STATEMENT OF PROBLEM: Direct denture reline resins tend to discolor during service in the oral environment by intrinsic and extrinsic factor. PURPOSE: This study was designed to evaluate the color stability of direct denture reline resins. MATERIAL AND METHODS: Mild Rebaron(GC Corp., Japan), Meta Base M(Sun medical Co., Japan), Mild Rebaron LC(GC Corp., Japan) and as a control group, Vertex SC(Dentimex Zeist, Holland) were chosen for this study. Ten specimens of each direct denture reline resins were fabricated. Treatment methods designed for this study were the coffee staining test(7days) and the accelerated aging test(100hours). The color changes before and after treatment were measured by Tristimulous colorimeter(Yasuda seiki seisakusho, Ltd. Japan) and analyzed. RESULTS: All the direct denture reline resins subjected to the coffee staining test and the accelerated aging test showed noticible difference in color change. After coffee staining test, Meta Base M showed the highest color change followed by Vertex SC, Mild Rebaron LC and Mild Rebaron. There were no statistical differences between Meta Base M and Vertex SC and between Mild Rebaron LC and Mild Rebaron(p>0.05). After accelerated aging test, Mild Rebaron LC showed the highest color change followed by Vertex SC, Meta Base M and Mild Rebaron. There were no statistical differences only between Mild Rebaron and Mata Base M(p>0.05) but among the others, there were statistical differences(p<0.05). CONCLUSION: Within the limitation of this study, all the direct denture reline resins subjected to the extrinsic and intrinsic factors showed noticible difference in color change, and there were differences among manufacturers.

PURPOSE: To study the relationship between the distribution of the ocular surface temperature and the nasal predominance in pterygium. METHODS: In order to identify the distribution of the ocular surface, the surface temperature was measured at four points on the eyelids and on the bulbar conjunctiva with a non-contact thermometer. The ocular surface temperature in the pterygium patient group was compared with that in a normal control group. The temperature difference at each point on the ocular surface was also examined. RESULTS: In both of the patient and the normal groups, the surface temperature on the eyelid was at its lowest on the temporal lower eyelid, and highest on the nasal upper eyelid. On the bulbar conjunctiva, the surface temperature of the nasal conjunctiva was at its lowest, and at its highest on the upper conjunctiva. The most distinctive temperature difference between the patient and control groups was shown on the nasal and temporal bulbar conjunctivas, and statistically significant (p<0.05). CONCLUSIONS: Considering the temperature distribution over the ocular surface, we propose that an intrinsic factor that causes the nasal predominance of pterygium could be the comparatively low local temperature of the nasal bulbar conjunctiva.

The cause of ureteropelvic junction obstruction(UPJ) is almost due to intrinsic factor. But a small percentage of UPJ obstruction have extrinsic obstruction as a result of a crossing renal aberrant vessel. The diagnosis of UPJ obstruction by aberrant vessel is usually made at surgery because the urographic findings are nonspecific and difficult to find cause. For the purpose of detecting aberrant vessel as a cause of UPJ obstruction, authors examined 20 patients with color Doppler ultrasonographic equipment, then cause of obstruction was confirmed at the time of surgery, from July 1991 to February 1994. In seven of twenty patients aberrant vessel was detected by color Doppler ultrasonographic equipment. Among the seven patients, only three cases were caused directly by aberrant vessel, color Doppler examination disclosed a vessel or vessels at the tapering distal end of UPJ. In other two patients, aberrant vessel was detected but it was not direct cause of UPJ obstruction. Even in two of seven patients, aberrant vessel was not detected at the time of surgery. However in one of thirteen patients with no color Doppler signal at the UPJ, aberrant vessel was confirmed in surgical field. Sensitivity was 75 % and specificity was 75 % as detecting aberrant vessel by color Doppler ultrasonographic equipment. We believe that color Doppler examination is a useful method in demonstration of aberrant vessel as a cause of UPJ obstruction, although accuracy was low.

