2.Cytohistopathologic comparative study of aspiration biopsy cytology from various sites.

A statistical analysis of the diagnostic value for 244 aspiration biopsy cytology(ABC) among a total 1,043 cases from various sites was performed. ABC, using diagnostic terminology similar to that of a surgical pathology reports, was compared to the final tissue diagnosis. For the entire series, a sensitivity of 91.8%, a specificity of 99.3%, a positive predictive value of 98.9%, a negative predictive value of 94.8%, and an efficacy of the test of 96.3% were shown. There were 8 false negative and 1 false positive diagnosis. The diagnostic accuracy was 89.8%. Those results indicate that the ABC is a considerably highly accurate procedure that should be routinely employed.

4.Production of IL-15 and lts Functional Study in Mouse Splenocyte Activation.

After the synthesis of IL-15 cDNA from the total RNA of mouse spleen, it was inserted into the prokaryotic expression vector, pRseta, and eukaryotic expression vector, pcDNA3.0, respectively. Subsequently, the insertion of gene and open reading frame were confirmed by sequencing of each plasmid, respectively. Using pRseta- IL-15 plasmid, the recombinant IL-15 protein was induced by IPTG under BL21 (DE 3) host cells and recombinant IL-15 was expressed at 14.5 KDa with time. Then, IL- 15 was separated by His-tag affinity chromatography and analyzed by SDS-PAGE to yield soluble IL-15 at 14.5 KDa as monomer and 29.0 KDa as dimer. In order to inspect the function and contribution of IL-15, the in vitro experiment was established using mononuclear cells separated from the mouse spleen. After 48h exposure of PHA to mouse splenocyte and 24h treatment with recombinant IL-15, the effects of cytokine inductions inspected against IL-2, IL-6, IL-10, IL-12, IFN-r, and GM-CSF. The results showed that comparing with the control, IL-6 increased, IL-2, IL-12 and IFN-r increased and similar, and GM-CSF decreased. In addition, the direct injection of pcDNA3.0-IL-15 plasmid into mice gave the similar results to in vitro studies. Namely, IL-6 and IL-12 increased, and IL-2, IFN-r and GM-CSF were similar or decreased. IL-10 was not induced in in vitro and in vivo experiments. These results suggested that the IL-15 induce the splenocyte activation and can be an important factor in proliferation and fuction recovery of weakened T-cell.

6.A Study of Stress, Stress Coping Style and Health Symptoms in 6th Grade Elementary School Children.

The purpose of this study was to examine the stressors and stress coping styles of 6th grade elementary school children and to explore the relationship between stressful life events and health symptoms and the effects of coping style which is theoretically considered to mediate the relationship between stress and health symptoms. The study subjects consisted of 329 in 6th grade elementary school children in Cheong-Ju city. Of the 329 subjects, 171 were boys and 158 were girls. For this study, three kinds of questionnaires were adopted as follows ; 1) Feel Bad Scale (FBS) by Lewis et al., 2) Lazarus-Folkman's Way of coping questionnaire 3) Hee Sun Shin's Health Symptom questionnaire(HSQ) The researcher visited the school and collected data in the class using the questionnaire method after an explanation of the purpose and procedures was given to the children. Data collection was done for 10 days(from 5th to 15th of July 1997). The data were analyzed by descriptive statistics, correlation analysis, multiple regression analysis using the SAS statistical program. The results of this study were as follows; 1. The mean score for the FBS was 211.37(range : 77-427). The most severe stressors perceived by children were parental divorce and other's smash or steal of my things. The most frequently experienced stressful life events were conflict with siblings and being scolded for other's fault. 2. The most frequently used stress coping style was the active coping (M=17.85), followed by passive(M=13.64) and magical one(M=13.42). 3. The mean score for the HSQ was 23.30(range : 0-72) The most frequently complained health symptoms were headache and having much worry about everything. 4. The was a significant relationship between stressful life events and health symptoms(r=.60, p<.001). Also, stressful life events were positively related with passive coping(r=.27, p<.001) and magical coping(r=.38, p<.001). Health symptoms were positively correlated with passive coping(r=.33, p<.001) and magical coping(r=.41, p<.001). 5. Stepwise multiple regression analysis revealed that the most powerful predictor was the variable of stressful life events. Health concerns, magical coping style, passive coping style and active coping style accounted for 49.15% of the variance in health symptoms. This study revealed that stressful life events correlated with health symptoms in 6th grade elementary school children and passive coping and magical coping had mediating effects on this relationship. The implication for nursing is that there is a need to develop supportive interventions for high risk population to decrease health problems due to stress.

