surgery. The aim of this study was to compare the potential analgesic effect of dexmedetomidine after intrathecal versus intra-articular administration following arthroscopic knee surgery. METHODS: Ninety patients undergoing unilateral elective arthroscopic knee surgery were randomly assigned into three groups in a double-blind placebo controlled study. The intrathecal dexmedetomidine group (IT) received an intrathecal block with intrathecal dexmedetomidine, the intra-articular group (IA) received an intrathecal block and intra-articular dexmedetomidine, and the control group received an intrathecal block and intra-articular saline. The primary outcome of our study was postoperative pain as assessed by the visual analogue scale of pain (VAS). Secondary outcomes included the effect of dexmedetomidine on total postoperative analgesic use and time to the first analgesic request, hemodynamics, sedation, postoperative nausea and vomiting, patient satisfaction, and postoperative C-reactive protein (CRP) levels. RESULTS: Dexmedetomidine administration decreased pain scores for 4 h in both the intrathecal and intra-articular groups, compared to only 2 h in the control patient group. Furthermore, there was a significant reduction in pain scores for 6 h in the intra-articular group. The time to the first postoperative analgesia request was longer in the intra-articular group compared to the intrathecal and control groups. The total meperidine requirement was significantly lower in the intra-articular and intrathecal groups than in the control group. CONCLUSIONS: Both intrathecal and intra-articular dexmedetomidine enhanced postoperative analgesia after arthroscopic knee surgery. Less total meperidine was required with intra-articular administration to extend postoperative analgesia to 6 h with hemodynamic stability.
