1.Immunolocalization of Desmoglein in Autoimmune Pemphigus.

BACKGROUND: Pemphigus foliaceus(PF) and pemphigus vulgaris(PV) have different target antigens which belong to the desmoglein(DG) subfamily of the desmosomal cadherins; DG in the case of PF and PV antigen(PVA) in the case of PV. OBJECTIVE: Because DG is also a normal major component of the intercellular adhesive core, we investigated the immunohistochemical distribution of DG in PF to compare and contrast the findings with those of PV. METHODS: Immunohistochemical analysis using streptavidin-biotin complex method with anti-DG monoclonal antibody was done in six cases of PF and six cases of PV, as well as two samples of normal skin as control. RESULTS: Both disorders showed abnormal intense diffuse cytoplasmic staining patterns in the lesional skin. Contrary to PF, showing complete loss of normal, rim-like, membranous staining, two of six cases of PV showed the relatively well preserved normal staining pat-terns in the upper epidermis. CONCLUSION: The differences in the expression of DG in the lesional skins between PF and PV suggest that PVA is distinct from DG, although an overlapping of antigens between PF and PV could exist.

5.Significance and diagnostic value of fine calcifications detected by mammography in female breast

In 1951, Leborgen pointed out the special diagnostic importance of punctate calcifications in the diagnosis of breast cancer by mammography. Calcifications within the breast may occure in both malignant and benign diseases, the characteristic findings have been reported subsequently by many other investigators. This study was done toreevalute the significance and diagnostic value of calcifications of female breast cancers. The clinical materals were 980 cases of those underwent mammography, and 76 cases of those histopathologically diagnosis was confirmed. Fine calcifications were radiologically found in 24 cases of these 76. A morphological analysis was made inregards to the relationship between the calcific particles and various mammary diseases. The results were asfollows; 1. The irregularly fine spiculate and punctate calcifications were found in 11 cases (45.8%) of cancergroup, and rather uniformly fine smooth calcifications were found in 14 out of 52 cases (26.9%) of benign conditions. 2. In cancer group, the calcifications were finely punctate or spiculate in shape and in numerable innumber with a tendency toward formation of densely packed clusters within and adjacent to the lesional areas. Nospiculate calcifications were observed in benign conditions. There were, however, no difference in either size ordensity of particles in both benign and malignant conditions. 3. These fine calcifications of the breast detectedby mammography seem to be a useful marker in localizing so small a lesion as not to be palpable and in obtaining proper biopsy specimen.

6.A Case of Kerion Celsi Caused by T . verrucosum in Chungcheong Province.

In Korea, Kerion celsi caused by T. verrucosum was first reported by Kim et al in the Honam province in 1986. Since then three mare cases have been reported in the Young Nam province. Herein present a case of kerion celsi caused by T. verrucosum from Chungcheong province. The patient, a 7-year old female, showed a 4x 4cm sized, oval, yellow crusted and boggy patch with alopecia on the scalp. She was living on a farm in a rural area of Chungcheong province. Cows on the farm also exhibited similar skin lesions on the buttock. The same T. verrucosum was identified by a characteristic colony morphology on Sabourauds dextrose agar and chains of chlamydoconidia with mycologic stain from scales of lesions of patient and cows. This skin lesion was successfully treated with griseofulvin.

7.Papulosquamous Skin Lesions in Systemic Lupus Erythematosus.

We present 6 patients with systemic lupus erythematosus who had papulosquamous skin lesions. These skin lesions were consistent with the findings of subacute cutaenous lupus erythematosus(SCLE) clinically and histopathologically. The. distribution of the skin lesions were on the face, upper extremities and upper trunk in one group, and the upper and lower extremities in other group of these patients. After confirmation of the skin lesions as a type of SCLE, they were regressed with anti-malarial and/or systemic corticosteroids, within 2 months in all patients. For the better management of the patient with lupus erythematosus, understanding of clinically distinct LE-specific skin lesions, including the papulosquamous cuta-neous manifestation of SCLE, may be necessary.

9.Adenoid Basal Cell Tumor of the Prostate: A case report.

Adenoid basal cell tumor of the prostate is a rare tumorous lesion that can be misdiagnosed as adenocarcinoma of the prostate. The malignant potential of adenoid basal cell tumor remains uncertain due to small number of reported cases. This 66-year-old man presented with symptoms of urinary tract obstruction. Under the impression of benign prostatic hyperplasia, a transurethral resection of the prostate (TURP) was performed. The patient was alive with no evidence of recurrence or metastasis 15 months after TURP. Microscopically, most of the lesions were composed of nodular collections of small nests of basaloid cells with peripheral palisading, and clusters of tumor cells forming cribriform pattern. Multiple areas of basal cell hyperplasia and atypical basal cell hyperpalsia were also observed. The coexistence of basal cell hyperplasia, atypical basal cell hyperpalsia, and adenoid basal cell tumor with cribriform pattern in this case supports a morphologic continuum from the benign hyperplastic lesion to malignant neoplasia.

10.Urinary Tract Infection and Vesicoureteral Reflux in Children.

Fine needle aspiration biopsy cytology (FNA) for diagnosis of a variety of breast tum- ors has been proven to be a simple, safe, and cost saving diagnostic methodology with high accuracy. Cytologic specimens from 1,029 fine needle aspirations of the breast during last 3-year period were reviewed and subsequent biopsies from 107 breast lesions were reevaluated for cytohistological correlation. FNA had a sensitivity of 81.6% and a specificity of 98.3%. One out of 107 cases bio- psied revealed a false positive result (0.9%) and the case was due to misinterpretation of apocrine metaplastic cells in necrotic backgound as malignant cells. A false negative rate was 8.4% (9 of 107 cases biopsied). Six of 9 false negative cases were resulted from insufficient aspirates for diagnosis, and remaining three of 9 false negative cases revealed extensive necrosis with no or scanty viable cells on smears. The results indicate that for reducing false positive and false negative rates of FNA, an experienced cytopathologist and a proficient aspirator are of great importance.
