This study was conducted to identify physical, psychological and environmental factors affecting elderly falls and to identify fall frequencies according to each dwelling places and types and to provide basic data for developing preventive strategies following fall accident of the elderly. The research design was a reprospective survey study which analyzed fall experiences during the past year. Data was collected from Nov, 1997 to Mar, 1998 for four months by personal interview. The research subjects were 475 people over 65 years old, who dwelled in their homes, two free nursing homes, one elderly sanatorium town and five general hospitals. Research results were as follow ; The mean age of the subjects was 73.1 years. Of the subjects, 69.9% was female, 64% was separate or bereaved, 55.8% had no education, 34.7% had no religion, 43.1% was extreme poverty. 73.1% of the elderly dwelled in their homes and 20% of the subjects were living at fishing and agraian villages. Mean number of diseases per subject was 2.63 of which musculo-skeletal problem was most common. Elderlies who experienced falls were 48.2%, 55.9% of them had fallen once a year. The most frequent fall occurrence time was from noon to 3 pm, the season was winter and the fall places were outdoors. Highest fall cause was sliding. 60.6% of the injuries were minor, 14.2% were serious( fracture, dislocation, amputation or head injuries ), which were treated by hospitalization or surgery. Especially most house facilities had potential risk factors of the fall. There were no handrail, slippy surface and narrow space of the bathrooms, high door sill and dangerous stairs etc. There were significant differences on the presence of falls in the elderly according to sex, education level, number of diseases, use of brace and living together with children. There were significant differences on the presence of the falls in the elderly according to toilet type, presence of threshold and stairs, surface of bathroom, depression level, cognitive function and ADL ability. Variables which affected ADL ability of the elderly were age, house type, self-rating health status and gait ability. Variables which affected depression of the elderly were age, religion, education level, marital status, living together with children, self-rating health status and number of diseases. Variables which affected cognitive function of the elderly were religion, age and house type. In conclusion, this study showed various physical, psychological and environmental fall risk factors of the elderly. So there is a need to development suitable intervention programs to reduce risk factors, to make elderly life sage and to increase the quality of their life.

The purpose of this study was to survey basic data for management of elderly day care centers. In this study. occupation. leisure life. and food. clothing and habituation. as well as social relationship. health state and behavior. and general characteristics of subjects were examined to identify the factors affecting their cognition. need and preference of the day care centers. The subjects of this study were 392 elderly. aged 60 years or over. living in Gangnung and neighboring districts. Data were collected by educated interviewers from November 4 through Novemver 22, 2002. The subjects were interviewed face to face. one for one after the interviewee's agreements on the survey. The collected data were analysed with logistic regression analysis by SAS (statistical analysis system). Logistic regression analysis was done to identify affecting factors for cognition. need and preference of the elderly day care centers. The major findings are as follows: 1. The factors affecting cognition for care centers were analysed. The the day subjects with an occupation and a lower satisfaction level of living environment and friendship. who were economically secure enough to manage a sudden accident. showed a higher level of cognition of the day care centers. The subjects with a higher level of mental health state and a lower level of IADL also showed a higher level of cognition. On the general characteristics younger female subjects showed a higher level of cognition. 2. The subjects with a lower level of perceived economic condition who did not own their housing and were not economically safe enough to manage a sudden accident. had showed a higher level of need for the day care centers. It showed that the subjects with a high level of mental health state. a bad eye sight and dental condition. a good perceived health condition. and a lower level of IADL. needed the centers. 3. The subjects who had an occupation, however, not capable of making their own daily expenses. and a low occupation satisfaction level. and who did not own their housing. and were economically poor not enough to manage a sudden accident. showed a higher preference for the day care centers. The subjects with higher levels of friendship satisfaction and perceived health condition. not living with their spouse. and a higher education level, showed a higher preference for the centers. In conclusion. the common factors affecting their cognition. need and preference of the day care centers were occupation, economic security enough to manage accidents. and friendship satisfaction level. Especially, the subjects who had an occupation, however. not economically secure enough to manage accidents. and who did not live in their own housing with a good perceived health condition, showed high levels of need and preference for the day care centers. These results can be used as basic data to develop the efficient elderly day care centers, thus contribute to the elderly welfare in a local community.