8.Effect of Sharps Injury Prevention Program on the Incidence and Reporting of Sharp Injury among Nurses.

BACKGROUND: This study aimed to identify the effects of a multifaceted needlestick injury (NSI) prevention program on changes in knowledge about bloodborne infectious diseases and postexposure coping, attitudes toward postexposure reporting, preventive measures, the number of NSIs, and postexposure reporting pre- and post-intervention among nurses. METHODS: A total of 429 and 420 nurses participated in the pre- and post-intervention periods, respectively. The intervention was performed from April to September 2007, comprising NSI guideline education, the use of containers with enhanced engineering, and the supply of safety devices. RESULTS: The average score of knowledge about bloodborne infectious diseases increased significantly from 8.3 to 8.9 out of 14 points (P<0.001), but the change in score of knowledge about postexposure coping was insignificant. The average score of attitude toward postexposure reporting increased significantly from 8.9 to 9.6 out of 12 points (P<0.001). Preventive measures such as "gloves are provided whenever needed" (P<0.001), "use one-hand technique" (P<0.001), and "needle containers are provided whenever needed" (P=0.031) increased significantly. The number of NSIs decreased by 40.4%, and the postexposure reporting rate increased by 552.8%. CONCLUSION: The multifaceted NSI prevention program positively affected knowledge about infectious diseases and postexposure coping, attitudes toward postexposure reporting, preventive measures, the number of NSIs, and postexposure reporting after intervention. Therefore, we recommend that this program be applied to various healthcare workers in hospitals.

10.The Effect of 30degree laterally Inclined Position Change on the Pressure Sores of Patients in Intensive Care Unit.

The purpose of this study was to test the effects of position change with 30degree laterally inclined position on pressure sores prevention. This study was designed as a Nonequivalent Control Group Quasi-experiment study. The subjects were collected with convenience sample of 30 patients who were hospitalized at ICU(Intensive Care Unit) of C university hospital in Seoul from March 1 to October 31, 1997. The patients were not able to change of position without help. The 15 patients were assigned to the control group and the other 15 patients to the experimental group. The both group were done position change every 2 hours and were observed whether pressure sores was developed for 2 weeks. For the experimental group, two positioning methods were alternatively used : 30degree right lateral, 30degree left lateral. For the control group, 90degree right and left lateral position were applied instead of 30degree lateral position. New Pressure Risk Assessment Scale was utilized to assess pressure sore risk. It is consists of 8 subscales which reflect sensory perception, skin moisture, activity, mobility, friction and shear, nutritional status, body temperature, and amount of medications(analgesics and sedatives). The results of this study are as follows ; 1. The incidence of pressure sores in total sample was 13 cases(43.3%) : 10 cases(76.9%) in control group, 3 cases(23.1%) in experimental group. the incidence rate of pressure sores in experimental group was significant lower than control group. 2. The sites of pressure scores development were 5 cases in trochanteric region, 4 cases in hip region, 2 cases in flank region, 1 cases in sacral region and 1 case in occipital region. Trochanteric region sores were not developed in experimental group. 3. The mean hospitalized period before pressure sores development were 7.3 day in experimental group and 4.1 day in control group. According to the results from this study, suggestions are as follows ; 1. Development of device which enable to keep 30degree laterally inclined position for 2 hours is needed. 2. It is necessary to study variously and objectively usefulness of 30degree laterally inclined position change which is applied to the other pressure risk assessment scale.
