A new rodenticide, RH-787, made in Rhom and Hass co. was introduced in this country because it was known relatively harmless to human and beasts. But many internists found that RH-787 induces diabetes in human after accidental ingestions. The effects of RH-787 on various organ are still controversial but the associaton of RH-787 with diabetes is now recognized through the clinical and experimental studies. The cataract is not infrequently seen in juvenile diabetics of several years' duration. In the case to be reported, the cataract was appeared more rapidly within about 2 years after known diabetes due to RH-787 intoxication than usual diabetic cataracts, and the visual results were good after bilateral extracapsular lens extractions with peripheral iridectomy, without any complication.

The first clinical trial of the Q-switch Nd:YAG laser iridectomy using Coherent model 9900 in Korea was carried out since March of 1985. 51 eye cases(48 patients) of acute and subacute narrow angle glaucoma were treated with a Q-switched Nd:YAG laser during the last one year and half. The results obtained are as follows; 1) Good results could be obtained by setting 3.0~4.9 mj with mean pulse energy of under 50. 2) Nd:YAG laser iridectomy site and normal intraocular pressure were maintained well. 3) Complications were encountered in 7 eyes(13.7%) but most cases were transitory without permanent damages.

In the special cases that iris was not naturally prolapsed from the ab externo incision at the peripheral iridectomy for glaucoma(Barkan, Scheie), the iris could be easily drawn out by applying the iris suction which was designed by author. Kong iris suction is similar to the Bell erisophake, but the cup of the iris suction is transparent so that the prolapsing process through the wound could be seen. The peripheral iridectomy with smallest size of the scleral incision can be performed uing the iris suction without introducing the iris forsceps into chamber.

Persistent remnants of the pupillary membrane are extremely common, but well-formed membrane which was covering almost entire pupil is very rare. We experienced one case a 15 year-old boy who had extensive dense persistent pupillary membrane with visual disturbance in left eye and optical iridectomy was performed with improvement of vision.

41 cases of complicated cataract in leprosy patients were operated on from May 1989 to March 1990 at National Sorok Island Hospital. In all cases, extracapsular cataract extraction was attempted and sector or peripheral iridectomy was performed. Inferior sphincterotomy was not done in any of the cases. The postoperative results were very satisfactory in most of the operating cases. The results are as follows; 1) Corrected vision above 0.1 was obtained in 29 eyes(70.7%). 2) Corrected vision from F.C. to 0.09 was obtained in 8 eyes(19.5%). 3) Corrected vision below H.M. was obtained in 4 eyes(9.8%). 4) 37 eyes(90.2%) among the total 41 showed an improvement of corrected vision.

A case of Vogt-Koyanagi-Harada's Sindrome chiefly affected the anterior uvea has been clinically studied with several references. It was a typical one with hearing disturbances and was complicated with a secondary glaucoma. Following the iridectomy the I.O.P. was back to normal limit and with steroid therapy the inflammatory signs were relieved. After a considerabie period the anterior uveitis recurred. In this case the age of the patient was 10 years older than those Parker presented.

During the last one year. 23 selected cases of various glaucomatous eyes were received trabeculectom y ab extemo (5 X 3mm scleral-flap with a large peripheral iridectomy and only two scleral-flap corners sutures) under surgical microscope. Success cases which were controlled ocular tension to normal are 21 eyes(91.3%) out of total 23 glaucomatous eyes. Especially, all 11 eyes of simple open angle glaucoma were obtained excellent normotension after trabeculectomy, But one case of absolute glaucoma was failed because of vitreous prolapse, and another failed case was in group of acute closed glaucoma due to surgical complication of total hyphema(Table 1, 2).

Authors found a case with a slight trauma in which two eye lashes penetrated into the anterior chamber. This 25 years old male patient came to our clinic 2 weeks after trauma on right eye and eyelid, complaining of photophobia, foreign body sensation and ocular pain which were persisted for two weeks. On slit-lamp examination, two eye-lashes on iris surface were found in the anterior chamber of his right eye and a dot like corneal superficial opacity was also found on 6 o'clock meridian near the limbus, but that is questionable whether it is penetrating wound or not. No other foreign body was noticed in the same chamber. The eye lashes were removed surgically and the one of them was carried out by iridectomy. The literatures were briefly reviewed for the mechanism of the penetration of the cilia was very mterestmg.

PURPOSE: This study was aimed to evaluate the effect of subconjunctival retention of perfluoropropane gas on the success rates in trabeculectomized rabbits. METHODS: Fifthteen white rabbits were trabeculectomized by fornix-base conjunctival flap, full thickness sclerotomy and iridectomy. The right eye was control and the left eye was injected with 0.2 ml perfluoropropane gas(C3F8) into subconjunctival space through the 30 guage long needle. Each rabbit was examined for bleb morphology under slit-lamp microscope and measured for intraocular pressure by tonopen at preoperative, postoperative 3 days, 1 week, 2 weeks, 4 weeks, and 6 weeks. The results were analyzed by Wilcoxon rank sum test in SAS release 6.12 program. RESULTS: Mean gas retention time was 15+/-2 days. Intraocular pressure measured at postoerpative 2 weeks was statistically significantly lower in gas injected group than control group. At postoperative 2 weeks, fibrovascularization was more prominent in control group than gas injected group. Fibroblast proliferation and collagen tissue formation were more prominent in control group than gas injected group at postoperative 6 weeks. CONCLUSIONS: Subconjunctival perfluoropropane gas(C3F8) bubbles, via "spacer" effect, aid the maintenance of filtering blebs in the early postoperative period. This technique may be useful in augmenting or salvaging blebs at high risk of failure.

Primary or secondary IOL implantation after cataract extraction was performed in 41 eyes of 36 lepromatous leprosy patients, who showed negative skin test, from October 1990 to April 1992 at Chilgok Catholic Skin Clinic. Mean follow-up period was 8.3 months. Visual improvement was achieved in 39 eyes (95.1%), including 25 eyes (61.0%) of corrected vision better than 0.5. There was no serious complication induced by IOL implantation. But photophobia was complained in 18 eyes (66.7%) of 27 iridectomized eyes. We think that we may as well implant IOL after cataract extraction in lepromatous leprosy patients with complicated cataract if there is no active inflammatiQn in anterior chamber, guarded iridectomy is performed and it is possible to follow up considerably.