The objective of this study was to apply a dementia prevention program to the subjects who were suspected of dementia and test its effect. This study was one group pretest-posttest design. The dementia prevention program was applied for 20 weeks to the 19 subjects who were suspected of dementia after a screen test among 638 subjects enrolled in 10 senior citizen's centers in G city. The scores of measured before and after the program. The data collected were analyzed using a SPSS (statistical analysis system) program, and frequencies, averages and standard deviations were obtained. The differences of the scores of the pretest and posttest were analyzed with Mann-Whitney test and sign test. The results were as follows: 1. After the program, the average score of self-efficacy was increased from 75.31 (standard deviation 11.99) to 84.26 (standard deviation 13.92). 2. There are no differences between the average scores of physical and psychological quality of life, however, the average score of social quality of life was slightly increased from 2.25 (standard deviation 0.40) to 2.53 (standard deviation 0.43), and the overall score of social quality of life was low. 3. The average score of cognition was significantly increased from 21.00 (standard deviation 2.60) to 24.58 (standard deviation 3.37), thus, it was found that the program was effective to improve the cognition level. 4. The score differences of self-efficacy, quality of life, cognition between the pretest and posttest were statistically significant. 5. The score differences of quality of life between the pretest and posttest were found to be dependent on marital status, exercise and regular chechup yes or no. In conclusion, the program was effective to prevent the subjects suspected of dementia from dementia.

Idiopathic ureteropelvic junction stricture is not an uncommon disorder with resultant hydronephrosis of affecting kidney, and most frequently encountered in infant and children. Multiple causes of stricture have been identified. Abnormal organization of smooth muscle bundles at the ureteropelvic junction has been reported, and electron microscopy has demonstrated excessive collagen deposition within the intramural stroma. However, the exact cause of obstruction of UPJ stricture is uncertain. We reviewed 23 cases with idiopathic hydronephrosis underwent dismembered pyeloplasty. Examination of strictured segment with light the normal interwoven pattern, were arranged into an abundant outer circular and an inner longitudinal layer in 21 of 23 cases. But muscle thickness was not changed, and collagen was normal in both amount and arrangement. This results suggested that the abnormal muscle arrangement at the ureteropelvic junction might be the cause of the idiopathic stricture.

PURPOSE: We investigated the role of myocardial perfusion SPECT in prediction of ventricular dilatation and the role of revascularization including thrombolytic therapy and PTCA in prevention of ventricular dilatation after an acute myocardial infarction (AMI). MATERIALS AND METHODS: We performed dipyridamole stress, 4 hour redistribution, and 24 hour reinjection Tl-201 SPECT in 16 patients with AMI two to nine days after attack. Perfusion and wall motion abnormalities were quantified by perfusion index (PI) and wall motion index (WMI). Left ventricular ejection fraction (LVEF), WMI and ventricular volume were measured within 1 week of AMI and after average of 6 months. According to serial changes of left ventricular end-diastolic volume (LVEDV), patients were divided into two groups. We compared WMI, PI and LVEF between the two groups. Relationships among degree of volume, stress-rest PI, WMI, CKMB, Q wave, LVEF and revascularization were analysed using multivariate analysis. RESULTS: Only initial rest perfusion index was significantly different between the two groups (p<0.05). While initial LVEF, stress PI, CKMB, trial of revascularization procedure, presence of Q wave and WMI were not significantly different between the two groups. Eight of 16 patients (50%) showed LV dilatation on follow-up echocardiography. Three of 3 patients (100%) who did not undergo revascualrization procedure documented LV dilatation. And only 5 (38%) of the remaining 13 patients who underwent revascularization revealed LV dilatation. There was no difference in infarct location between the two groups. By multivariate linear regression analysis in patients only undergoing revascularization, rest perfusion index was the only significant factor. CONCLUSION: Myocardial perfusion SPECT performed prior to revascularization was useful in prediction of LV dilatation after an AMI. Rest perfusion index on myocardial perfusion plays as a significant predictor of left ventricular dilatation after AMI. And revascularization appears to be a valuable procedure in alleviating LV dilatation after AMI with or without viable myocardium in a limited number of patients studied retrospectively.