Dental erosion is defined as tooth structure loss by acidic chemical substance. It is caused by extrinsic factors such as acidic foods, drugs, and working environments, and also intrinsic factors such as gastric acid regurgitation in gastro-esophageal disorder or intensive vomiting in bulimia nervosa. These lesions can be treated with direct resin filling, laminate or full contour crown depending on the intensity of clinical problem. This is a case report about treatment of rare clinical case: labial erosion of anterior tooth caused by frequent intake of acidic fruit and palatal erosion of anterior tooth caused by intensive vomiting in bulimia nervosa.

BACKGROUND: Melanocytes synthesize melanin pigment by the action of specific enzyme tyrosinase. In normal skin, melanocytes maintain their constant population and steady melanogenic activity. But several stimuli to the skin such as UV irradiation and chemical irritation may accelerate the proliferation and differentiation of melanocytes and subsequently lead to hyperpigmentation of the skin. Endothelin-1(ET-1), produced by human keratinocyte acts as a strong mitogen on human melanocyte and has an effect on paracrine linkage between keratinocytes and melanocytes, and plays an important role in UVB induced hyperpigmentation as an intrinsic factor. OBJECTIVE: In this study, the role of ET-1 on melanocyte proliferation and melanogenesis was investigated, and the effects of ETRA(endothelin receptor antagonist) in cultured melanocyte was also evaluated. METHODS: Primary melanocytes from neonatal foreskins were cultured in complete MCDB 154 medium. To examine the effects of ET-1 on melanocytes, cultuled melanocytes were treated with ET-1 at various concentration(5nM, 10nM, 15nM, 20nM). Three days later, melanocyte proliferation and melanin content were measured. Also, we examined the effects of ETRA on ET-1 treated melanocytes. Five kinds of ETRA were added into 10nM of ET-1 treated melanocyte. After three days cultuviation, inhibitory effect of ETRA on ET-1 activity was measured. RESULTS: 1. The number of melanocytes increased in the groups of incubation with melanocyte medium of concentration in 10nM ET-1 than others. 2. The melanin contents increased most in the groups of incubation with melanocyte medium of concentration in 10nM ET-1 than others. 3. The number of melanocytes decreased in the groups of inhibited with ETRA I. 4. The melanin contents decreased in the groups of inhibited with ETRA II. CONCLUSION: This study provided an important confirmation of the proposal that ET-1 is intrinsic factor for proliferation and differentiation of human melanocytes, and ETRA has inhibitory effect on ET-1 induced melanogenesis and melanocyte proliferation. These findings suggest that ET-1 is an important mediator in epidermal pigmentation, and we will be able to use ETRA as a new whitening agent.

With increased participation in sporting activity, overuse tendon injuries are a major problem in sports medicine. Tendinopathy is generic descriptive term for the clinical conditions in and around tendons arising from overuse. Tendinopathy is debilitating condition that results in significant deficits in performance and prolonged time away from activity. Histological studies show either absent or minimal inflammation. There are three main theories to explain the development of tendinosis, including the mechanical theory, vascular theory, and neural theory. Risk factors are divided into intrinsic factors and extrinsic factors. The management of tendinopathy revolves around modulating tendon pain, as pain is the presenting and limiting factor for activity. Despite an abundance of therapeutic options, the scientific evidence base for managing tendinopathies is limited. The aim of this review is to report the options for most widely used conservative management of tendinopathy.

