Renal scarring can cause hypertension, proteinuria, chronic kidney disease, and even end stage renal disease. To understand the risk factors of renal scarring in children is helpful for its early detection, diagnosis, prevention, and treatment. High grade vesicoureteric reflux, recurrent urinary tract infection, and delayed treatment are risk factors for kidney scarring. However, there are still some controversies about the relationship between renal scarring and the some factors such as gender, age, and congenital factors. Recent studies have found that noninvasive urine indicators such as urine neutrophil gelatinase associated lipid delivery protein, urinary endothelin-1, and risk prediction models can predict the formation of renal scar. However, prophylactic use of antibiotics can reduce occurance of urinary tract infections, but do not reduce the risk of renal scarring. This article reviews the possible risk factors and predictors of renal scarring in children, providing a basis for early detection, timely intervention, and effective prevention of it in clinic, so as to reduce the formation and progression of renal scarring.

Objective To observe the short-term fusion rate of using osteophyte filled cage in ante-rior cervical disectomy and fusion. Methods From May 2007 to May 2009, 103 patients receiving anterior cervical disectomy, spine fusion by different surgeons were performed with cages containing exclusively au-tologous osteophyte collected from both anterior and posterior vertebral edges adjacent to the intervertebral space. There are 69 males, 34 females, from 25 to 77 years of age, average 53.6 years. 60 patients received one level fusion. 33 and 10 patients received two-level and three-level fusion respectively. Patients were re-viewed 3 months after operation and fusion was evaluated by X-ray film, reconstructive CT and MR. Results 1. Only 1.9% (2/103) of patients' X-ray films indicate non-fusion (internal fixation failure) while 31.7% (32/ 103) of patients' CT and 30.1% (31/103) of patients' MR indicate non-fusion or being confusing to make a diagnosis. 2. The diagnostic coherence between CT and MR as to evaluate fusion is good (kappa=0.686, P= 0.00). 3. The total fusion rate was 77.7%(80/103) while the highest fusion rate achieved by single surgeon was 90%(18/20). The total one level fusion rate was 80%(48/60) while the highest rate by single surgeon was 88.2%(15/17). The total two-level fusion rate was 78.6%(26/33) while the highest rate by single sur-geon was 90%(9/10). The total three-level fusion rate was 60%(60/100) while the highest rate by single surgeon was 100%(3/3). No statistic significant difference in fusion rate was found between any two of the three groups. Conclusion The method of using osteophyte filled cage can acquire ideal short-term fusion rate in one and two-level anterior cervical disectomy, fusion and plate fixation.

ObjectiveTo study the clinical effect of sodium aescinate on the edema of limb tibial plate fracture.Methods69 patients with edema of limb of tibial plate fracture were randomly divided into two groups,treatment group(46 cases)and control group(23 cases).Both of them were transfused with 20％ mannitol(250ml,Bid ×7days).In addition,these patients in the treatment group were treated with sodium aescinate(30mg Qd × 7days).ResultsThe repercussive effect of the edema grade in the treatment group were obviously better than those in the control group(P ＜0.05 ).ConclusionSodium aescinate had a remarkable repercussive effect on the limb edema of tibial plate fracture.

Methods:The determination was performed on a Ultimate? AQ-C18 column(250 mm × 4. 6 mm, 5 μm,Welch Inc. ) with the detection wavelength at 220 nm. The mobile phase was composed of methanol and 0. 07 mol·L-1 sodium heptanesulfonate solution (14 ml triethylamine diluted with water to1 000ml, adjusting pH to 3. 5 with diluted orthophosphoric acid) with gradient elution, and the flow rate was 1. 0 ml·min-1. Results: Methionine, vitamin B1 and vitamin B2 was in good linearity within the range of 0.527 0-1.510 0 mg·ml-1 - (r =0.999 9), 0.045 52-0.136 56 mg·ml-1 (r =0.999 9) and 0.010 10-0. 060 58 mg·ml-1(r=0. 999 9), respectively. The mean recovery was 100. 5%, 97. 7% and 101. 5% with RSD of 0. 5%, 1. 0%and 1. 4%(n=9) accordingly. Conclusion:The method is simple, accurate, reliable and appropriate in the simultaneous determina-tion of methionine,vitamin B1 and vitamin B2 in Lingzhi Erwei methionine capsules.

BacKground:Lymphocyte function-associated antigen-1( LFA-1 ) pIays a cruciaI roIe in the pathogenesis of infIammatory boweI disease by reguIating the activation and function of T ceIIs. Aims:To investigate the effect of LFA-1 deficiency( LFA-1-/-)on differentiation of mice na?ve T ceIIs into Th17 ceIIs in vitro. Methods:LFA-1-/- mice were breeded and the progeny genome DNA was extracted from the taiIs for genotyping by PCR. CD4+CD62L+ na?ve T ceIIs were separated from spIenic mononucIear ceIIs of LFA-1-/- progeny mice and wiId type ( WT ) C57BL/6J mice, respectiveIy,by magnetic-activated ceII sorting( MACS),and then the purity of separated ceIIs was determined. Na?ve T ceIIs obtained were cuItured in different inducing systems[ transforming growth factor-β( TGF-β),TGF-β + interIeukin-6 (IL-6),and TGF-β + IL-6 + IL-23]in vitro for Th17 ceII differentiation;ceIIs in each inducing system were coIIected for anaIyzing the ratio of Th17 ceIIs by fIow cytometry,and the expressions of Th17-specific transcription factor ROR-γt and Th17-specific marker IL-17A were measured by qRT-PCR and ELISA methods. Results:AII fifteen progeny mice were identified as LFA-1-/- genotype. Purity of CD4+CD62L+ na?ve T ceIIs separated by MACS was above 95%. Th17 ceIIs couId be induced by Iow-dose TGF-βcombined with IL-6,and the differentiation ratio was increased obviousIy when IL-23 was added. In inducing system containing TGF-β,IL-6 and IL-23,na?ve T ceIIs from LFA-1-/- mice produced more Th17 ceIIs than those from WT mice(17. 2% ± 1. 4% vs. 5. 7% ± 0. 2%,P<0. 001),expressions of ROR-γt mRNA and IL-17A mRNA were up-reguIated(P<0. 001),and IL-17A concentration in ceII cuIture supernatant was increased(P<0. 01). Conclusions:Deficiency of LFA-1 promotes the differentiation of mice na?ve T ceIIs into Th17 ceIIs in vitro.
