1.Change of causative inhalant allergens in respiratory allergic patients in Chungbuk district.

BACKGROUND: Detection of causative allergens in respiratory allergy patients is important for diagnosis and selection of treatment modality. Allergen sensitization depends on the genetic susceptibility and environmental exposure to allergens. MATERIAL AND METHOD: In this study, we tried to find causative inhalant allergens in respiratory allergy patients who underwent skin prick test with common 55 inhalant allergens and RAST at Chungbuk National University Hospital in Chungbuk district. We also evaluated the changes of causative allergens because the environment here may be quite different, since the main population lives in the urban area and industrialization began later than in the other cities and districts. RESULTS: The major allergen was house dust mite, and its sensitivity rate did not change annually. Other indoor allergens such as cockroach, and animal dander including cat and dog showed similar trends. But the sensitization rate to tree and weed pollen except grass had significantly increased since 1995. Among pollens, the increase of sensntization to weed pollen was more marked. Ratio of sensitization rate to double or triple kinds of allergens also increased. CONCLUSION: Taken together, house dust mite is still the major allergen, but rate to pollen sensitization, especially to weed pollen has increased rapidly and significantly. It is suggested that causative allergens in repiratory allergy patients has begun to shift to pollen recently.

7.Analysis of Angiographic Outcome by Thrombolysis in Myocardial Infarction(TIMI) Frame Count for Primary Stenting in Patients with Acute Myocardial Infarction.

BACKGROUND AND OBJECTIVES: Primary intervention by stent implantation during acute myocardial infarction is a novel strategy to provide better myocardial perfusion compared to thrombolysis or baloon angioplasty. We aimed to assess the reperfusion achieved by primary stenting, employing TIMI frame count for more objective and quantitative measurement. MATERIALS AND METHOD: Measurements for number of frames required to opacify standardized angiographic landmark branch(TIMI frame count) were determined for the coronary arteries of 77 normal controls and 65 patients with acute myocardial infarction who underwent primary stenting within 12 hours of symptom onset. RESULTS: In normal subjects, TIMI frame count for left anterior descending artery(LAD) was 1.3 times of mean count of right coronary artery(RCA) and left circumflex artery(LCx), and significant less than that of TIMI study(22.3+/-4.9 vs 36.2+/-2.6, p<0.05). TIMI frame count for RCA and LCx was similar to results of TIMI study. In infarct related arteries(IRA), corrected TIMI frame count(CTFC) after primary stenting was similar to those of normal control. Frame counts of RCA was larger compared to that of normal control, but statistically insignificant(23.0+/-7.5 vs 17.6+/-3.5, p>0.05). There was no difference of CTFC of non-infarct related arteries between patients and normal controls. CONCLUSION: The TIMI frame count of LAD artery in normal Korean subjects was significantly less than that of American counterpart. In patients with acute myocardial infarction, primary stenting appeared to provide improved coronary flow similar to that observed in normal subjects, as measured by TIMI frame counting.

The tricuspid valve is not a simple but a complex structure, tricuspid valve complex. This complex is composed of tricuspid orifice, annulus, valve leaflet, chordae tendineae and papillary muscles. There are flew articles about these structures. The authors studied tricuspid valve complex in 53 cases of normal korean adults, such as circumference of tricuspid annulus, the maximum diameter of the tricuspid orifice, height and breadth of the cusps, including commissures, the ratio of rough to clear zone, the number of scallops of posterior cusp, the number, length and pattern of arrangement of the several types of chordae, the number and morphological characteristics of papillary muscles.
