1.Management of Irritable Bowel Syndrome.

Management of patients with irritable bowel syndrome (IBS) is based on the positive diagnosis of the symptom complex, limited exclusion of underlying organic disease and institution of a therapeutic trial. As a general approach, physician should establish an effective therapeutic relationship by providing clearly understood explanation to patients of the causes and implications of their symptoms, supported by reassurance and appropriate therapy. Treatment of IBS needs to be individualized, focusing on patients' predominant symptoms. In diarrhea- predominant IBS, loperamide and some antispasmodic agents are efficacious. In constipation-predominant IBS, fiber and bulk laxatives are used empirically, but their efficacy is variable and may aggravate bloating. The 5-HT4 receptor agonist, tegaserod is efficacious in female patients with IBS and constipation. In patients with IBS and abdominal pain, antispasmodics and antidepressants can be used but there is weak evidence of potential benefit. New novel pharmacological agents are being carefully appraised as potential drugs for the future.

4.Studying the therapeutic effects of the traditional remedy of "tu than hoan" in the treatment of irritable bowel syndrome of diarrhea state

Background: Irritable bowel syndrome is a diagnosis of exclusion. It is a functional bowel disorder characterized by chronic abdominal pain, discomfort, bloating, and alteration of bowel habits in the absence of any detectable organic cause."Tu than hoan" is a traditional remedy applied for the treatment of syndromes of traditional medicine which have the similar characteristics to irritable bowel syndrome of diarrhea state of modern medicine. Objectives: The study had two purposes: (1)To evaluate the therapeutic effects of the remedy on the clinical symptoms of patients with irritable bowel syndrome of diarrhea state comparing with Duspatalin; (2)To evaluate the side effects of the remedy. Subjects and method:162 patients diagnosed with irritable bowel syndrome of diarrhea state were treated at Bach Mai hospital from March to August 2005. They divided into 2 groups, the control group included 77 patients and the study group included 85 patients. : clinical test, comparing with controls. Results: 80% of patients recovered from diarrhea; 82.4% of patients with defecation returned to normal; 93.6% stopped mucous feces; 76.5% stopped bellyache. Good therapeutic effect was 61.2% (p < 0.05). Conclusions: Tu than hoan had good therapeutic effects in the treatment of irritable bowel syndrome of diarrhea state. 6.Irritable bowel syndrome in China: a review on the epidemiology, diagnosis, and management.

Irritable bowel syndrome (IBS) is a common functional gastrointestinal disease worldwide. Current guidelines of IBS are mostly based on the western populations and expected to vary in different communities. China has a large population and a vast literature is available on IBS. Due to linguistic variations in the literature, the studies are not widely known and their conclusions thus remain largely obscured to the western medical literature. In this article, we reviewed the published literatures on the investigations of IBS epidemiology, diagnosis, and management in the Chinese population and emphasized the different findings gleaned from the western publications. The detailed literature review will benefit understanding of and promote future study on IBS.

7.Acupuncture combined with massage for treatment of irritable bowel syndrome.

OBJECTIVETo observe the therapeutic effect of acupuncture combined with massage on irritable bowel syndrome.
METHODSNinety-three cases of irritable bowel syndrome were randomly divided into three groups: group of acupuncture combined with massage (group A), simple acupuncture control group (group B), simple massage control group (group C), 31 cases in each group. Their therapeutic effects were compared.
RESULTSThe cured rate and the effective rate in the group A were significantly higher than those in both group B and group C (P < 0.05).
CONCLUSIONAcupuncture combined with massage therapy shows a better therapeutic effect on irritable bowel syndrome, and they should be jointly used in clinical treatment.

8.Diarrhea-predominant irritable bowel syndrome of spleen deficiency and damp excess treated with fire needling therapy with filiform needle and acupoint application therapy: a randomized controlled trial.

OBJECTIVE:
To compare the clinical therapeutic effect on diarrhea-predominant irritable bowel syndrome (IBS-D) of spleen deficiency and damp excess among fire needling therapy with filiform needle combined with acupoint application therapy, simple acupoint application therapy and pinaverium bromide tablets, and explore the mechanism on the improvements in IBS-D.
METHODS:
A total of 150 patients with IBS-D of spleen deficiency and damp excess were randomized into a combined treatment group (50 cases, 14 cases dropped off), an acupoint application group (50 cases, 16 cases dropped off ) and a western medication group (50 cases, 13 cases dropped off ). In the western medication group, pinaverium bromide tablets were prescribed for oral administration, 50 mg per time, three times a day. In the acupoint application group, the herbal plaster of
RESULTS:
Except the score for abdominal pain in the acupoint application group and the scores for abdominal pain and abdominal distention in the western medication group after 7 days of treatment, the scores for abdominal pain, abdominal distention, defecation frequency, stool form and incomplete bowel movement after 7 and 28 days of treatment were all reduced as compared with those before treatment in each group separately (
CONCLUSION
The combined treatment with the fire needling therapy with filiform needle and the acupoint application therapy effectively relieves the clinical symptoms, improves the quality of life and strengthens the immunity in the patients with IBS-D. The therapeutic effect of this combined regimen is better than either simple acupoint application therapy or the oral medication of pinaverium bromide. The mechanism may be related to the regulation of the levels of T lymphocyte subsets and the modulation of the expressions of 5-HT3R and 5-HT4R mRNA in colon tissue.

9.CHU Hao-ran's experience in treatment of diarrhea-predominant irritable bowel syndrome with acupuncture and moxibustion.

Professor CHU Hao-ran believes that the clinical diagnosis and treatment of diarrhea-predominant irritable bowel syndrome with acupuncture and moxibustion should concentrate on the integration of disease and syndrome differentiation. According to the evolution of pathogenesis, this disease is divided into 4 stages, i.e. invasion of exogenous pathogens, liver stagnation and spleen deficiency, spleen and kidney yang deficiency and interaction of cold and heat. Hence, the treatment should be provided by stages. Focusing on regulating the spleen and stomach, a specific acupuncture-moxibustion prescription is proposed, including Tianshu (ST 25), Zhongwan (CV 12), Zusanli (ST 36) and Shangjuxu (ST 37). Additionally, the combination of distal and nearby acupoints is considered and an unique needling manipulation, named the fight between dragon and tiger, is exerted. On the base of syndrome and meridian differentiation, the modern medicine is combined, stressing on the mind regulation and the mutual application of acupuncture and moxibustion in treatment. Eventually, a distinctive treatment system with acupuncture and moxibustion is formed for "regulating the spleen and stomach and differentiating meridians".

The article puts forward the treatment of irritable bowel syndrome through regulating heart vitality since it is held that the pathological factors of the disease lay in dysfunction of heart and intestines as well as disorder of qi circulation. At the same time, the internal-external relationship between the heart the small intestine is discussed from the theory of Brain-gut Axis in modern medicine, which provides theoretical base of modern medicine for the treatment of irritable bowel syndrome through regulation of the heart functions.
