1.A Mycological and Epidemiological Study of Otomycosis.

As increasing numbers of antibiotics, adrenocorticosteroids, and antitumor agents have come into use, so increasing numbars of mycotic diseases have been identified. Qtomycosis, a mycotic infection of the extcrnal ear which is occasionally accompanied by dermatitis of the surrounding skin, is corsidered to be such a disease. While opinion vari.es as to whether otomycosis is caused by a saprophytic or a pathogenic organism, our clinical obs:rvations revealed pathological inflammation of the external auditory canal with profuse fungal elements. During August 1975, l3 specimens of ear wax were taken from patients whv visited the out-patient ENT clinic of this hospital. All patients had had their ears clea,ned at a barber shop prior to visiting the ENT clinic. Because ar cleaning instrumenta (scoops and brushes) are re-used m Korean Sarber shops, 204 of theae in- strum.ent;s were randomly selected from different barber shops and examined. KGH mounts and Lactophenol cot@ten blue stains were used to examine the specimens. Qf the lR ear wax specimens, I2 samples (92. 3%) reveaIed Aspergillus sp. and 1 sample (7. 7%) revealed Penicillium sp. Of the 204 ear cleaning instrumenta tested, l94 colonies grew in culture. Of these cultures, 146 (75. 3 %) were Aspergillus sp., 9 (4. 6%) were Mycelia sterila, 5 (2. 6%) were Fenicillium sp., 4 (2. 1 %) wr Al- terna,ria, 2 (l. 0%) were Stemphylium sp., 1 (0. 5%) was Geotrichum sp., and 27 (13. 9%) were of an unknown species. Of the 146 Aspergillius sp. cultures, 99 (67. 8%) were Asp. terreus, 17 (11.6%) were Asp. glaucus, 5 (3.4%) were Asp. niger, 17 (11. 6%) were Asp. flavus-oryzRC, R (2. 1 %) Were Asp. versicolor, and 5 (3. 4 %) were athers. Beca.use of the close correlation between the fungi cultivated from ear wax apc imens and those cultured from ea,r cleaning instruments, the authors believe that the ear cleaning instruments used in the barber shops could well have been the primary source of infection. In this study the authors also discuss the probable causative factbrs of otomycosis arrd a review of the literature is presented.

3.Leiomyosarcoma of the Skin: Report of A Case.

Leiomyosarcoma arising in the skin is rare tumor, and diagnosis usually is made microscopically. After local excision, these lesions recur in large proportion of pat ients. The authors herein report a 53-year-old male with leiomyosarcoma appeared in the skin of the right forearm and presenting as a dark reddish colored, 5*6cm in diameter, superficial ulcerated single firm nodule with intermittent pain. Histopathological examination showed poorly circumscribed tumor consisting of interlacing bundles of spindle shaped smooth muscle cells in the middle and lower parts of the dermis. The nuclei were hyperchromatic, large, vacuolated, and irregular in shape. Electron microscopic findings revealed cytoplasmic organelles such as rough endoplasmic reticulum and mitochondria of malignant smooth muscle cells in the paranuclear area, Characteristic subsarcoelmmal caveolae and dense plaque were noted and myofilaments were distributed in the peripheral cytoplasm. The tumor did not recur in 10 months' follow-up.

4.Adenosine Deaminase Activities in Sera and Erythrocytes of Patients with Psoriasis.

Psoriasis has been known to have various humoral and cellular immune abnormalities. And adenosine deaminase (ADA) activities are known to be decreased in immune deficiency diseases. The present study was designed to measure the activities of ADA in sera and erythrocytes of psoriasis patients by Giustiss method. There were no significant differences in the erythrocytes ADA activities between normal subjects (9, Gp+4 43 units/10 cells) and psoriasis patients (7, 29+3. 64 units /10 cells). The ADA activities in sera of the psoriasis patients (13. 15+3. 43 units/ L) showed lowered activities than those of normal subjects (20. 44-2, 07 units/L).

5.A Study of Hair Growth in Korean.

