2.The Effect of Minizide(R) in the Hypertensive Patients.

The effect of Minizide(R) (prazosine 1mg and polythiazide 0.5mg) was studied in 34 hypertensive patients (female 16 cases, male 18 cases) at the Department of Internal Medicine, Paik Hospital, In-Je Medical College between July, 1979 and August 1980 for 14 months. The patients didn't take the any drugs for one week before the clinical trial with Minizide(R), and thereafter they took minizide 1 tablet 3 times daily for 2 weeks. The blood pressure was checked before and after the trial in the supine and standing position. The results were as followings: 1) The systolic blood pressure before the treatment with Minizide in the supine position was 196+/-24mmHg and in the standing position 188+/-22mmHg. The systolic blood pressure after the treatment with minizide in the supine position was 148+/-16mmHg and in the standing position 139+/-9mmHg. 2) The diastolic blood pressure before the treatment with minizide in the supine position was 118+/-11mmHg, and in the standing position 113+/-10mmHg. The diastolic blood pressure after the treatment with minizide in the supine position was 87+/-6mmHg and in the standing position 84+/-5mmHg. 3) There was no significant orthostatic hypotension after the 34 patients. 4) There was no changes of laboratory findings after the treatment. 5) 3 of 34 hypertensive patients after the treatment complained of headache, 3 palpitation, 2 edema, 2 drowsiness, and 1 fatigability and 1 patient complained of weakness.

3.A clinical observation on antiarrhythmic efficacy of propafenone for atrial fibrillation.

BACKGROUND: Propafenone is effective in the treatment of ventricular arrhythmias and paroxysmal supraventricular tachycardia. Propafenone has been shown to be also useful in preventing the recurrence of atrial fibrillation. But to date, date concerning the efficacy of propafenone for patients experiencing paroxysmal and chronic atrial fibrillation have not been examined in Korea. METHODS: In patients with paroxysmal atrial fibrillation propafenone was given at a dose of 450mg daily without dose titration and symptomatic recurrences were evaluated by patient interview during follow-up. In patients with chronic atrial fibrillation, who had no previously attempted cardioversion, propafenone was given at a dose of 450~675mg daily without dose titration, and after a minimum of 3 days pharmacological cardioversion was assessed. Those patients in whom sinus rhythm had been restored pharmacologically or by combined direct current cardioversion were followed with maintaining propafenone therapy also at a dose of 450~675mg daily. RESULTS: Of 7 patients with paroxysmal atrial fibrillation, 4 (57%) patients had marked symptomatic improvement and they were followed for 197 to 460 (mean 286) days Of 13 patients with chronic atrial fibrillation, 7 patients incuding 4 pharmacologically converted patients were initially controlled to sinus rhythm. In 5 (38%) patients maintenance of sinus rhythm was followed for 90 to 415 (mean 224) days. CONCLUSION: Although these observations were made in small number of patients and are so limited, the results are comparable to the previous studies which showed the potential role of propafenone in the management of atrial fibrillation.

4.Overexpression of the E1193-283 find E2384-649 Proteins of Hepatitis C Virus in GST Fusion Forms in E. coli and Their Immunogenicity.

The truncated E1192-283 and E2384-649 genes of hepatitis C virus (HCV) linked to the gene for glutathione 5-transferase (GST) were constructed and their expressions were analyzed. The GST-E1192-283 fusion gene overexpressed the fusion protein in E. coli as a soluble form, while the GST-E1192-383 plasmid did not express expected fusion protein. The purified GST-E1192-283 fusion protein was efficiently cleaved by thrombin. More than 90% pure, HCV E1192-283 protein was obtained by GST-agarose chromatography. The truncated GST-E2384-649 fusion gene expressed the fusion protein mainly as an insoluble form, whereas the GST-E2384-740 did not express the fusion protein. The truncated GST-E1 182-283 and GST-E2384-649 fusion proteins reacted specifically with an HCV patient serum. In addition, mice immunized with either the purified E1192-283 or GST-E2384-649 proteins generated specific antibodies to each antigen. The results suggested that hydrophobic carboxyl portions of the E1 and E2 proteins might affect expression levels as well as the solubility of each fusion protein in bacteria. Also, the truncated E1 protein with Tyr-192 to Ser-283 contained antigenic epitope(s) which could be specifically recognized by an HCV patient serum.

