3.A case of angiolymphoid hyperplasia with eosinophilia and its treatment with carbon dioxide laser.

A 45-year-old female noticed a pruritic erythematous firm pea sized nodule on the left earlobe about 18 years prior to uisiting our clinic. The lesion has increased in size gradually and involved the whole earlobe. On March 1988, the earlobe became enlarged with extreme pruritus, and showed a tendency of easy bleeding by scratch. The skin lesions consisted of multiple pea to bean sized, shiny dusky red colored, moderate firm nodules or tumors with edema on the left. ear helix, antehelix, external auditoy meatus, and retroauricular fold. The histopathologic studies revealed vascular proliferation, presence of histiocyt.oid endothelial cells, and prominent inflammatory infiltration including large numtaer of eosinophils with fibrosis in the dermis. Hematological studies revealed eosinophilia(9%). Excision of several nodules and intralesional injection of trinmcinolon acetonide did not show complete favorable effect and treatment with carbon dioxide laser was attempted. With the patient under local anesthesia with 2% lidocaine, the lesion were treated with three t,o four passes at 25,100 second intervals with ieradiance 5W(cm. The pro- cedure was repeated until no abnormal findings could be seen in the treatment zone. Treatment with carbon dioxide Iaser on the patient obtained a satisfactory result.

4.Immunohistochemical obervation of EGF,TGF - alpha and EGF-R in psoriatic epidermis.

Epidermal growth factor(KGF) and transforming growth factor arpha(TGF-a) are polypeptides of 53 amd 50 amino acid residuies. Both bind to epidermal growth factor receptor (EGR-R) leading to phosphorylation of the receptor, enhancement of its tyrosine-specific kinase activity and ultimately to stimulation of cell growth. To study he role of EGF, TGF-a, and EGF-R in differentiation and hyperproliferation of cell, we se lected psoriasis vulgaris, because the affected keratinocyte may house both an abnormally increased proliferative capacity and an abnormally differentiated state. The biopsy specimens were taken from involved and uninvolved skin of 20 patients with psoriasis and immunoperoxidase studies with formalin-fixed paraffin-embedded tissues were performed with EGF, TGF-a and EGF-R useing the Vectastain ABC irnmunoperoxidase stain system. The antibodies were used at a concentration of 6 ug/ml In involved psoriatic skin, distributions of TGF-a and EGF-R were increased in all layers of epidermis as compared to normal, uninvolved psoriatic skin, in which chev were showed to the basal and parabasal layers. However, distribution of EGF was weekly positive in the basal layers of epidermis in both involved and uninvolved skin with no difference between toem. These results suggest that increased distribution of TGF-a and EGF-R may be involved in hypoproliferative state of epidermal keratinocytes in psoriatic lesion.

5.Effect of Hypo-osmotic Swelling (HOS) Test on Subsequent Post-thaw Testicular Spermatozoa.

OBJECTIVES: We have previous reported that thawed testicular sperm and sperm extracted from seminiferous tubule could achieved optimal fertilization and pregnancy in azoospermic patients. However, thawed testicular sperm did not show motility in many cases. Therefore we studied viability of immotile sperm extracted from frozen-thawed seminiferous tubule using hypo-osmotic swelling (HOS) test and eosin-Y test. MATERIALS AND METHODS: After sperm extraction using for ICSI, the remained sections of seminiferous tubules were frozen with computerized freezer. For thawing and preparation of testicular sperm, the seminiferous tubules were thawed by removing from LN2 and letting them at room temperature for 10 min followed by 37degrees C water bath for 10 min. The prepared samples were washed for free of preservation medium and sperm preparation method described previous. Sperm was suspended in 0.1 ml hypoosmotic solution. After 30 minutes, the type of distally coiled sperm were assessed. RESULTS: In 44 cases of cryopreservation of seminiferous tubules in obstructive azoospermic patients, the fertilization rates with 2PN were 71.4% and pregnancy rates were 34.1%. The presence of motile spermatozoa on subsequent post-thaw testicular sperm remarked 15.1% and were increased to 77.3% just before ICSI. After sperm extracted from frozen-thawed seminiferous tubule, 3 hrs later in in vitro culture, the cases of presence of motile sperm, reaction of hypo-osmotic swelling test and viable sperm were 63.6% (28/44), 93.2% (41/44), and 77.3% (34/44), respectively. CONCLUSIONS: Just after post-thawed testicular sperm did not showed motility. Although motility was gained after in vitro culture, many cases showed non-motile sperm until optimal insemination time. However, HOS test showed positive reaction in non-motile sperm. Therefore, HOS test is an alternative method for the selection of viable sperm for ICSI.

