1.Inhibin B levels of serum and seminal plasma in fertile and infertile males.

OBJECTIVESTo investigate the possible differences in the inhibin B levels of seminal plasma and serum between fertile and infertile males and to obtain information on the relation between serum inhibin B or seminal plasma inhibin B and spermatogenesis.
METHODSSemen and blood samples were collected from fertile(n = 20), oligospermia(n = 20), asthenospermia(n = 22) and non-obstructive azoospermia(NOA) (n = 20) males at 8:00 am = 10:00 am. Semen parameters were analyzed. Levels of inhibin B in seminal plasma and serum, ACP, Fru, alpha-Glu in seminal plasma, serum levels of FSH, T, LH were determined.
RESULTSBoth levels of serum inhibin B and levels of seminal plasma inhibin B correlated significantly negatively with serum FSH(r = -0.536, P < 0.001 vs r = -0.288, P = 0.01), and statistically positively with sperm concentration(r = 0.49, P < 0.001 vs r = 0.48, P < 0.001). There was positive correlation between levels of seminal plasma inhibin B and activity of alpha-Glu in seminal plasma (r = 0.377, P = 0.001). The difference in levels of seminal plasma inhibin B was found only between fertile males or asthenospermia and NOA (P < 0.01 and P < 0.05, respectively). However, significant differences in levels of serum inhibin B were found not only between males with normal sperm concentration (including fertile males and asthenospermia) and NOA (P < 0.01), fertile males and oligospermia (P < 0.05), but also between oligospermia and NOA (P < 0.05). There was no correlation between serum inhibin B and seminal plasma inhibin B.
CONCLUSIONSBoth levels of serum inhibin B and seminal plasma inhibin B could reflect testis spermatogenesis status. Levels of seminal plasma inhibin B could also reflect the function of seminiferous duct, but the wide range of values limited its applicability.

2.Improved experimental varicocele increases follicle-stimulating hormone and reduces inhibin B in rats.

OBJECTIVETo evaluate the effects of improved experimental left varicocele (ELV) on follicle-stimulating hormone (FSH) and inhibin B (InhB).
METHODSELV models were established by the improved method in 30 SD rats, and another 30 were included in a sham operation group as controls. Three months after the operation, the concentrations of the FSH and InhB were assayed by ELISA.
RESULTSThe concentration of FSH in the serum was significantly higher in the experimental group ([37.56 +/- 9.72] ng/ml) than in the control ([26.69 +/- 5.33] ng/ml) (P < 0.05), while that of InhB significantly lower in the former ([349.93 +/- 99.48] pg/ml) than in the latter ([768. 83 +/- 146.96] pg/ml) (P < 0.05).
CONCLUSIONImproved ELV can increase FSH and reduce InhB in rats, which may be associated with subfertility.

5.Serum inhibin B and varicocele: An update.

Inhibin B, a glycoprotein produced predominantly by Sertoli cells and preferentially suppressing the production and secretion of follicle-stimulating hormone (FSH) in the pituitary, is closely related to spermatogenesis. Varicocele is the abnormal dilatation and tortuosity of the pampiniform plexus veins, which may contribute to spermatogenic dysfunction and male infertility. More and more evidence has shown that the level of serum inhibin B is negatively correlated with the severity of varicocele. Determination of the inhibin B level may help assess the severity of spermatogenic dysfunction of the patient and predict the outcomes of varicocele repair and therefore has a potential application value in the diagnosis and treatment of varicocele.

Inhibin, which is important for normal gonadal function, acts on the pituitary gonadotropins to suppress folliclestimulating hormone (FSH) secretion. The level and cellular localization of the inhibin isotypes, alpha, beta(A) and beta(B), in the testis of mice were examined during postnatal development in order to determine if inhibin expression is related to testicular maturation. Mouse testes were sampled on postnatal days (PNDs) 1, 3, 6, 18, 48 and 120, and analyzed by Western blotting and immunofluorescence. Western blot analysis showed very low levels of inhibin alpha, beta(A) and beta(B) expression in the testes at days 1 to 6 after birth. The levels then increased gradually from PND 18 to 48-120, and there were significant peaks at PND 48. Inhibin alpha, beta(A) and beta(B) were detected in testicular cells during postnatal development using immunohistochemistry. The immunoreactivity of inhibin alpha was rarely observed in testicular cells during PND 1 to 6, or in the cytoplasmic process of Sertoli cells surrounding the germ cells and interstitial cells during PND 18 to 120. Inhibin beta(A) and beta(B) immunoreactivity was rarely observed in the testis from PND 1 to 6. On the other hand, it was observed in some spermatogonial cells, as well as in the interstitial space between PND 48 and PND 120. We conclude that the expression of inhibin isotypes increases progressively in the testis of mice with increasing postnatal age, suggesting that inhibin is associated with a negative feedback signal for FSH in testicular maturation.