5.A Case of Duodenal Leiomyosarcoma.

Leiomyosarcoma of small intestine is an uncommon tumor, comprising less than 20 percent of all primary malignant tumors of small intestine. Duodenal leiomyosarcoma is rare disease which amount to about 20 percent of all small bowel malignancy but potentially curable tumors often diagnosed at an advanced age, so its five-year survival following resection approximates 50% in reported series. Recently, several cases of leiomyosarcomas of duodenum were diagnosed by hypotonic duodenograhy in Korea. We experienced a woman with duodenal leiomyosarcoma that was diagnosed by endoscopic duodenal biopsy and received currative resection of the tumor(Whipple's operation). We report the case with review of the literatures.

6.Studies on Sarles' phenomenon of the excysted larvae of Clonorchis sinensis.

The occurrence of Sarles' phenomenon (Cerkarien Hullen Reaktion) are proved in the excysted metacercariae of Clonorchis sinensis which were incubated in the sera from rabbits given a varying number of metacercariae of C. sinensis per orally. The precipitates were formed around the oral sucker and excretory bladder of the excysted larvae. Sarles' phenomenon began to be positive from the 2nd to 3rd week after infection. In the excysted larvae of C. sinensis precipitates were not produced in the sera of rabbits infected with Metagonimus yokogawai and Capillaria hepatica. The number of eggs in the feces (E.P.G.), intradermal reaction, the course of infection and Sarles' phenomenon was studied in 6 clonorchiasis patients. Sarles' phenomenon appeared in the sera of some clonorchiasis patients. However, it assumed that this phenomenon correlated with the degree and the course of infection of clonorchiasis.

7.Differential Diagnosis of Vascular Dementia and Alzheimer's Disease.

Differential diagnosis of Alzheimer's disease (AD) and vascular dementia(VaD) has an important bearing on the diagnosis and management of patients with dementia. This article provides a guideline for the differential diagnosis through 1) history taking, 2) neurological examination, 3) neuropsychological tests, and 4) neuroimaging studies. VaD consists of etiologically and clinically heterogeneous subtypes that include multi-infarct dementia (MID), single strategic infarct dementia, and subcortical vascular dementia. Patients with MID and single infarct dementia con be easily differentiated from patients with AD. However, clinical manifestations of subcortical vascular dementia can mimic those of AD, which may lead primary physicians to misdiagnose subcortical vascular dementia as AD. The issue of differential diagnosis is further complicated by the fact that many patients may have AD with concomitant VaD (mixed dementia).

10.Comparison of Epinephrine and Epinephrine with Vasopressin Mixture on the Cerebral "No-Reflow" in a Rabbit Model of Cardiopulmonary Resuscitation.

BACKGROUND: The more vital organ blood flow during cardiopulmonary resuscitation (CPR), the more successful outcome. The worldwide CPR drug of choice, epinephrine also has some limitations and is often challenged by another catecholamine drug. This study was designed to compare the effects of epinephrine with those of vasopressin and epinephrine mixture on cerebral no-reflow during closed-chest CPR in a rabbit model of ventricular fibrillation. DESIGN: Prospective, randomized, experimental study. SETTING: University research laboratory. SUBJECTS: Domestic rabbits,3 to 6 months of age InteNentions : Four rabbits were randomly allocated to receive only 0.020 mg/kg of epinephrine(group 1) and another four rabbits were received both 0.020 mg/kg of epinephrine and 0.8 U/kg of vasopressin after 5 mins of cardiac arrest(group 2). MEASUREMENTS AND MAIN RESULTS: Carotid arterial blood flow and arterial gas analysis were showed no statistical difference between two groups but in the points of cerebral no reflow area and fluorescence exposure time, there were significant differences(group 1,5.15+/-.45%, group 2, 6.38+/-.54%, p=.029/ group 1; 29.65+/- 17.09 seconds, group 2;17.98+/- 18.75 seconds, p=.014). CONCLUSIONS: In cardiac arrest there is some synergistic effect with epinephrine and vasopressin mixture on cerebral no-reflow phenomenon.
