Renal angiography provides the ultimate in detailed vascularization of the renal vessels and vascular architecture of kidney. It is used in diagnosis and treatment of renal diseases e.g. renal injury, tumor, cyst and inflammation. A clinical observation of renal angiography was made on 49 cases of renal diseases which had been admitted to the Department of Urology, National Medical Center from 1975 to 1979. The diagnostic accuracy of renal angiography was 87.8%.

Vasculitis, characterized by inflammation of vessel walls, is comprised of heterogeneous clinicopathological entities, and thus poses a diagnostic challenge. The most widely used approach for classifying vasculitides is based on the International Chapel Hill Consensus Conference (CHCC) nomenclature system. Based on the recently revised CHCC 2012, we propose computed tomography (CT) features of vasculitides and a differential diagnosis based on location and morphological characteristics. Finally, vasculitis mimics should be differentiated, because erroneous application of immunosuppressive drugs on vasculitis mimics may be ineffective, even deteriorating. This article presents the utility of CT in the diagnosis and differential diagnosis of vasculitides.

Endophthalmitis is a rare but severe form of ocular inflammation due to infection of the intraocular cavity that can lead to irreversible visual loss if not treated properly and timely. It can be classified as exogenous or endogenous based on the transmission route of the infectious source. Exogenous endophthalmitis occurs when infecting organisms gain entry into the eye via direct inoculation, while endogenous endophthalmitis occurs when infectious agents hematogenously spread into the eye from a distant focus of infection. The diagnosis of endophthalmitis depends mostly on the clinical findings on ophthalmological examination. Delayed diagnosis of endogenous endophthalmitis can lead to not only visual loss, but also increased risk of mortality. Since ocular and systemic symptoms of endophthalmitis are usually non-specific, early diagnosis relies on the alertness of clinicians. Early diagnosis and proper treatment are keys to saving the eye. Following advances in vitreoretinal pharmacotherapy and surgical technology, early surgical intervention is the current trend in the management of endophthalmitis.

The infected endocarditis related permanent pacemaker occurs rare and most of them occur at generator pocket but endocarditis related permanant pacemaker lead itself occurs very rarely. The rate of infection after pacemaker implantation is reported as 0.13-7% or 0.13-19.9% and mortality rate is up to 24-33%. Focal inflammation of generator pocket is easily detected but it is difficult to diagnose endocarditis related pacemaker lead and it has poor prognosis. Especially, early diagnosis is most important because endocarditis related pacemaker is fatal. Thirteen years ago, a womon was inserted the permanent pacemaker and then only generator was removed after one month. We report a case that we had removed the pacemaker lead by open thoracostomy and cardiopulmonary circulation to treat endocarditis related pacemaker lead.

For allergists, itch is the most important symptom of atopic dermatitis (AD), and at the same time, the most difficult-to-manage complaint of atopic patient. Rather than just one symptom of AD, itch has more profound clinical significance in the following sense: itch, as a key component of so-called 'itch-scratch' vicious-cycle, plays a central role in the development and persistence of AD, thus, treating itch is not just alleviating one symptom of AD, but stopping genesis and progress of AD. In other words, itch is AD in itself. Pathomechanism underlying development of itch in AD could be summarized as follows: First, exogenous triggering factors penetrate impaired barrier, leading to allergic skin inflammation. Resulting characteristic itch of AD is amplified through the 'itch --> scratch --> inflammation --> (again) --> itch -->...' vicious-cycle. Thus, pathomechanism-based management strategy could be provided accordingly-avoidance of triggering factor, repair of impaired barrier, control of skin inflammation, and decreasing desire for itch/scratch. Among them, controlling inflammation is thought to be most effective measure based on the clinical experiences, for skin inflammation, as one of key components in 'itch-scratch-inflammation' vicious-cycle, at the same time, is a crucial connecting link between OUTSIDE and INSIDE. However, individualized approach should be taken for patient per se. Finally, following topics may be future topics for doctors always hungry for new weapon against AD and patients suffering from this out-of-place (a-top-y) disease: central itch pathway-emerging new hero; neural sensitization - still in the Dark-Ages in terms of diagnosis and management; and scratch-just in its infancy.

An esophageal injury after an anterior cervical discectomy and fusion with plating is a well known but a fatal complication. If the diagnosis is delayed, inflammation can spread out into the neighboring main organs resulting in a very poor prognosis despite vigorous treatment. We report a case of esophageal fistula formation as a result of infected metal loosening after an anterior cervical discectomy and fusion with plating, which was treated successfully with a satisfactory clinical outcome.

A mass formed around a cotton matrix left within the body is termed as gossypiboma. It is a rare complication of surgery and often occurs mainly after abdominal operations. The incidence of gossypiboma is underreported, due to legal implications of their detection. Furthermore, a correct diagnosis can be made only in one-third of cases. Clinical presentation is variable, depending on the location of the foreign body and on the type of inflammatory reaction. The recommended choice of treatment is excision, with the most important consideration given to prevention. Xanthogranulomatous inflammation is an uncommon form of chronic inflammation, which is destructive to affected organs. To the best of our knowledge, we report the first case of gossypiboma associated with xanthogranulomatous inflammation. A gauze accidently left in the lateral neck after a neuro-surgical intervention was found during resection of mass-like lesion and xanthogranulomatous inflammation was diagnosed on histopathologic exam.

Endoscopy plays a crucial role in the management of inflammatory bowel disease (IBD) in terms of diagnosis, monitoring of mucosal status, and surveillance of colitis-associated neoplasia. Mucosal healing evaluated by endoscopy has been recognized as the target of treatment in the era of powerful biologics therapy. The optimal modality for identifying dysplasia in IBD has yet to be well defined. Increasing progress has recently been made in endoscopic technologies to more accurately assess mucosal inflammation and more effectively detect dysplasia. Here we review the data of advanced endoscopic imaging techniques such as chromoendoscopy, virtual chromoendoscopy, endocytoscopy, and confocal laser endomicroscopy in the management of IBD.
