3.A Case of Retinal Periarteritis.

One case of retinal periarteritis, probably tuberculous in nature, is presented. The patient reported here is a male, aged 26, who developed retinal periarteritis associated with a large tuberculosis of retina in hes left eye. The arteries had the appearance of being covered with a series of white rings, and the peculiar changes were seen on the superior and inferior nasal artery, while the vein were not involved.

5.Comparative Analysis of Acromial Morphology in Normal and Impingement Syndrome

To identify whether acromial shape, osteophyte, and acromio-humeral interval have effects on impingement syndrome or rotator cuff tear, we reviewed 40 cases of normal group (F:M=22:18), and 30 cases of impingement syndrome(F:M=16:14). Forty cases of normal group aged from 40 to 69 who had no pain, no abrasion sign, no limitation of motion, and normal function of shoulder joint were selected. Thirty cases of impingement syndrome were managed by acromioplasty of direct repair from October, 1993 to May, 1994. Twenty-five cases of 30 were identified rotator cuff tear(RCT), and the others were turned out subacromial abrasion. We reviewed the acromial thickness, the acromial shape, the anterior protuberance, the presence of osteophyte, and the acromio-humeral interval to compare the difference between two groups. Forty-seven point five per cent of normal group had a flat, type I acromion, 47.5% had a curved, type II acromion and 5% were identified by a hooked, type III acromion. However, in subjects with impingement syndrome and RCT, 37% had type I, 20% had type II, and 43% displayed type III. Type III was considerably noticed in the massive tear. In regarding to acromial thickness, normal group had type A(less than 8mm)-37.5%, type B(8-12mm)-62.5%, and the impingement syndrome or RCT group had type A-53%, type B-47%. We couldn’t find any significant difference with each group in type III(more than 8mm)-15% in normal, and type I-17%, type II-33%, type III-50% in the impingement syndrome or RCT. It was suggested that the anterior protuberance was related with the evidence of RCT. A-H interval was 10.25mm±1.46mm in normal, and 9.44mm±1.70mm in the impingement syndrome or RCT. There was on significance in A-H interval except rotator cuff arthropathy. Thirty three percent of normal group had osteophytes and 40% of impingement syndrome or RCT had osteophytes on the undersurface of acromion.

6.Pattern of histologic progression from acute and chronic hepatitis B to cirrhosis.

The chronic active or aggressive hepatitis B, in which periportal inflammation(piecemeal necrosis) is conventionally accepted as the pattern of progression to cirrhosis', implies little concern with intralobular necroinflammation as a process responsible for aggravation and progression. Some published observations, however, refer to episodes of acute necrotizing bouts(subacute hepatic necrosis) have been reported". For technical reasons, relatively few sequential biopsy studies of chronic hepatitis B are available and particularly, initial acute hepatitis B is barely documented since biopsies are now rarely perfomed in acute stages of hepatitis4. Moreover, most published sequential studies are complicated by therapeutic intervention. The availability of Korean sequential biopsy specimens and the increasing interest in parenchymal changes in the evolution of hepatitis B encouraged the description of the progression of acute and chronic hepatitis B to cirrhosis. During the long-term follow-up study, acute bouts of intralobular parenchymal necroinflammation are observed whenever the stage of aggravation is caught and the biopsy is possible. Whatever the result of these studies, the observation presented suggests a major role of parenchymal changes in the evolution of chronic hepatitis B and this experience focuses on the important lesions of the lobular parenchyma in classification and evaluation of chronic hepatitis'. Seven patients are selected on the basis of elevated S-ALT level, and submit to repated needle biopsies during a 6-month period". Of these, 2 cases show chronic active hepatitis with early cirhosis constantly, and in 5 cases CAH appear 1 or 2 times in each case during the follow-up period. In clinical follow-up after the sequential biopsies, these 7 patients were all healthy at least for an average of 18.6 years ranging from 14 to 27 years, except for one who developed into HCC without complication of cirrhosis. Thus, while the sample size of this study is small, continuous piecemeal necrosis seems not to be a pattern of progression to cirrhosis. In conclusion, acute and chronic hepatitis B experienced in Korea appear to progress to cirrhosis by means of repeated acute episodic bouts of intralobular necroinflammation, and the conventional concept that continuous progress by piecemeal necrosis seems not to hold true.