7.Metabolic Changes after Hypothermic Cardiopulmonary Bypass Surgery .

Temperature changes after cardiopulmonary bypass surgery(CPB) markedly affect postoperative ventilation. Despite core rewarming after CPB, on admission to the intensive care unit the rectal temperature is about 34~35 degrees C over the next 12 hours, the temperature rises to 38~39 degrees C. Unless minute ventilation is increased hypercarbia occurs presumably because dead space and/or CO2 production are increased. This prospective review was designed to confirm these clinical impressions and to provide direction for future studies. Temperature changes, PaCO2, pH, sodium and potassium values of 42 patients who had undergone cardiac valvular replacement surgery with hypothemic cardiopulmonary bypass were observed during surgery and during the first 12 postoperative hours in the intensive care unit. The results are as follows: 1.The mean nasopharyngeal temperature was 26.3 degrees C, 34.4 degrees C, 34.9 degrees C, and 36.4 degrees C during bypass, rewarming, admission to the intensive care unit, and after 4 hours, respectively. The temperature curve was sigmoid rather than linear. 2. During rewarming, the most common abnormality of PaCO2 on anesthetic mechanical ventilation was acute respiratory acidosis(PaCO2 45mmHg, pH 7.35) which occurred in 48% of the patients. After bypass and in the intensive care unit, respiratory alkalosis occurred in 36% and 45% of the patients, respectively. 3. The serum sodium values decreased in 19% of the patients during bypass but not significantly. 4. The serum potassium value increased in 21% of the patients during bypass surgery, but not significantly, and after surgery it returned to normal limits. This suggests that ventilatory management in the early postoperative period after hypothermic cardiopulmonary bypass surgery should be carefully monitered and adjusted as necessary to the increased metabolic rate during rapid rewarming.

8.The Relationship between the Age of Onset and Concentrations of Plasma Homovanillic Acid and 3-Methoxy-4-Hydroxyphenylglycol before and after the Neuroleptic Treatment in Patients with Schizophrenia.

OBJECTIVES: The authors investigated the possibility of homovanillic acid (HVA) and 3-methoxy-4-hydroxyphenylglycol (MHPG) concentrations in plasma to be biological markers before and after the pharmacological treatment of schizophrenia. METHODS: Twenty-six patients with schizophrenia were enrolled after two week washout of neuroleptics. Baseline sampling was done after washout. Consequent samplings were done at two and four week time-points after neuroleptic treatment. The concentrations of HVA and MHPG were analysed with clinical variables, such as age, age of onset, duration of illness, period of hospitalization, and changes of clinical state. The HVA and MHPG were assayed using high pressure liquid chromatographyelectrochemical detection method. RESULTS: A significant association was observed between the age of onset and plasma HVA concentration in washout state of antipsychotics. The earlier onset group had lower plasma HVA concentration than the late onset group. A significant association was observed between the age of onset and plasma MHPG concentration in washout state of antipsychotics. The earlier onset group had lower plasma MHPG concentration than the late onset group. CONCLUSIONS: The present findings suggest that the activities of dopamine and norepinephrine are different with respect to age of onset in the neuroleptic-naive schizophrenia. Plasma HVA and MHPG concentration can be biological markers for the subgrouping of schizophrenia.
