4.Observation on therapeutic effect of TCM iontophoresis on chronic hepatitis with splenomegaly.

OBJECTIVETo search for an effective therapy for chronic hepatitis with splenomegaly.
METHODSFive hundred and seventy eight cases of chronic hepatitis with splenomegaly were randomly divided into a treatment group (n=292) and a control group (n=286). The two groups were treated with intravenous drip of hepatocyte growth-promoting factors and Danshen Injectio, with TCM iontophoresis added to the treatment group, once daily, 20 sessions constituting one course. After one course, their therapeutic effects were observed.
RESULTSBoth the cured rate and the effective rate on splenomegaly were 66.1% and 98.6% in the treatment group, which were higher than 18.2% and 76.2% in the control group. The treatment group in the therapeutic effect in improvement of liver function and attenuation of the spleen and remission of hyperfunction of the spleen was better than the control group (P < 0.05).
CONCLUSIONTCM iontophoresis has obvious therapeutic effect on chronic hepatitis with splenomegaly.

6.Dermatological Applications of Iontophoresis.

Transdermal drug delivery is applicable to only a few drugs because the stratum corneum is an excellent barrier to percutaneous absorption. Iontophoresis is a non-invasive technique which increases the drug deliveries into the skin through an electric current. In iontophoretic systems, the movement of ionized drugs is facilitated by electrical energy according to the basic electrical principles of "like charges repel each other and opposite charges attract". Iontophoresis has been explored for many dermatological applications with considerably successful reports. This article reviews the principles and advantages/limitations of iontophoresis, the factors affecting iontophoresis and its applications in various dermatological conditions.

Radio frequency and iontophoresis are two commonly used types of medical skin care equipment, utilizing electrical energy. Radio frequency technology has become an important treatment in aesthetic medicine, and has many applications, including non-ablative tightening of the cutaneous tissue to improve skin laxity and to reduce wrinkles. Iontophoresis on the other hand uses a small electric charge to deliver medicine or other chemicals through the stratum corneum, the principle barrier to percutaneous absorption, without the use of a needle. It is used mainly for aesthetic reasons, including treatment of melasma, and for anti-aging therapy. Effective cosmetic results have been demonstrated for both these techniques in a number of studies. However, recklessness and incorrect use of the machines by unqualified practitioners can cause side effects. Here, we review the possible side effects and related coping strategies associated with radio frequency and iontophoresis.

8.Effect of NaF iontophoresis and Nd:YAG laser irradiation on the abrasion-resistance of root surface.

The purpose of this study was to evaluate the abrasion-resistance of root surface after NaF iontophoresis, Nd:YAG laser irradiation and combined treatment 50 anterior teeth with flat interproximal root surface that had been extracted due to periodontal destruction were selected. All teeth were treated by the same procedure as conventional periodontal root treatment, such as scaling and root planing, root conditioning with tetracycline HCl(100mg/ml, 5min). The pre-treatment weight of each tooth was measured by a dial scale(SHIMADEU Co., LIBROR EB-220HU, capacity 220.000 g, Japan). All teeth were divided into 5 groups as follows; Nd:YAG laser irradiation(group 1, 1 W, 100 mJ, 10Hz, fiberoptic-root surface distance=5mm, 10 sec.x6times, EL.EN.EN060, Italy); NaF iontophoresis(group 2, 150micronA, 4 min.); Nd:YAG laser irradiation following NaF iontophoresis(group 3); NaF iontophoresis following Nd:YAG laser irradiation(group 4); No treatment(control group). Electric toothbrushing (Oral-B, Brown Co., Germany) was conducted during 1 hour(10 min.x6 times). Subsequently post-treatment weight was remeasured by the same method as pre-treatment weight measurement. The difference of abrasion rate among all groups was statistically analyzed by ANOVA(SAS program). Following results were obtained; 1. The abrasion rate was significantly lower in Nd:YAG laser irradiation group than NaF iontophoresis group(p < 0.001). 2. The abrasion rate was significantly lower in combined groups of Nd:YAG laser irradiation and NaF iontophoresis than either Nd:YAG laser irradiation group or NaF iontophoresis group(p < 0.001). 3. There was no significant difference in abrasion rate according to application order in the combined groups(p > 0.05). 4. The abrasion rate was significantly lower in all experimental groups than control group(p < 0.001). The results suggest that combined treatment of Nd:YAG laser irradiation and NaF iontophoresis on exposed root surface after periodontal therapy can enhance the abrasion-resistance of root surface and may inhibit the root caries development.

9.Vitamin C-iontophoresis in Melasma.

BACKGROUND: An inhibitory effect of ascorbic acid(AsA) on melanogenesis has been described. Iontophoresis is emerging technologies capable of enhancing drug penetration through stratum corneum, the principal barrier to percutaneous absorption. OBJECTIVE: Our purpose of this study was to evaluate the efficacy of vitamin C-iontophoresis in patients of melasma. METHODS: The treatment was performed twice weekly in 15 volunteer patients for a period of 6 weeks by application of vitamin C under a constant direct current of 0.4-0.8A for 15minutes. Clinical evaluations using the Melasma Area and Severity Index(MASI) and bioengineering measurement using Chromameter CR 300(R) were taken before and after treatment. RESULTS: Decreased MASI and light reflectance were noted at the end of 6 weeks, significant clinical improvement in the melasma was seen compared to before treatment. CONCLUSION: Vitamin C-iontophoresis is an effective, noninvasive treatment of melasma without significant side-effects.

10.Treatment of Palmar Hyperhidrosis with Tap Water Iontophoresis: A Randomized, Sham-Controlled, Single-Blind, and Parallel-Designed Clinical Trial.

BACKGROUND: Palmar hyperhidrosis is a common disorder of excessive sweating. A number of studies have demonstrated the effectiveness of iontophoresis in the treatment of palmar hyperhidrosis. However, controlled clinical studies on iontophoresis for palmar hyperhidrosis have been limited. OBJECTIVE: To determine the efficacy and safety of iontophoresis in the treatment of palmar hyperhidrosis with a randomized, sham-controlled, single-blind, and parallel-designed study. METHODS: Twenty nine patients with significant palmar hyperhidrosis were enrolled in this study. They received active iontophoresis treatment (group A) or sham treatment (group B). Iontophoresis was performed 20 minutes each time, five times per week, for 2 weeks. Its efficacy was assessed with starch-iodine test, mean sweat secretion rate, and hyperhidrosis disease severity scale. RESULTS: Twenty-seven of the 29 patients completed the 2-week treatment. After completion of 10 times of treatment, results of the starch-iodine test showed clinical improvement in 92.9% of patients in group A and 38.5% of patients in group B (p=0.001). The mean sweat secretion rate was reduced by 91.8% of patients in group A and by 39.1% of patients in group B (p<0.001). Improvement in quality of life was reported by 78.6% of patients in group A and by 30.8% of patients in group B (p=0.028). In group A, one case of localized adverse event was noted, although no adverse event was encountered in group B. CONCLUSION: Tap water iontophoresis could be used as an effective and safe treatment modality for palmar hyperhidrosis.