7.A Case of Hidradenitis Suppurativa.

Hidradenitis suppurativa is a rare disease which represents a chronic suppurative and scarring process, affecting the apocrine glands due to persistent bacterial infection and apocrine dysfunction. We have experienced a case of hidradenitis suppurativa, 22-year-old female which involved both axillary region. Diagnosis was established by clinical characteristicsymptoms, histopathological findings and bacteriological examinations.

8.Reversed Tendon Sheath Wrapping on Indirect Rupture of Tendo Calcaneus.

In the operative treatment of an indirect rupture of tendo calcaneus, we should consider difficult repair and postoperative adhesion with surrounding tissues. In this paper, the new surgical technique for indirectly ruptured tendo calcaneus was introduced and evaluated. From July 1994 to Sep. 1995, we experienced 10 cases of closed rupture of Achilles tendon which was treated with reversed tendon sheath wrapping of juncture site after direct repair. This technique consists of conventional core suture with strong suture material followed by fine tide up sutures around the ragged outer margin of the ruptured end. Then, we made a delicate incision on the synovial sheath of the tendon two inch proximal to the tendon juncture site and elevated it carefully and rolled it back reverse direction to wrap around the juncture site, then hooked it up with the synovial sheath of distal part of ruptured tendon. It was very helpful to prevent adhesion by lowering the endotendon cellular ingrowth between tendon proper and tendon sheath by wrapping the ragged end of rupture site. Their least follow-up period was 12 months. The clinical outcome of the patients was assessed according to the criteria of Hooker, and there were eight cases of excellent (80%), and two cases of satisfactory results. We report good results with the reversed tendon sheath wrapping around juncture site after direct repair of Achilles tendon, as a new technique the treatment of indirectly ruptured tendo calcaneus.

9.Arthroscopic Synovectomy in Wrist

The application of arthroscopic surgery in wrist is not widely advocated because of technical difficulty. We analysed 11(10 patients) wrists of which were performed arthroscopic synovectomy in wrist from July 1991 to September 1992 in Kyung Hee University Hospital. The age of the patients ranged from 23 to 55 years old. Follow up period was from 12 months to 26 months. The application for the arthroscopic synovectomy in that group of patients were based on such disabling clinical symptoms as severe pain, swelling, limitation of range of motion and roentgenographic type from mild to severe stage. Postoperative diagnosis was histologically confirmed with rheumatoid arthritis in nine wrists (nine patients) and gout in two wrists (one patient). The results were assessed subjectively and objectively with pain, swelling and range of motion. Satisfactory overall results were obtained and there were no complications. Conclusively, arthroscopic synovectomy in the wrist is effective produce to relieve pain of the rheumatoid arthritis and gout but arthroscopic synovectomy of wrist is technically demanded.

10.Statistical Survey on the Amputees

We have made a statistical survey on 2441 amputated limbs of 2150 patients who were treated at the Depart- ment of Orthopedic Surgery and who were prescribed the prosthetics at the Prosthetic Center of Veterans Hospital from Mar. 1972 to Feb. 1982. 1. The peak incidence was in the age group from 21 to 30 years with 1546 cases (63.3%). Of 2150 patients only 9 patients were female. 2. The main causes of amputation were trauma with 1901 cases (77.9%), vascular insufficiency with 326 cases (13.4%) and chronic osteomyelitis with 157 cases (6.4%). And the most fre#quent cause of traumatic amputation was explosive injury with 1080 cases (56.8%). 3. Of the 2441 amputated limbs, 1756 cases (71.91°) were at the lower extremity and the most frequent site was below the knee with 1000 cases (57%). 4. Minor limb amputation was performed on 326 cases (13.4%). 5. Multiple limb amputation was performed on 255 patients (11.9%) and of these 12 patients were amputated on 4 extremities. 6. Reoperation was performed on 420 cases (17.2%). It was most prevalent in the lower extremity with 365 cases (86.9%) and their main cause was neuroma with 171 cases (40.7%). The most frequent site was below the knee with 240 cases (57.2%).