PURPOSE : The purpose of this study was to determine proper position and angulation of an implant for immediate implantation. MATERIALS AND METHOD : From the years 1997 to 2000, 52 Denta scanR views, 22 upper and 32 lower jaw with an average age of 43 and 40 respectively, were investigated, which comprise intact upper and lower 6 anterior teeth and premolars. on the Denta scanR, the optimal placement for the immediated implantation was simulated. The measuring methods included. 1) Angulation difference between tooth long axis and alveolar bone process. 2) Angulation difference of long axis between tooth and installing fixture 3) Distance between center of tooth at cervical area and center of fixture. 4) Distance from root apex to the bone limit of vital structure. One sample t-test was used for statistical analysis. RESULT : The results were as follows. 1) At the maxillary central incisor and lateral incisor, angulation difference of long axis between tooth and installing fixture was respectively 0.5 and 3.2degrees with the fixture center's palatally positioned 2mm apart from tooth center. 2) At the lower anterior 6 teeth, that was about -2.8degree to -4.6degree with the fixture center's lingually positioned 1mm apart from tooth center. 3) At the maxillary canine and premolar, that was respectively 11.8degree and 7.2degree with the fixture center palatally positioned 2~2.4mm apart from tooth center. 4) At the lower premolar area, that was about 0degree to 2degree with the fixture center's lingually positioned 0.5~1mm apart from tooth center. 5) Distance from root apex to the bone limit of vital structure, at the maxillary anterior and premolars, was the range of 10 to 12mm, and at the mandibular anterior teeth and the 1st premolar, that was the range of 18 to 20mm. CONCLUSION : The proper implant position of maxillary anterior and premolar teeth is as paralleled as or more buccally angulated than long axis of tooth with the fixture center's palatally positioned. in mandibular anterior region, long axis of implants is lingully angulated compared with compared with long axis of tooth and in premolar, almost parelleled with long axis of tooth and alveolar process.

The study was done by applying a phenomenological study, which is qualitative research methods, in order to understand the meaning of the lived experiences, to confirm and describe the meaning structure, and to prepare nursing interventive strategies centering around the meanings of the inpatients' families in the intensive care units. In the study, the family members were the main important nursing providers for in the inpatients' who were admitted in the neuro-surgical intensive care unit in K-university hospital and who agreed to participate in the study after being given on explanation about the purpose of the study. The data were collected from the seven participants who had feelings of trust and intimacy favorable toward the researcher as they were families of patients who had been cared for by the researcher in the ICU where the researcher has been assigned. The data were collected from April to October, 1999. The participants described their experiences as candidly as possible. The researcher described closely the lived experiences with their own words and the observations of the researcher. A tape recorder was used with the consent of the participants to prevent nursing information and communication. The analysis of the data was made through the phenomenological analytic method suggested by Giorgi; as an unit of description, which include the participants'expressions and the researcher's observations, the analysis was used based on the data described from the expressions of the participants and the details of observations of the researcher. The conclusions of the study were as follows: The meanings of the lived experience of the inpatients'families in the ICU was confirmed by indepth interviews and observations including these of the participatants: 1. Psychological impact: confusion, impatience, surprise, insensibility; 2. Physical suffering: fatigue, discomfort, indigestion; 3. Psychological suffering: heartbreaking emotion, anxiety, annoyance, fear, compassion, grief; 4. Economical suffering: economical difficulties; 5. Psychological disagreement: escape from reality, personnel avoidance, grudge, powerlessness, carefulness, transposition of life-tract, abandonment, role-crisis, hope, lack of understanding, regret, feeling of ambivalence(progressive process, medical personnel interest); 6. Psychological dependency; self-reliance groupsupport, family support, religious support; 7. Psychological acceptance; acquaintance, gratitude, reassurance; The study will offer better understanding of experiences therefore, based on the experiences confirmed by the study, it may facilitate more appropriate nursing interventive strategies for health maintenance and to prevent occurrence of possible problems with the inpatients'families in the ICUs.