The hair growth of the scalp is influenced by many systemic and local factors. So the study of standard hair growth rate is very important for the diagnosis and treatment of some pathological conditions. That study has not been done well in Korea. The present study was to establish the standard hair growth rate of the scalp of healthy Koreans at different ages. The investigations were carried out on 509 individuals of both sexes whose ages ranged between 2 and 69 years. The subjects were grouped according to 5 age periods. The results of this study were as follows: 1. The mean value of the daily scalp hair growth rate was 0.401+ 0.037mm. 2. The daily hair growth rate of each groups were as follows: under 10 years of age; 0.367+0.032mm, in the teenagers; 0.419,+0. 032mm, in the twenties0.398+0.037mm, in the thirties and fourties; 0.394+0.034mm, above 50 years of age 0.386+029mm. 3. Sex differences of each age groups in the scalp hair growth were not statistically significant.

6.Two Cases of Psoriatic Arthritis.

A psoriatic patient may have rheumatoid arthritis, psoriatic arthritis(or both), osteoarthritis or gout. In so far as possible, each of these must be distinguished on clinical grounds with some help from laboratory tests. Psoriatic arthritis is very similar to rheumatoid arthritis but clinically, it is regarded as a unique disease entity, which is found in 1% to 32% of psoriatic individuals. We herein report two cases of psoriatic arthritis that are thought to be distal type and arthritis mutilans on the basis of clinical, serological and radiological features.

7.A Case of squamous Cell Carcinoma Arising from an Actinic Cheilitis and Its Surgical Treatment.

A sixity-three years old male admitted our department of dermatology showing generallized scaly, fissured, and swollen lower lip, we could diagnose it actinic cheilitis by the finding of skin biopsy specimen. And so we recomrnended him vermilioneciomy for the prophylaxis of squamous cell carcinoma, but he refused. After 2 months, above Iesion was changed a rapidly proliferating protruded mass in the centcr of the lower lip, which was diagnosed squamous cell carcinoma. For the treatment of squamous cell carcinoma and remained actinic cheilitis, we performed V-shaped excision of tumor mass and verrniliomy. After 5 months follow-up evaluation, the patient appeared excellent results without recurrence.

Over the last 20 years, the frequency of multiple pregnancy has increased mainly because of the introduction of exogenous pituitary gonadotropins in the treatment of infertility. Since the advent of assisted reproductive technology, the concern about ectopic implantation of embryos has increased dramatically and it continues to be a major complication of in vitro fertilization and embryo transfer (IVF-ET). Bilateral tubal pregnancy is the least common type of ectopic implantation of two embryos. Of all extrauterine pregnancies, 1:725~1:1580 are bilateral1,2. Simultaneous tubal pregnancies have been reported in natural cycles, recently, after ovulation induction, in-vitro fertilization2,3. The high incidence of ectopic pregnancy associated with IVF-ET continues to be a problem and frequencies of between 2.4 and 12.4% have been only a few reports of simultaneous bilateral tubal pregnancies following IVF-ET4. This paper describes a case of a simultaneous bilateral tubal pregnancy after IVF-ET in a 33 year old patient. The diagnosis was confirmed by diagnostic laparoscopy performed 25 days after embryo transfer(sixth week of gestation), which revealed bilateral tubal pregnancy. Bilateral salpingectomy was performed. With a review of the literature on this topic, diagnostic aspects and treatment options are discussed.

10.Intramural Pregnancy after in vitro Fertilization and Embryo Transfer.

Intramural pregnancy is an unusual ectopic gestation located within the uterine wall, completely surrounded by myometrium and separate from the uterine cavity, fallopian tube, or round ligament. It is known to be difficult to diagnose, and associated with a high rate of uterine rupture. We report a case of intramural pregnancy in which early diagnosis was made and successful treatment was done by dilatation and curettage. Diagnostic laparoscopy confirmed the absence of uterine rupture during the procedure. Therefore, conservation of fertility can be possible with early diagnosis of intramural pregnancy. To our knowledge, this is the first case report of intramural pregnancy following IVF-ET in Korea.