Premature ovarian failure (POF) is menopause before the age of 40 years. The frequency of POF is about 1% of all women. Recently inhibin alpha gene (INHalpha) has been indicated as candidate in POF pathogenesis. Inhibin, a glycoprotein, is a gonadal hormone, which can inhibit the synthesis and secretion of pituitary follicle-stimulating hormone (FSH), which has an important role in the recruitment and development of ovarian follicles during the folliculogenesis. G769A variation of INH alpha, alanine, is highly conserved across species, and has an important role of its receptor binding. We screened a G769A transition in the INHalpha from the total population of the patients of 84 women with POF and 100 normal fertile women. We found no variation between the normal subjects and the POF patients. G769A variation of INHalpha is rare in Korea women with POF.

8.Impact of varicocele on semen quality and inhibin B concentration in serum and seminal plasma.

OBJECTIVETo investigate the influence of varicocele (VC) on semen parameters and the concentration of inhibin B in the serum and seminal plasma of VC men.
METHODSWe collected semen and peripheral blood samples from 95 infertile VC patients and 55 normal fertile men. We performed semen routine examination by computer-assisted semen analysis and sperm morphology examination by modified Papanicolaou staining, and measured the levels of inhibin B in the peripheral blood and seminal plasma of the subjects by ELISA.
RESULTSCompared with the normal fertile controls, the infertile men with grade-I, -II and -III VC showed significantly lower percentages of morphologically normal sperm ([7.5 +/- 5.2]% vs [6.3 +/- 6.5]%, [2.6 +/- 3.0]% and [1.0 +/- 0.7]%, P < 0.05) and progressively motile sperm ([43.9 +/- 22.7]% vs [33.3 +/- 20.8]%, [28.9 +/- 19.8]% and [13.5 +/- 8.4]%, P < 0.05). The majority of the morphologically abnormal sperm were of the type of head deformity. The concentrations of inhibin B in the peripheral blood and seminal plasma were evidently lower in the infertile men with grade-I VC ([160.9 +/- 48.9] pg/ml and [208.3 +/- 28.1] pg/ml), grade-II VC ([150.6 +/- 44.7] pg/ml and [201.5 +/- 83.5] pg/ml), and grade-III VC ([132.6 +/- 41.5] pg/ml and [150.2 +/- 51.6] pg/ml) in comparison with those of the fertile control group ([201.0 +/- 38.1] pg/ml and [225.3 +/- 82.5] pg/ml).
CONCLUSIONVaricocele reduces sperm motility, increases sperm abnormality, decreases the concentration of inhibin B in the serum and seminal plasma, and consequently damages male fertility.

9.Concentration of seminal plasma and serum inhibin B: a predictor in the diagnosis of azoospermia.

OBJECTIVETo evaluate the concentrations of seminal plasma and serum inhibin B in the differential diagnosis of obstructive and non-obstructive azoospermia.
METHODSWe included 37 infertile men with obstructive azoospermia, another 33 with non-obstructive azoospermia and 25 normal fertile men as controls, and measured the concentrations of their FSH serum, seminal plasma and serum inhibin B, using Testicular Histology Johnson Score for the azoospermia infertile men.
RESULTSThe concentration ratio of seminal plasma to serum inhibin B was 2.17 in the control and 3.63 in the non-obstructive azoospermia group, with no significant difference (P = 0.29) in between, but obviously lower in the obstructive azoospermia group (0.18), significantly different from the above two (P < 0.01).
CONCLUSIONThe concentration ratio of seminal plasma to serum inhibin B can be used as a predictor in the diagnosis of obstructive and non-obstructive azoospermia.