PURPOSE: To systematically review research articles that examined factors affecting Korean clinical nurses' intention to stay in their jobs. METHODS: Articles related to Korean clinical nurses and published both in Korean and English languages were retrieved from computerized databases using a manual search. Data extraction, quality assessment, and analysis including meta-analysis were completed. RESULTS: The review included 15 studies. There was strong evidence for the effects of 19 factors on Korean clinical nurses' intention to stay. Factors were categorized into three major categories; demographic, extrinsic, and intrinsic factors. In the meta-analysis of 10 articles, career experience of more than 6 years was the main factor that led to high intention to stay (OR=−0.5, p < .001). Job performance related factors (28.1%) and nursing work environment or organizational climate for caring (21.9%) were studied as the main extrinsic factors. Job related attitude (28.1%) including job satisfaction and organizational commitment and nursing value (15.6%) were studied as the main intrinsic factors. CONCLUSION: Expert nurses are more likely to retain their current positions as well as quality workplace environments or psychological capitals. Healthcare organizations must be responsible for improving professional and psychological capital of beginner nurses and affecting change for a warm organizational environment.

Falls are the most common cause of accidents among the older population, leading to both fatal and non-fatal injuries. Falls is a syndrome resulting from the cumulative effect of various extrinsic and intrinsic factors. It is considered to be a multifactorial disorder. Medication use is considered a risk factor for falls. We reviewed medications associated with falls in older individuals. In geriatrics populations, polypharmacy is associated with falls. Medical doctors should be aware of the possibility that starting a new medication, such as antihypertensive agents, benzodiazepine, antidepressants, opioid agents and antihistamines, may act as a trigger for the onset of a fall.

Objective: To explore the positive rate of intrinsic factor antibody (IFAb) and level of vitamin B(12) (VitB(12)) in normal physical examination population and the possible relation between IFAb, VitB12 and sex, age, number of RBC, HGB and MCV. Methods: A total of 1 427 people who came to Peking Union Medical Colleague Hospital (PUMCH) for physical examination were enrolled. There were 758 males with average age of (52.5±14.5) years-old and 669 females with average age of (50.3±14.3) year-old. Beckman DxI800 automatic biochemical-immune analyzer and corollary reagents were used to analyze the level of serum IFAb and VitB(12). The results in different sex, age were documented and their correlation with the value of whole blood cell count was tested later on. Results: Among the 1 427 normal subjects, 66 (4.63%) were positive for IFAb. The positive rate for IFAb in the population≥40 years-old was higher than those<40 years-old (5.66% vs 1.48%, χ(2)=7.46, P=0.006). The deficiency rate of VitB(12) in the population<40 years-old, 40-59 years-old and ≥60 years-old was 2.22%, 2.51% and 5.50%, respectively (χ(2)=8.55, P=0.014). There were no difference between people with different sex in the positive rate of IFAb (5.15% for males and 4.04% for females, χ(2)=0.99, P=0.320) or in the deficiency rate of VitB(1)2 (3.83% for males and 2.69% for females, χ(2)=1.44, P=0.230). The results of multiple linear regression showed that HGB level of IFAb positive subjects was 3.05 g/L lower on average than those of IFAb negative, but IFAb had no effect on both RBC and MCV. There was no correlation between VitB(1)2 deficiency and HGB, RBC and MCV. Conclusion: The positive rate of IFAb and deficiency rate of VitB(1)2 increase as age increases. But the presence of VitB(12) deficiency is later than the positive findings of IFAb. IFAb showed some effects on the level of HGB, which may compensate the limitations of VitB(12) detection to some extent. It is necessary to check the IFAb and level of VitB(12) in people with middle or old ages.

Plantar corn is a circumscribed and sharply demarcated hard mass of traumatic hyperkeratosis, which has a central core impacting the dermis deeply and causes pain while walking. The cause of the corn is the mechanical stress to the skin induced by several causative factors; extrinsic(tight shoes) or intrinsic(bony prominence), or combined(the claw toe). We found 7 cases of patients with a painful recurrent plantar corn which had an epidermal cyst under its lesion. These corns and cysts were excised totally and there was no recurrence in all cases during two to four months follow-up period. We think that an epidermal cyst may be another intrinsic factor for the development of a plantar corn. So, an epidermal cyst should be considered once in a recurrent plantar corn to be recalcitrant against popular treatments.
